Cardiogenic shock (CS) remains a high morbidity and mortality condition worldwide frequently complicating acute myocardial infarction (AMI) and decompensated heart failure (HF). Within the management of CS, mechanical circulatory support (MCS) devices play a critical role in maintaining hemodynamic stability, preserving end-organ perfusion and bridging patients through to recovery, implantation of durable support or transplantation. Despite their use, optimal timing of initiation, as well as patient and device selection remain unclear. This review explores the current landscape of MCS devices,

BACKGROUND: We sought to describe trends and outcomes of mechanical circulatory support (MCS) use in acute myocardial infarction and cardiogenic shock (AMI-CS) patients undergoing coronary artery bypass grafting (CABG).
METHODS: We identified all patients with AMI-CS undergoing CABG in The Society of Thoracic Surgeons Adult Cardiac Surgery Database from 2011 to 2022. Preoperative, intraoperative, and postoperative use of intra-aortic balloon pump (IABP), catheter-based temporary MCS (tMCS), and extracorporeal membrane oxygenation (ECMO) were examined using multivariable regression and trends

BACKGROUND: Ventricular septal perforation (VSP) after acute myocardial infarction (AMI) is accompanied by the worsening of rapid hemodynamics, resulting in a poor prognosis. In our department, infarct lesions are preoperatively detected with electrocardiogram (ECG)-synchronized contrast computed tomography, and the scope of approach and exclusion is determined. Furthermore, to effectively prevent a residual shunt, modified double patch repair and infarct exclusion techniques were used in combination to preserve left ventricular (LV) function. This method is reported because it considers both

BACKGROUND: Emergent percutaneous cardiopulmonary support (PCPS) has the potential to rescue patients in cardiac arrest who might otherwise die. We retrospectively reviewed the results of PCPS using preassembled, heparin-coated, and autopriming devices in patients in cardiac arrest.
METHODS: From November 2003 to July 2005, 22 patients in cardiac arrest underwent PCPS using the Capiox emergent bypass system (Terumo, Tokyo, Japan). The mean +/- SD age was 63 +/- 14 (range, 31 to 85) years. In six patients, the underlying disease causing cardiac arrest was not diagnosed before PCPS. The procedu

Cardiac failure remains a life-threatening complication for certain patients undergoing intracardiac repair. Despite improvements in surgical techniques, methods of myocardial protection, and postoperative care, patients are frequently at risk to develop postoperative low output syndrome. Approximately 1% of cardiac surgical patients cannot be weaned from extracorporeal circulation in spite of adequate volume loading, the use of inotropic support, and initiation of intraaortic balloon pumping. In these cases, ventricular assist devices (VAD) can mechanically aid the failing heart and reverse t

From 1979 through 1986, 90 patients required intraaortic balloon pumping (IABP) due to low cardiac output complicating open heart surgery. Preoperatively, functional class III or IV was present in 92%; in patients with coronary artery disease (n = 54), three-vessel or left main disease was found in 72%; patients with valvular disease (n = 37) generally had multiple surgical procedures or valve re-replacement performed. Intra-IABP mortality was 49% (n = 44) and 30-day mortality 61% (n = 55). The latter figure decreased from 75% in 1979-81 to 53% in 1985-86. Cumulative 5-year survival was 22%. L

Inevitably, a patient in shock will present to your office. The findings may be obvious, or they may show the more subtle changes of mild tachypnea, tachycardia, and/or changes in mental status. In either event, the perfusion pressure either has already decompensated or will do so momentarily. Whether you initiate therapy then and there might well determine whether your patient will survive. Accordingly, each office should have available for the pre-hospital management of shock those items listed in Table 3. As clinicians, you must be prepared to begin treatment in your office. Although the ho

BACKGROUND: Bilateral cardiac sympathetic denervation (CSD) performed via video-assisted thoracoscopic (VAT) surgery shows potential in managing ventricular tachycardia (VT), thereby reducing arrhythmic burden. In this setting, the scarcity of studies addressing both perioperative and long-term outcomes creates a substantial gap in the optimal management of patients with multiple comorbidities and limited treatment options. This observational study aimed to assess the medical comorbidities, as well as the short- and long-term outcomes of patients who underwent CSD for VT refractory to catheter

OBJECTIVES: Because of its reduced invasiveness, MitraClip (Abbott Vascular, Menlo Park, CA) therapy usually is reserved for patients with extreme left ventricular dysfunction or severe comorbidity contraindicating surgery. The appropriate post-procedural care in this high-risk population is yet to be defined. In this study, the postoperative course of such patients is reported, focusing on early complications and need for intensive care unit (ICU) management.
DESIGN, SETTING, AND PARTICIPANTS: A retrospective analysis of patients with severe mitral regurgitation undergoing transcatheter mitr

BACKGROUND Acute renal failure (ARF) is a critical complication following open-heart surgery, significantly impacting morbidity and mortality. This study aimed to evaluate the association between intraoperative renal near-infrared spectroscopy (NIRS) findings and postoperative ARF in 357 patients undergoing cardiac surgery with cardiopulmonary bypass (CPB). MATERIAL AND METHODS This prospective study included 357 patients undergoing open-heart surgery with CPB. ARF diagnosis was based on KDIGO criteria. NIRS sensors were placed bilaterally at the T12/L2 level under ultrasound guidance, and ren

Background: Patients in cardiogenic shock (CS) often require prolonged mechanical circulatory support lasting longer than 14 days. Prolonged support with Impella 5.5 can improve outcomes in these patients. Here, we describe our experience with prolonged Impella 5.5 support. Methods: From January 2023 to June 2024, 64 patients receiving Impella 5.5 support for more than 14 days were identified. Information including demographics, heart failure etiology, and hospital course were collected. Results: Of the 64 patients identified, 54 were male, with an average age of 55.9 years. One patient was cl

OBJECTIVES: To determine the perioperative predictors of long-term mortality after aortic valve replacement (AVR). The authors hypothesized that perioperative variables are more important predictors of mortality than patient-prosthesis mismatch (PPM).
DESIGN: A retrospective study of prospectively collected data.
SETTING: A tertiary care university hospital.
PARTICIPANTS: One-hundred-ninety-nine adult patients who underwent AVR.
INTERVENTIONS: After Research and Ethics Committee approval, the authors studied consecutive adult patients that underwent AVR in 1999 from the time of procedure t

Intraaortic balloon pump (IABP) is being used in cardiac surgery in an increased ratio. IABP therapy involves considerable risk, mainly vascular complications, postoperative bleeding and infection can represent danger. Between 1999 and 2004 out of 4443 open heart surgery operations we have performed intraaortic balloon pump treatment in case of 75 patients. The mean age was 64 years, 23 patients had diabetes mellitus, 47 patients had hypertension, 20 patients had peripheral vascular disease as well. We performed IABP therapy most frequently during isolated coronary bypass operations (42 cases)

OBJECTIVE: Unstable angina (UA) is characterized by a state of coronary artery vascular inflammation and endothelial dysfunction. Statins mitigate inflammation and endothelial dysfunction and decrease mortality associated with percutaneous interventions for UA. We determined whether preoperative statin use is associated with decreased mortality and morbidity following coronary artery bypass+/-valve surgery for UA.
METHODS: Patients with CCS Class IV angina having CABG+/-valve surgery were identified (n=1706). A logistic regression model determined the association of preoperative statin use wi

BACKGROUND: Standardized hemodynamic-guided weaning criteria are needed for patients with cardiogenic shock receiving temporary mechanical circulatory support (tMCS). We aimed to determine whether hemodynamic indices prior to tMCS explant predict mortality and native heart survival and whether performance varies across device platforms.
METHODS: We analyzed 385 cardiogenic shock patients from the multicenter Cardiogenic Shock Working Group registry who received tMCS. Four indices were evaluated: blood pressure response index (BPRI), organ perfusion pressure (OPP), aortic pulsatility index (AP

BACKGROUND: Subclinical hypothyroidism (SCH) is associated with major adverse cardiovascular events. Despite the recognized negative impact of SCH on cardiovascular health, research on cardiac postoperative outcomes with SCH has yielded conflicting results, and patients are not currently treated for SCH before cardiac surgery procedures.
METHODS: We performed a study-level meta-analysis on the impact of SCH on patients undergoing nonurgent cardiac surgery, including coronary artery bypass grafting and valve and aortic surgery. The primary outcome was operative mortality. Secondary outcomes we
