Pattern electroretinogram as a function of spatial frequency after retrobulbar optic neuritis.

Steady-state (8-Hz) pattern electroretinograms in response to counterphased sinusoidal gratings of variable spatial frequency (0.6-4.8 c/deg) were recorded in 17 patients who had had retrobulbar optic neuritis in one or both eyes (23 eyes with a clinical history of optic neuritis) and in 21 age-matched normal subjects. Amplitude and phase of the Fourier-analyzed pattern electroretinogram second harmonic were measured. The mean pattern electroretinogram amplitude of patients was significantly reduced compared with that of controls. Amplitude reductions were more marked at intermediate (1-1.4 c/…

Rapid response of syphilitic optic neuritis to posterior sub-tenon's steroid injection.

An HIV-positive man with subacute syphilitic meningitis developed severe bilateral visual loss from optic neuritis. His visual acuity improved remarkably within 24 hours after single posterior sub-Tenon's injections of triamcinolone (Kenalog) were given. Periocular steroid injections should be considered as an adjunctive treatment of syphilitic optic neuritis.

[Visual evoked potentials in chessboard pattern reversion in children with optic neuritis].

Visual evoked potentials (VEP) to chess pattern reversion were studied in 17 children with optic neurites during the acute stage and in the course of convalescence 1-2 months and 1-3 years after the disease onset. Coarse VEP changes were detected in the patients during the acute stage of optic neuritis. A positive time course of VEP was followed up in late periods after the acute attack of the disease. Despite normalization of vision acuity, no complete restoration of VEP to small-cell chess patterns was seen. The findings evidence that studies of VEP to chess pattern reversion should be carri…

Temporal and spatial response properties of optic neuritis patients manifesting statokinetic dissociation.

Statokinetic dissociation (SKD) refers to a greater reduction in sensitivity to stationary visual stimuli relative to similar targets in motion. We evaluated SKD perimetrically in nine optic neuritis (ON) patients and in nine normals by measuring the difference between static and 4 degrees /s kinetic thresholds for a size I target (0.11 degrees ) along the four oblique meridians. To assess temporal response characteristics, we used flicker perimetry to measure the sensitivity to 2-, 8-, and 20-Hz sinusoidal flicker at 5 degrees intervals along the oblique meridians. The ON patients' sensitivit…

Immunological studies on patients with optic neuritis without evidence of multiple sclerosis.

We studied 6 patients with idiopathic optic neuritis (ON) after a mean follow-up period of 12 years. No evidence of multiple sclerosis (MS) was found on clinical grounds, nor by magnetic resonance imaging (MRI) or evoked responses. Levels of spontaneous proliferation, serum gamma-interferon (IFN-gamma) and lymphocyte MHC locus II antigen were similar in ON and in 6 patients with progressive MS, but different in healthy controls. This suggests that similar immunological conditions in vivo prevail after an idiopathic ON and in MS. A normal PHA-induced interleukin-2 receptor expression and lympho…

Cerebrospinal fluid antibodies to myelin basic protein in acute idiopathic optic neuritis.

Free and bound levels of anti-myelin basic protein (anti-MBP) antibodies were measured by radioimmunoassay in the cerebrospinal fluid of 20 patients with acute idiopathic optic neuritis, 133 patients with multiple sclerosis (MS) divided into three clinical subgroups, and 76 normal control subjects. Patients with idiopathic optic neuritis had elevated levels of anti-MBP predominantly in free form, resulting in an elevated (above unity) free/bound anti-MBP ratio similar to that of MS patients with acute relapses. These data suggest that acute idiopathic optic neuritis, like active MS, is associa…

Regional threshold contrast sensitivity within the central visual field in optic neuritis.

Contrast sensitivity was measured at nine locations within the central 10 degrees of the visual field in cases of recovered optic neuritis having varying degrees of residual deficit. A sample of 82 patches of visual field was obtained in 14 cases. Circular patches of vertically orientated sinusoidal gratings, 2.5 degrees in diameter, were used. The gratings were modulated in time at 8 Hz and the effect of spatial frequency on the threshold loss determined at each visual field location. As anticipated from what is known of visual field changes in the disorder there was considerable variation in…

Visual fatigue phenomenon and prescribing tinted lenses in patients with optic neuritis.

The fatigue phenomenon of grating visual acuity was examined in 36 patients with optic nerve disorders by means of a laser interferometric acuity device. The phenomenon was found in 6 of 36 cases even under a luminance corresponding to the Japanese standards of office illumination. This fact indicated that the patients suffered visual fatigue in their everyday vision. After they were prescribed tinted lenses to eliminate the fatigue, the patients had comfortable sustained vision in their daily work.

CSF myelin basic protein levels in acute optic neuritis and multiple sclerosis.

Normal CSF-MBP levels as determined by a RIA were less than 6.2 ng/ml CSF (mean 3.9). Eighty percent of patients with acute optic neuritis have CSF-MBP levels greater than 6.2 ng/ml (mean 7.6 ng/ml CSF). Five of 7 patients with acute internuclear ophthalmoplegia due to an initial exacerbation of demyelination have CSF-MBP levels above 6.2 ng/ml (mean 6.8 ng/ml). Fifty percent of MS patients with chronic progressive disease have CSF-MBP levels above 6.2 ng/ml (mean 6.7 ng/ml). MS patients experiencing monosymptomatic exacerbations show elevated CSF-MBP levels in 75% in 75% of cases (mean 8.2 ng…

Subtle disturbances of vision after optic neuritis elicited by studying contrast sensitivity.

Eight patients with a subjective disorder of vision yet normal Snellen acuities after optic neuritis were shown to have an abnormal contrast sensitivity function in their affected eye. It appears that certain disorders of vision are associated with an abnormality of the contrast sensitivity function in spite of near normal visual acuity. Such an abnormality may affect pattern recognition without having an influence on Snellen acuity because of the high contrast of the latter and its predominant association with the higher spatial frequencies. Contrast sensitivity function is thus the only tool…

Relation between Genetic markers and oligoclonal IgG in CSF in optic neuritis.

Thirty patients with acute, unilateral optic neuritis (ON), where re-examination after a mean observation period of 5 years did not reveal any aetiology, were investigated with regard to laboratory abnormalities frequently observed in multiple sclerosis. Eleven patients had oligoclonal IgG in CSF. In 5 of these a measles virus antibody response within the CNS was demonstrable. The remaining 19 patients did not display oligoclonal CSF IgG, nor an antibody response. The major histocompatibility antigens HL-A3 and HL-A7 occurred at similar frequencies in ON and in controls, irrespective of the pr…

[Does a correlation exist between Myambutol dosage and occurrence of optic neuritis].

The frequency of retrobulbar neuritis during a long lasting myambutol therapy increases proportionally with the increased doses. This may be seen in retrospection on 2007 cases divided into 16 groups with doses from 15 till 60 mg per kg of bodyweight. Graphically a function may be established between myambutol doses and frequency of optic neuritis (y = -23.2 + 1.1 x). This shows for 20 mg/kg: 0%, for 25 mg/kg: 5%, for 30 mg/kg: 10% frequency of optic neuritis. Those findings allow a rough evaluation of neuritis risk and confirm clinical observations. A planed examination of patients groups wit…

Current and future potential of retinal optical coherence tomography in multiple sclerosis with and without optic neuritis.

Multiple sclerosis (MS) is a disorder characterized by inflammation and neuroaxonal degeneration. The latter is held responsible for the irreversible disability in patients with MS. The eye is a unique window into the brain. With the advent of optical coherence tomography, accurate quantification of retinal layer thickness has become feasible. Neuroaxonal degeneration affecting the retinal layers is structurally and functionally related to pathology in the visual pathways, which is most severe following MS optic neuritis. This is relevant to recognize because MS optic neuritis may mask the sub…
