Anatomic Investigation of Commonly Used Landmarks for Evaluating Rotation During Forearm Fracture Reduction.

BACKGROUND: Evaluating rotation during open reduction and internal fixation of a forearm fracture is of paramount importance. In challenging cases, surgeons may utilize radiographic relationships between proximal and distal radial and ulnar landmarks to assess rotational position. However, to our knowledge, limited anatomic data are available to confirm these relationships. METHODS: Six hundred cadaveric radii and ulnae were obtained. Digital representations of the bicipital tuberosity, the radial styloid, the coronoid process, and the ulnar styloid were acquired, and the rotational profiles

Transient hemiplegia in posterior instrumentation of scoliosis.

STUDY DESIGN: A case of transient hemiplegia during posterior correction and instrumentation of scoliosis in an 18-year-old woman. OBJECTIVE: To present a case of transient hemiplegia most probably resulting from an arteriovenous fistula. SUMMARY OF BACKGROUND DATA: Neurologic impairment in spinal surgery is a feared complication. Common reasons are direct or indirect trauma to neural elements, intraoperative hypotension, ischemia, bleeding, metabolic dysbalances, or drug effects. Review of the literature did not reveal any case of transient hemiplegia similar to the presented one in which n

[Open surgical therapy of the rotator cuff].

Almost 100 years have passed since the first report of rotator cuff repair in 1898 by W. Müller. Even now new operative techniques are being developed. Various solutions for difficult extended forms of rotator cuff lesions are available besides the closed and semi-closed arthroscopic techniques. Anatomical reconstructive procedures such as local or distant tendon transfer compete with extra-anatomical procedures such as simple debridement, equatorial partial reconstruction or replacement with a delta flap. Hemiarthroplasty of the shoulder has proven to be more and more the treatment of choice

[Primary aorto-enteric communication].

BACKGROUND: Primary aortoenteric communications are a rare and severe complication of abdominal aortic aneurysms or erosions by neoplastic diseases. Early diagnosis and surgical treatment are crucial. Postoperative morbidity and mortality are high. Diagnosis if often made intraoperatively, because the typical clinical feature (digestive haemorrhage, abdominal aneurysmal mass, abdominal pain) is often incomplete and critically ill patients require quick surgical decision and do not allow the use of sophisticated diagnostic tools. MATERIAL AND METHODS: Eight cases, observed through 13 years, ar

Perforator-Aware Explainable Microsurgical Intelligence for Basilar Trunk Aneurysm Reconstruction: A Human-Centred Clinical AI Framework for Precision Neurovascular Surgery-A Retrospective Single-Centre Study.

Basilar trunk aneurysms (BTAs) are rare intracranial aneurysms. These aneurysms pose substantial clinical risk because they are located close to vital cranial structures and perfusion-sensitive perforating arteries. In addition, outcomes from BTA repair are dependent upon several factors, which include proximity to the brainstem, preservation of perforators, corridor access to the aneurysm, successful clipping of the aneurysm through reconstruction, the need for intraoperative rescue manoeuvres and the ability of the patient to recover from complications. Many previous studies have documented

Late-presenting dural tear: incidence, risk factors, and associated complications.

BACKGROUND CONTEXT: Unrecognized and inadequately repaired intraoperative durotomies may lead to cerebrospinal fluid leak, pseudomeningocele, and other complications. Few studies have investigated durotomy that is unrecognized intraoperatively and requires additional postoperative management (hereafter, late-presenting dural tear [LPDT]), although estimates of LPDT range from 0.6 to 8.3 per 1,000 spinal surgeries. These single-center studies are based on relatively small sample sizes for an event of this rarity, all with <10 patients experiencing LPDT. PURPOSE: This investigation is the large

Morphological Variations of the Human Liver: A Cadaveric Study With Clinical Correlations in a South Indian Population.

Introduction The liver is the largest gland and one of the most vital organs in the human body, performing essential metabolic, detoxification, and synthetic functions. Although the general anatomy of the liver is well described, morphological variations of its lobes and surfaces are frequently encountered. These variations may include accessory lobes, accessory fissures, diaphragmatic grooves, and unusual shapes of hepatic lobes. Such anatomical variations may mimic pathological lesions on imaging studies or complicate hepatobiliary surgical procedures. Therefore, understanding these variatio

Congenital Agenesis of the Obturator Foramen in an Asian Population and Its Clinical Significance.

BACKGROUND: The obturator foramen, an ovoid opening in the hip bone formed by the ischium and pubis, allows passage of the obturator artery, vein, and nerve through the obturator canal. Mostly covered by the obturator membrane, its congenital agenesis or partial underdevelopment is extremely rare and scarcely documented, yet it carries significant implications for pelvic surgery, orthopedic, vascular procedures, and radiological interpretation. OBJECTIVE: This study aimed to determine the prevalence, morphological characteristics, and clinical relevance of congenital agenesis of the obturator

The negative impact of pleural adhesion on pulmonary function: a retrospective study.

BACKGROUND: Pleural adhesion refers to fibrotic or proliferative changes resulting in lesions between the visceral and parietal pleura, which can be caused by various factors. It is common and often asymptomatic, but may lead to impaired pulmonary function, especially in cases of severe or extensive adhesion. Existing studies mainly focus on pleural thickening caused by conditions such as tuberculous pleuritis. However, research on mild or localized pleural adhesions and their impact on preoperative pulmonary function remains limited. Therefore, this study aims to evaluate the specific effects

Neonatal unilateral testicular torsion in the first 16 hours of life: A case report and literature review.

RATIONALE: Neonatal testicular torsion (NTT) is an uncommon but urgent urological condition that predominantly occurs in the prenatal or immediate postnatal period (up to 28 days of life). Diagnosis is often delayed due to subtle clinical signs and the inability of neonates to express discomfort. Early recognition is critical to prevent irreversible ischemic damage and to preserve future endocrine and reproductive function. This case emphasizes the importance of routine detailed neonatal physical examination, even when infants are hospitalized for unrelated conditions or otherwise normal. PAT

The "double-intra" pertrochanteric femur fracture: an irreducible and redisplaceable fracture pattern.

BACKGROUND: Irreducible pertrochanteric femur fractures (PFFs) are associated with a higher risk of mechanical instability and poor clinical outcomes. Awareness of the specific patterns of these irreducible PFFs can help surgeons better prepare for fracture reduction and reduce unnecessary attempts at closed manipulations. This article aimed to introduce a specific subtype of irreducible PFFs, characterized by intramedullary head-neck varus impaction and the anterior fracture line located intracapsularly, termed "double-intra" PFFs. METHODS: Patients with "double-intra" PFFs treated with ceph

Pulling the uterus directly instead of relying on the uterine manipulator: a better surgical technique choice for laparoscopic radical resection of cervical cancer.

Cervical cancer is one of the most common malignant tumors among women. The appropriate treatment plan is determined based on factors such as the clinical stage, age of the patient, fertility requirements, general condition and medical expertise. Patients are typically treated with surgery or radiotherapy, along with chemotherapy if necessary. Surgical treatment is typically reserved for patients with early-stage cervical cancer (IA-IIA). In recent years, advancements in screening technology and increased individuals' awareness of physical examinations have led to a higher rate of early diagno

Navigating the Nexus: A Rare Case of Choledochal Cyst With Concomitant Pancreas Divisum.

Choledochal cysts (CCs) are quite rare and are characterized by anomalous dilations of the biliary tree, mostly due to anomalous pancreaticobiliary junction (APBJ). A less frequent congenital anomaly due to incomplete fusion of pancreatic ducts, pancreas divisum (PD) can complicate the clinical course of CC. Although rare, the coexistence of CC and PD presents significant clinical challenges. With very few documented cases globally, our experience with this case adds to our understanding of this unique condition. This report aims to highlight the complex relationship between these anomalies an

A case series on parathyroid carcinoma: Diagnostic challenges and therapeutic approaches.

INTRODUCTION: Parathyroid carcinoma is an exceptionally rare endocrine malignancy, constituting <1 % of primary hyperparathyroidism (PHPT) cases. It presents with more severe hypercalcemia and higher PTH levels than benign parathyroid diseases, requiring increased clinical awareness for accurate identification and specialized management. The results of this case series may provide insight into the clinical presentation, diagnostic workup, surgical management, and prognosis of parathyroid carcinoma, supplemented by a comprehensive review of current diagnostic and therapeutic approaches. Performance of the SEDLine Monitor: Age Dependency and Time Delay.

BACKGROUND: Devices monitoring the hypnotic component of general anesthesia can help to guide anesthetic management. The main purposes of these devices are the titration of anesthesia dose. While anesthesia at low doses can result in awareness with intraoperative memory formation, excessive administration of anesthetics may be associated with an increased risk of postoperative neurocognitive disorder. We have previously shown for various indices that they are significantly influenced by the patient's age and that the monitors have a significant time delay. Here, we evaluated the influence of p

Robotic- and orthosensor-assisted versus manual (ROAM) total knee replacement: a study protocol for a randomised controlled trial.

BACKGROUND: Robotic-arm-assisted knee arthroplasty allows for more accurate component positioning and alignment and is associated with better patient-reported outcomes compared to manually performed jig-based knee arthroplasty. However, what is not known is whether the addition of an intra-articular sensor (VerasenseTM) to aid intraoperative balancing of the total knee replacement (TKR) offers improved functional outcomes for the patient. The purpose of this research is to compare the outcomes of patients undergoing a conventional manual knee replacement to those undergoing TKR using robotic-a

Safety and efficacy of perioperative benzodiazepine administration: study protocol for a systematic review and meta-analysis.

INTRODUCTION: Perioperative benzodiazepines are used because of their anxiolytic, sedative and amnestic effects. Evidence has demonstrated an association of benzodiazepines with adverse neuropsychiatric effects. Nonetheless, because of their potential benefits, perioperative benzodiazepines continue to be used routinely. We seek to evaluate the body of evidence of the risks and benefits of benzodiazepine use during the perioperative period. METHODS AND ANALYSIS: We will search Cochrane CENTRAL, MEDLINE, EMBASE, PsychINFO, CINAHL and Web of Science from inception to March 2019 for randomised c

A Contemporary Medicolegal Analysis of Implanted Devices for Chronic Pain Management.

BACKGROUND: Analysis of closed malpractice claims allows the study of rare but serious complications and likely results in improved patient safety by raising awareness of such complications. Clinical studies and closed claims analyses have previously reported on the common complications associated with intrathecal drug delivery systems (IDDS) and spinal cord stimulators (SCS). This study provides a contemporary analysis of claims from within the past 10 years. METHODS: We performed a closed claims analysis for a period January 1, 2009 to December 31, 2013 for cases with pain medicine as the p
