Right posterior paratracheal lymph nodes metastasis is one of the predictive factors in right-sided papillary thyroid carcinoma.

BACKGROUND: Lymph nodes in the right paratracheal region are separated as anterior and posterior on the basis with right recurrent laryngeal nerve. Dissection of the right posterior paratracheal lymph nodes is sometimes overlooked during a central neck dissection. Therefore, this study was designed to assess the clinicopathologic risk factors and prognostic implication for recurrence related to the presence of right posterior paratracheal lymph nodes metastasis in patient with right-sided papillary thyroid carcinoma. METHODS: Records from 763 patients with papillary thyroid carcinoma who unde…

Central neck lymph node removal during minimally invasive video-assisted thyroidectomy for thyroid carcinoma: a feasible and safe procedure.

BACKGROUND AND PURPOSE: In 1998, we developed a technique for video-assisted thyroidectomy (VAT) which we proposed using also in patients with small low-risk papillary thyroid carcinomas (PTC). In some cases, enlarged lymph nodes are incidentally found at surgery for PTC. These nodes should be removed because of the risk of metastases. In this paper, we report on the patients in whom we removed enlarged central neck lymph nodes during VAT for PTC and discuss the feasibility and safety of video-assisted central neck lymph node dissection (VALD). PATIENTS AND METHODS: The procedure is performed

Significance of retropharyngeal lymph node dissection in hypopharyngeal cancer.

OBJECTIVE: We investigated the risk factors for metastasis to retropharyngeal lymph nodes (RPLNs) and the significance of dissection of RPLNs in hypopharyngeal cancer. Metastasis to the RPLNs is an important prognostic factor in head and neck cancer, especially in hypopharyngeal cancer. METHODS: Study subjects were 129 cases who received primary treatment at nine leading medical facilities in the field of head and neck cancer management in Japan. Focusing on RPLNs, we compared prognosis in RPLN-metastasis-positive, RPLN-metastasis-negative, RPLN-dissected and RPLN-non-dissected cases. RESULT

Accessory Nerve Anatomy in Anterior and Posterior Cervical Triangle: A Fresh Cadaveric Study.

OBJECTIVE: To understand the variations and normal course of the accessory nerve (CNXI) to help more accurate and confident neck dissection. METHODS: The course of the CNXI in the neck, its relationship to the surrounding anatomic structures and the factors affecting its course were investigated. RESULTS: A total of 100 neck dissections were performed on 50 fresh cadavers. Eleven division variations were observed at the anterior triangle. The location of CNXI at the posterior border of the sternocleidomastoid muscle (PBSCM) was investigated and the ratio between the distance from the mastoid…

Bifrontal interhemispheric approach for carotid-ophthalmic aneurysms.

The authors report their experience with the surgical treatment of carotid-ophthalmic aneurysms in 29 cases, and describe their surgical technique. The technique can be summarized as follows. When dissecting the aneurysm, temporary vascular occlusion of the common carotid artery and external carotid artery is done in the neck under the administration of cerebral protective substances. Through a bifrontal craniotomy, wide dissection of the Sylvian fissures and the interhemispheric fissure is performed. When necessary, the anterior clinoid process and the roof of the optic canal are removed. Thi

Carotid artery dissection after the intracarotid amobarbital test.

A retrospective chart review of 435 consecutive intracarotid amobarbital tests (IAT) was performed to determine the frequency of carotid artery dissection (CAD) associated with IAT. Three patients with a CAD were found (0.7%). Mean age of patients with dissection (51.3 years) was higher than the average age of 432 patients without dissection (31.7 years) (p < 0.05). All patients had clinical symptoms including face or neck pain. Patients undergoing the IAT are at risk of CAD. Age may be a risk factor.

Facial numbness and dysesthesia. New features of carotid artery dissection.

Facial numbness and dysesthesia have not been emphasized as presenting features in spontaneous internal carotid artery dissection. Progressive facial pain, accompanied by oculosympathetic paresis, altered taste, and facial numbness suggest the possibility of basal skull neoplasm. We describe a patient, with previously undiscovered fibromuscular dysplasia, who presented with severe neck and face pain, dysgeusia, oculosympathetic paresis, and markedly reduced facial sensation due to a spontaneous vascular dissection. Altered facial sensation should now be included in the symptomatology of intern

When initial postexcision PTH level does not fall appropriately during parathyroidectomy: what to do next?

BACKGROUND: Inadequate fall in the intraoperative parathyroid hormone (PTH) level after removing enlarged parathyroid gland(s) typically signifies additional hyperfunctioning gland(s), prompting further neck dissection, but it may also be a false negative result. We analyzed intraoperative management of patients with an inadequate fall on PTH after excision of enlarged parathyroid gland(s). METHODS: Analysis involved a prospective database of 189 patients undergoing 193 procedures for primary hyperparathyroidism. The PTH level was determined before neck incision and 10-15 min after excision o…

Surgical approach and outcomes for revision surgery of the central neck compartment.

Revision surgery of the central neck compartment is still a controversial subject, and data are scarce in the literature regarding surgical approaches and outcomes. This might be a result of the small number of patients in need of revision of the central neck compartment. Therefore, the purpose of this study was to document the approach and outcomes for revision surgery of the central neck compartment performed in our clinic. The files of patients who had undergone revision surgery of the central neck compartment in the Clinic of Otorhinolaryngology, Ankara Numune Training and Research Hospita

Minimally invasive thyroid and parathyroid surgery is not a question of length of the incision.

INTRODUCTION: Postoperative cosmesis in the neck is often a major concern of patients, particularly women, undergoing thyroid or parathyroid surgery. Therefore, a reduction in the length of the cervical incision, and even more so, having no scar in the neck, is particularly appealing to these patients. Over the last years, many different so-called minimally invasive procedures have been proposed for the treatment of thyroid and parathyroid diseases, the primary aim being to improve the cosmetic results. Nevertheless, the concept of surgical invasiveness cannot be limited to the length or to th

Do not forget the contralateral side: late onset bilateral carotid dissecting aneurysms.

Internal carotid artery dissection may occur spontaneously or following a blunt or penetrating trauma to the head and/or neck. Bilateral dissections and pseudoaneurysm formations are extremely rare and usually detected simultaneously. In this report, we present a trauma case with bilateral dissecting pseudoaneurysms of the carotid arteries; in whom, the one side was initially occult and then developed during the follow-up of the contralateral side.

Primary thyroid squamous cell carcinoma presenting as a left-sided neck lump.

The elderly patient presenting with a neck lump often raises concerns regarding a malignancy. Thyroid gland malignancies are well recognised and subtype characteristics thoroughly researched, whereas rarer types of thyroid carcinoma are reported infrequently and often behave more aggressively. An 83-year-old woman was referred from the general practitioner (GP) to otolaryngology due to a 7-month history of an unexplained enlarging left-sided neck swelling. A fine-needle aspiration revealed cytology consistent with squamous cell carcinoma (SCC). Staging imaging failed to reveal evidence of a pr

Carotid artery dissection after treadmill running.

The case is presented of carotid artery dissection causing Horner's syndrome in a 25 year old woman after treadmill running. She presented with characteristic eye signs and mild pain. Early diagnosis and treatment was associated with no long term, serious complications. It is now recognised that carotid artery dissection is more common than previously thought. Better imaging of the carotid artery has facilitated the diagnosis, and it is now essential to rule out this potentially fatal condition in patients presenting with postganglionic Horner's syndrome. It is accepted that relatively trivial

Surgery of the thyroid gland.

Total thyroid lobectomy is feasible with minimal morbidity utilizing a surgical procedure that commences at the inferior pole of the thyroid gland to identify the recurrent laryngeal nerve as the initial step. The superior pole vessels are carefully ligated as the last step, care being taken to avoid the external branch of the superior laryngeal nerve. The parathyroid glands can be easily identified and preserved, and if necessary they can be reimplanted in thin sections in either a strap muscle, sternocleidomastoid muscle, skin incision, or skin of the forearm. Muscle implantation is not perf

[Carcinoma metastases in neck lymph nodes in occult primary tumor: survival rate depending from localization, stadium, histology and therapeutic method (48 patients)].

In patients with metastatic cancer in cervical nodes without evidence of a primary cancer, radiation therapy eventually supplemented by surgery is indicated. The treated volume should include the regions of the possible primary (pharynx, base of tongue, tonsil) besides the cervical node regions. Five-year cure rates are: about 40% in cases with cervical nodes only, more than 50% in cases with cervical nodes in one region only and not fixed, less than 10% in cases with supraclavicular nodes. In about 30% a primary tumor is found later. About 50% of these primaries is located in the supraclavicu
