Demyelinating polyneuropathy with preferentially-proximal involvement.

A 47-year-old man showed progressive, symmetrical weakness in the limbs for 6 months. There was muscle atrophy, fasciculations, and acute denervation without motor conduction abnormalities below the elbows or knees, and motor neuron disease had once been suspected. However, compound muscle action potentials (CMAPs) after proximal stimulation showed an amplitude reduction between axilla and Erb's point for the median and ulnar nerves on both sides. His weakness as well as the amplitude reduction improved after administration of prednisolone. Demyelinative conduction abnormalities can be limited

Transplanted olfactory ensheathing cells remyelinate and enhance axonal conduction in the demyelinated dorsal columns of the rat spinal cord.

Olfactory ensheathing cells (OECs), which have properties of both astrocytes and Schwann cells, can remyelinate axons with a Schwann cell-like pattern of myelin. In this study the pattern and extent of remyelination and the electrophysiological properties of dorsal column axons were characterized after transplantation of OECs into a demyelinated rat spinal cord lesion. Dorsal columns of adult rat spinal cords were demyelinated by x-ray irradiation and focal injections of ethidium bromide. Cell suspensions of acutely dissociated OECs from neonatal rats were injected into the lesion 6 d after x-

Effects of 2.5 s mouse nerve growth factor on regeneration of injured sciatic nerves in mice and rats.

AIM: To study the promoting activity of 2.5 s murine nerve growth factor (NGF) for the regeneration of injuried sciatic nerve. METHODS: The sciatic nerves of mice and rats were crushed. The axon counts and differential axon counts were made. Evoked potential latency (NMEPL) was recorded at sites of proximal (Pro) and distal (Dis) segment of soleus muscles. RESULTS: In mice, NGF im 0.5-1 kBU.kg-1 increased the axon counts of regenerated nerves 20 d after crush; 2-4 kBU.kg-1 simultaneously alleviated the denervated atrophy of soleus muscles. In rats, NGF i.m. increased the axon counts at dose

Anterior intramuscular transposition of the ulnar nerve for cubital tunnel syndrome.

Forty-five sequential cubital tunnel releases with anterior transposition of the ulnar nerve in an intramuscular fashion were performed over a 4-year period. All patients had a positive Tinel's sign at the cubital tunnel and reported numbness and tingling in the ring and small finger of the affected arm. Thirty-three cases had preoperative electrodiagnostic studies performed. Twenty-three cases had positive electromyographic and nerve conduction velocity findings for cubital tunnel syndrome, whereas 10 cases had normal electromyographic and nerve conduction velocity studies. Twenty-four cases

Carpal tunnel syndrome: neurophysiological results of surgery based on preoperative electrodiagnostic testing.

Fifty-three hands with carpal tunnel syndrome had pre- and postoperative evaluations of median nerve distal motor latency (from wrist to thenar muscles) and orthodromic sensory nerve conduction velocity (from thumb and middle finger to wrist). At 6 months we observed a neurophysiological return to normal in all cases with normal preoperative distal motor latency and in about 50% of the hands with preoperative distal motor latency between 4 and 6 ms. Prolongation of the distal motor latency over 6 ms was not followed by return to neurophysiological normality, although some degree of sensory fun

Neuropathic symptoms and musculoskeletal pain in carpal tunnel syndrome: prognostic and therapeutic implications.

BACKGROUND: Repetitive activities in the workplace can not only jeopardize the median nerve across the carpal tunnel but also the musculoskeletal structures, such as the ligaments, synovia, tendons and muscles producing pain, and local tenderness at the wrist, elbow, and shoulder. The occurrence of this latter condition can dominate the overall clinical picture and affect the outcome of treatments for this very common condition. METHODS: A clinical evaluation of 30 patients who previously underwent a successful and uncomplicated carpal tunnel release surgery was conducted. These patients were

An intracellular study of space and time representation in primary visual cortical receptive fields.

In contrast with previous knowledge based on extracellular recordings, the recent development of intracellular techniques in vivo (sharp electrode or 'blind patch') ideally allows experimenters to analyze and dissect the contribution of feedforward and lateral connectivity in the functional expression of a synaptic 'integration field'. We will present recent data which demonstrate that the visual receptive field of cortical neurons described at the level of subthreshold synaptic events extends over much larger regions of the visual field than previously thought, and that the capacity of cells

Nerve conduction velocity in human limbs with late sequelae after local cold injury.

Cold-induced neuropathy may play a dominant role in the long-term sequelae with cold sensitivity after local cold injuries (LCIs). Somatosensory functions were assessed and nerve conduction velocity (NCV) and motor distal delay (MDD) were measured in the limbs of 31 Norwegian former soldiers with persistent cold intolerance 3-4 years after the primary LCI. NCV measurements were performed in 24 lower and 16 upper extremities. NCV was related to degree of overall subjective complaints quantified by means of a visual analogue scale (VAS). Motor (MNCV) and sensory conduction velocity (SNCV) in the

AAEM minimonograph #44: diseases associated with excess motor unit activity.

Stiff-man syndrome is due to hyperexcitability of anterior horn cells, possibly related to interference with the synthesis or action of gamma-aminobutyric acid. Unexpected acoustic and exteroceptive stimuli produce exaggerated muscle responses. Needle electrode examination of involved muscles yields nonspecific findings and demonstrates involuntary motor unit activity. The appearance and firing pattern of motor units are normal except that agonist and antagonist muscles may contract concurrently. Continuous muscle fiber activity (Isaacs' syndrome) comprises a heterogeneous group of hereditary

Nerve palsy during femoral lengthening: MRI, electrical, and histologic findings in the central and peripheral nervous systems--a canine model.

Six dog's femurs were lengthened until complete peroneal palsy was present. Ten-24 days later MRIs of the spinal cords were normal. EMGs located lesions at the spinal foramina in five dogs and at the lumbar plexus in one. In all dogs nerve conduction below the lesions was normal and amplitudes were diminished. Muscle histology confirmed neurogenic atrophy. Nerve histology at the sites of the lesions showed myelin and axon changes, but there was no Wallerian degeneration at or distal to those sites. Thus, the nerve lesions were either a variant of Sunderland first degree injury or were developi

Trembling chin--a report of this inheritable dominant character in a four-generation Canadian family.

Trembling chin is an unusual, benign, dominant trait. We describe the condition in a four-generation Canadian family. Investigations, which included E.E.G., electromyograph and facial nerve conductance studies on affected and unaffected individuals, were normal. Premature hearing loss, present in all affected members of this family over age 50, strongly suggests an association of trembling chin with otosclerosis. The merits of early diagnosis and preventive treatment for otosclerosis in persons affected by trembling chin are discussed.

Martin-Gruber anastomosis and unusual sensory innervation of the fingers: report of a case.

We report the electrophysiological study of a patient with carpal tunnel syndrome who, besides the features of median nerve entrapment at the wrist, showed an aberrant motor innervation of Martin-Gruber type and an unusual distribution of the sensory innervation of the fingers. Studies based on collision between sensory volleys showed that the fibers supplying the anomalous innervation did not follow the same path as the aberrant motor fibers. This finding is consistent with the hypothesis that Martin-Gruber anastomosis is made up of motor axons only, and supports the idea that other abnormali

Neuralgic amyotrophy in association with radiation therapy for Hodgkin's disease.

We describe 4 patients with Hodgkin's disease who developed neuralgic amyotrophy in the setting of radiation therapy. In contrast to tumor progression or radiation plexopathy, the symptom onset was abrupt and occurred within days to weeks of receiving radiation treatments. There is an association between Hodgkin's disease, radiation therapy, and neuralgic amyotrophy.

A syndrome of asymmetric limb weakness with motor conduction block.

We describe 3 patients with asymmetric limb weakness, fasciculations (2 patients), relatively preserved reflexes, normal cranial nerves, and few or no sensory abnormalities. The symptoms had been progressive over 1 to 15 years. Detailed motor nerve conduction studies showed conduction block and slowing localized to sharply circumscribed areas 30 to 100 mm long in several nerves in each patient. By contrast, the sensory conduction studies over the same nerve segments were normal, indicating very selective involvement of motor fibers. Sural nerve biopsies showed minor changes that varied among t

Anterior interosseous/median nerve latency ratio.

This report describes a new diagnostic technique for evaluating anterior interosseous nerve injuries (Kiloh Nevin syndrome). The resultant "AIM" (anterior interosseous/median nerve) score establishes a normal ratio of the pronator quadratus latency to the abductor pollicis brevis latency after one stimulus to the proximal median nerve and simultaneous recordings of the evoked response at the respective muscles. One hundred normal nerves were tested in 61 patients. The result was an AIM score of 0.60 (SD = 0.06). Five patients with Kiloh Nevin syndrome were evaluated. The average AIM score was

Motor reflex responses elicited by cutaneous stimulation in the regenerating nerve of man: axon reflex or ephaptic response?

In 57 of 60 nerves (29 median and 31 ulnar) sutured at the wrist, forearm and arm, we recorded motor responses in thenar or hypothenar muscles by electrical stimulation of the corresponding fingers. Recordings were made at different times during the process of regeneration, ranging from 3 months up to 11 years. The responses showed a constant shape and latency to every stimulation (simple or repetitive). The latency was shorter the more distal the level of injury and the greater the elapsed time from the reinnervation. The point of "reflexion" of the responses is at or very near the line of ne

Neurologic sequelae of minor electric burns.

Although neurologic pathology is a well-documented sequela of major electric burns, there are few reports of complications arising from less extensive electrically induced trauma. Two cases of patients with significant disease subsequent to minor electric burns are presented. In one patient with a 2% superficial partial-thickness burn of the right forearm, nerve conduction studies, electromyography, and somatosensory evoked potential studies were consistent with right median mononeuropathy at the wrist and a distal slowing of the right ulnar nerve. The second patient presented with persistent

Electrodiagnostic findings in the early stages of Bell's palsy and Ramsay-Hunt's syndrome.

The incidence of denervation, as determined electrodiagnostically (nerve excitability test and electroneuronography), was analysed during the course of the disease in 150 patients with Bell's palsy and 45 with Ramsay-Hunt's syndrome. Of all 150 patients, 14.2% of those with Bell's palsy and 44% of the patients with Ramsay-Hunt syndrome either showed denervation at the time of the initial examination or developed denervation from neurapraxia within 3 weeks after the onset of paralysis.

Auditory event-related potentials in adult patients with Gilles de la Tourette's syndrome in the oddball paradigm.

In 20 Tourette patients and 20 control subjects auditory event-related potentials evoked in an oddball paradigm were studied in 2 conditions: a non-motor condition (NMC) in which subjects had to attend tones, and a motor condition (MC) in which they had to press a microswitch to deviant tones. In the NMC patients had a reduced P2 in response to the standards. The deviant-standard subtraction wave forms of the NMC showed a discernible MMN-P165-N2b-P3 complex in the controls, whereas in the patients only the P3 was well developed. In the MC patients had a reduced N1 to the standards. Both groups

Neuropathies with prolonged conduction block, single and grouped fasciculations, localized limb myokymia.

Fourteen patients presented prolonged conduction block (CB) related to hereditary neuropathy with liability to pressure palsies (n = 7), neuropathy with proximal multifocal persistent CB (n = 2), radiation plexopathy (n = 3) and chronic acquired polyneuropathy (n = 2). All subjects presented both single and grouped fasciculation potentials that resulted in visible limb myokymia in 9 patients. This ectopic activity was recorded only in muscles affected by the CB, was more important in severe or complete CB, and often arose from terminal branches of blocked axons. These fasciculations and groupe
