Rhinophyma is the end stage of acne rosacea, which results in a large nose due to a proliferation of sebaceous glands and fibrous tissue. In some patients, this disorder is a cosmetic problem. Use of the CO2 laser in the defocused mode to vaporize skin in a bloodless fashion is an effective treatment of rhinophyma, as demonstrated in three patients described in this report.

A 21-year-old man received an accidental laser injury to three separate foci of the retina of his right eye from a Q-switched Nd:YAG laser target designator emitting at 1,064 nm, operating at 10 Hz, with a pulse duration of 20 ns. The nominal output of this device was 50 mJ, and the diameter of the output beam was approximately 4 cm. Besides surgical intervention, there are virtually no palliative treatment modalities available for bleeding and/or obstructing colorectal malignancy. The usefulness and safety of laser photocoagulation was prospectively investigated in 63 patients with colorectal cancer. A series of pigmented rabbit eyes were treated with pulsed 10 msec (free running mode) neodymium:YAG laser burns to achieve transscleral cyclocoagulation. Grossly evident burns were created in the ciliary body with laser pulses of 1.5 joules of energy. Side effects that have occurred in over 250 endobronchial laser treatments are reported. The laser has been used in severe tracheal and in bronchial obstructions. A number of ocular complications are seen to be associated with the use of Nd:YAG laser for posterior capsulotomy after cataract surgery. We have outlined possible hazards to the cornea, trabecular meshwork, vitreous and intraocular lenses and have discussed their consequences. It would appear that the degree of severity of complications is related to the pulse energy used upon the capsule.

Since 1980, we have been using CO2 laser in treating carcinomas of the tongue. Carcinomas confined to the mucosa have been treated with laser excision alone. Five patients were treated in this way and they are all alive with no evidence of disease. Follow-up ranges from 11 months to 3 years. Carcinomas with a superficial muscular invasion have been treated with mass reduction laser surgery followed by chemoradiotherapy. There were 19 patients which fell in this category. Of these 19, 17 are alive with no evidence of disease, 1 died of an intercurrent disease, and 1 died of cervical metastasis.

Xanthelasma of the eyelids traditionally has been treated by excision, followed by suturing of the defect. A new technique of ablating these lesions using a superpulsed CO2 laser was used to treat a series of nine patients with xanthelasma. Advantages of this technique include: (1) superb hemostasis intraoperatively; (2) magnified view of tissue removal, enhancing complete excision; (3) the absence of suturing; (4) the avoidance of reconstructive procedures; and (5) a high degree of patient satisfaction, with minimal or no postoperative pain.

A one-stage sclerostomy procedure was performed "noninvasively" in four cynomolgus monkeys solely with the neodymium-YAG laser.
