Total parenteral nutrition in pregnancy: case review and guidelines for calculating requirements.

Although many aspects of the use of total parenteral nutrition (TPN) in pregnancy are controversial, the long-term sequelae of maternal malnutrition in fetal health are not. To avoid these complications, TPN is advocated for use in pregnancies complicated by maternal starvation. The purpose of this paper is to outline an easy to follow method for prescribing TPN solution to meet the needs of the gravid patients.

Malnutrition and clinical outcomes: the case for medical nutrition therapy.

Malnutrition is not a new or a rare problem. In studies involving more than 1,327 hospitalized adult patients, 40% to 55% were found to be either malnourished or at risk for malnutrition, and up to 12% were severely malnourished. Surgical patients with likelihood of malnutrition are two to three times more likely to have minor and major complications as well as increased mortality; and their length of stay can be extended by 90% compared with the stay of well-nourished patients. Hospital charges are reported to be from 35% to 75% higher for malnourished patients than for well-nourished patient

[The effect of the lipid emulsions used in parenteral nutrition on the immune response capacity].

The preservation or improvement of the immune response capacity in patients on TPN is a priority objective, since in this type of patient, immune deficiencies caused by the main illness, malnutrition or the therapy applied are constant and reflected in the great incidence of septic complications. The use of lipid emulsions in TPN has an obvious clinical repercussion in this respect, due to the intake of EFA (eicosanoid precursors) and the possible negative effect suggested in some studies on the reticulo-endothelium system and other immune functions. This review analyzes the effect of lipid em

Risks of alternative nutrition in infancy: a case report of severe iodine and carnitine deficiency.

A 7.5-month-old infant with failure to thrive, developmental delay, muscular hypotonia, a visible goitre and severe osteopenia is described. Laboratory examination revealed a markedly increased serum TSH with low free T4, severe iodine and carnitine deficiency. The infant was breastfed until the age of 2.5 months and was then given a mixture of almond extract in water. The mother is a strict vegan and the father a lactovegetarian. The nutritional intake of the child was severely depleted in calories (-46%), calcium (-73%) and iodine (-88%). The restrictive alternative nutrition was responsible

The association of xerostomia and inadequate intake in older adults.

Recent studies indicate that nearly one in five older adults has xerostomia (dry mouth). Salivary gland dysfunction and/or inadequate saliva increases the difficulty of these older adults in obtaining proper nutrition. Problems in lubricating, masticating, tolerating, tasting, and swallowing food contribute notably to the complex physiological and psychological manifestations of aging. To our knowledge, the literature has not demonstrated an association between xerostomia and malnutrition in the elderly. We randomly selected 67 older adults from institutionalized and free-living geriatric popu

Nutrition and esophageal cancer.

Several case control studies in esophageal cancer employing a food-frequency dietary questionnaire suggested a fairly uniform dietary pattern which could be implicated in the causation of the disease. Although various micronutrients (vitamins and trace elements) have also been found to have either a positive or negative association, findings were more clear-cut for the different food items contributing the micronutrients than for the specific micronutrients themselves. The difficulties encountered in dietary interviews in case control studies are discussed. Nutritional supplementation has a th

Parenteral nutrition in the management of gastrointestinal Kaposi's sarcoma in a patient with AIDS.

A patient with acquired immunodeficiency syndrome (AIDS) who required aggressive nutritional intervention via home parenteral nutrition therapy is described, and nutritional status, etiology and therapeutic management of AIDS-associated malnutrition, role of nutrition support, and factors for consideration in using parenteral nutrition in AIDS patients are discussed. Parenteral nutrition therapy was initiated in a 30-year-old AIDS patient with Kaposi's sarcoma lesions of the gastrointestinal tract because of rapid weight loss, low serum protein levels, and malnutrition. He had previously under

Treatment of undernutrition and electrolyte disturbances in anorexia nervosa.

Medical care of undernutrition and hydro-electrolytic disturbances in anorexia-nervosa differs according to the stage of weight loss, as appreciates through "corpulence index". In each case, it will depend on a precise appreciation of biological data, mainly hydro-electrolytic ones. One has to particularly stress the frequence of hypochloremic-hypokalemic alkalosis which must be treated by isotonic saline perfusion till the normalisation of plasma electrolytes. Refeeding can be achieved either by oral, parenteral or nasogastric tube: this last method being, in our hand, the most successful. Th

Interactions of nutrition and infection.

Interactions between malnutrition and infection contribute directly to the health of individuals and communities. The relevance of this concept to the practice of clinical medicine, dentistry, and public health is supported by an imposing collection of evidence from the clinic, laboratory, and field. Malnutrition can interfere with any body mechanism that acts as a barrier to the multiplication or progress of infectious agents. This includes formation of specific antibodies, number and activity of phagocytes, integrity of skin, mucous membranes, and other tissues. Some of the less definite, no

Assessment of Nutritional Status in the Diagnostic Evaluation of the Child with Growth Failure.

Current clinical guidelines provide information about the diagnostic workup of children with growth failure. This mini-review focuses on the nutritional assessment, which has received relatively little attention in such guidelines. The past medical history, in particular a low birth size and early feeding problems, can provide information that can increase the likelihood of nutritional deficits or several genetic causes. The current medical history should include a dietary history and can thereby reveal a poorly planned or severely restricted diet, which can be associated with nutritional defi

The gut microbiome in anorexia nervosa: relevance for nutritional rehabilitation.

Rapidly accumulating evidence supports the important role of gut microbiome in the regulation of mood, behaviour, appetite, gastrointestinal symptomology, and nutrient metabolism. These are all core features frequently altered in individuals with anorexia nervosa (AN). Current treatment recommendations for AN support the use of high-calorie diets as an essential part of nutritional rehabilitation, commonly achieved by elevating the fat content of the diet. However, in contrast to this approach, there is accumulating evidence suggesting the importance of balanced, high-fibre diets on the gut mi

Dysfunctional eating in the eating disorders.

Aberrant eating patterns in the eating disorders have been observed across various laboratory-based and clinical studies. It is now clear that problems in experiencing and expressing hunger, appetite, and satiety in anorexia and bulimia nervosa are likely to perpetuate the disorders once established. Whether problems in appetite regulation are primary or secondary to the development of the disorders is unknown. In studies examining indices of appetite regulation after treatment, there still remain significant levels of eating abnormality. This suggests that the main goals of treatment, includi

Chronic Guillain-Barré syndrome and refractory diarrhea after RYGB: a 9-year longitudinal case report.

We present a 46-year-old male who developed chronic Guillain-Barré syndrome and refractory diarrhea after undergoing Roux-en-Y gastric bypass, with progressive neurological deterioration and 34.9% total weight loss over 9 years. Serial evaluations demonstrated severe deficiencies include vitamin D, calcium, iron and other nutritions, accompanied by evidence of autoimmune dysregulation. The patient's condition progressed to require partial parenteral nutrition due to worsening neurological and gastrointestinal dysfunction. This case highlights the critical need for ongoing nutritional monitorin

Anaemia and iron deficiency disease in children.

Iron deficiency is the single most common nutritional disorder world-wide and the main cause of anaemia in infancy, childhood and pregnancy. It is prevalent in most of the developing world and it is probably the only nutritional deficiency of consideration in industrialised countries. In the developing world the prevalence of iron deficiency is high, and is due mainly to a low intake of bioavailable iron. However, in this setting, iron deficiency often co-exists with other conditions such as, malnutrition, vitamin A deficiency, folate deficiency, and infection. In tropical regions, parasitic i

The "First 1,000 Days+" as Key Contributor to the Double Burden of Malnutrition.

Growth from conception through age 2 years, the "First 1,000 days," is important for long-term health of the growing fetus and child and is influenced by several factors including breastfeeding and complementary feeding. Low- and middle-income countries face a complicated array of factors that influence healthy growth, ranging from high food insecurity, poor sanitation, limited prenatal or neonatal care, and high levels of poverty that exacerbate the "vicious cycle" associated with intergenerational promotion of growth retardation. It is now well recognized that the period prior to conception,

Gastrointestinal and nutritional problems occur frequently throughout life in girls and women with Rett syndrome.

OBJECTIVE: We conducted a nationwide survey to determine the prevalence of common gastrointestinal and nutritional disorders in Rett syndrome (RTT) based on parental reporting and related the occurrence of these problems to age and methyl-CpG-binding protein 2 (MECP2) gene status. METHODS: We designed a questionnaire that probed symptoms, diagnoses, diagnostic tests, and treatment interventions related to gastrointestinal and nutritional problems in RTT. The International Rett Syndrome Foundation distributed the questionnaire to 1666 family-based members and forwarded their responses for our
