Using home monitoring technology to study the effects of traumatic brain injury on older multimorbid adults: protocol for a feasibility study.

INTRODUCTION: The prevalence of traumatic brain injury (TBI) among older adults is increasing exponentially. The sequelae can be severe in older adults and interact with age-related conditions such as multimorbidity. Despite this, TBI research in older adults is sparse. Minder, an in-home monitoring system developed by the UK Dementia Research Institute Centre for Care Research and Technology, uses infrared sensors and a bed mat to passively collect sleep and activity data. Similar systems have been used to monitor the health of older adults living with dementia. We will assess the feasibility

Cortical network fingerprints predict deep brain stimulation outcome in dystonia.

BACKGROUND: Deep brain stimulation (DBS) is an effective evidence-based therapy for dystonia. However, no unequivocal predictors of therapy responses exist. We investigated whether patients optimally responding to DBS present distinct brain network organization and structural patterns. METHODS: From a German multicenter cohort of 82 dystonia patients with segmental and generalized dystonia who received DBS implantation in the globus pallidus internus, we classified patients based on the clinical response 3 years after DBS. Patients were assigned to the superior-outcome group or moderate-outco

Prolactin-Linked Plasma Cell-Macrophage Immune Phenotype in Synchronous Bilateral Plasma Cell Mastitis: A Two-Center Prediction Study with Tissue Correspondence.

BACKGROUND: Plasma cell mastitis (PCM) is a chronic inflammatory breast disease characterized histologically by periductal plasma cell-rich infiltration and ductal ectasia. Bilateral involvement occurs in approximately one fifth of patients, but whether synchronous bilateral PCM reflects more extensive local disease or a systemic immunoendocrine phenotype remains unclear. OBJECTIVE: To examine whether synchronous bilateral PCM is associated with a prolactin (PRL)-centered circulating immune profile that corresponds to local tissue immune features, and to develop a combined biomarker model for

Added value of FDG-PET for detection of progressive supranuclear palsy.

BACKGROUND: Diagnostic criteria for progressive supranuclear palsy (PSP) include midbrain atrophy in MRI and hypometabolism in [18F]fluorodeoxyglucose (FDG)-positron emission tomography (PET) as supportive features. Due to limited data regarding their relative and sequential value, there is no recommendation for an algorithm to combine both modalities to increase diagnostic accuracy. This study evaluated the added value of sequential imaging using state-of-the-art methods to analyse the images regarding PSP features. METHODS: The retrospective study included 41 PSP patients, 21 with Richardso

Angiographic phenotypes predict FFR/iFR discordance in severe aortic stenosis: A cluster analysis.

BACKGROUND: Assessing coronary artery disease in severe aortic stenosis (AS) is challenging due to altered hemodynamics, causing discordance between fractional flow reserve (FFR) and instantaneous wave-free ratio (iFR). The role of angiographic lesion characteristics in predicting this discordance remains unclear. OBJECTIVES: To identify angiographic predictors of FFR/iFR discordance and define distinct angiographic phenotypes using unsupervised machine learning. METHODS: This prospective, single-center registry evaluated 401 intermediate coronary lesions from 221 patients with severe AS (ao

FLP: Factor lattice pattern-based automated detection of Parkinson's disease and specific language impairment using recorded speech.

BACKGROUND: Timely detection of neurodevelopmental and neurological conditions is crucial for early intervention. Specific Language Impairment (SLI) in children and Parkinson's disease (PD) manifests in speech disturbances that may be exploited for diagnostic screening using recorded speech signals. We were motivated to develop an accurate yet computationally lightweight model for speech-based detection of SLI and PD, employing novel feature engineering techniques to mimic the adaptable dynamic weight assignment network capability of deep learning architectures. MATERIALS AND METHODS: In this

Perfluoroalkyl substances (PFAS) and lead (Pb) as "cardiovascular disruptors" in 9-11-year-old children living in Syracuse, New York, United States.

OBJECTIVE: Per- and polyfluoro-alkyl substances (PFAS) and lead (Pb) are ubiquitous environmental toxicants with apparent impact on cardiovascular disease (CVD) risk. As one possible mechanism for this increased risk, we have previously demonstrated an association between Pb exposure and heightened cardiovascular reactivity to acute psychological stress, a CVD risk factor. The present study expands this approach and considers both PFAS and Pb exposures (and the possible interaction). METHODS: We assessed 14 serum PFAS and whole blood Pb concentrations in a sample of 9-11 year-old children (N

Temporal and tissue-specific activation of aryl hydrocarbon receptor in discrete mouse models of kidney disease.

Emerging evidence in animal models of chronic kidney disease (CKD) implicates Aryl Hydrocarbon Receptor (AHR) signaling as a mediator of uremic toxicity. However, details about its tissue-specific and time-dependent activation in response to various renal pathologies remain poorly defined. Here, a comprehensive analysis of AHR induction was conducted in response to discrete models of kidney diseases using a transgenic mouse line expressing the AHR responsive-promoter tethered to a β-galactosidase reporter gene. Following validation using a canonical AHR ligand (a dioxin derivative), the transg

White matter connections reflect changes in voluntary-guided saccades in pre-symptomatic Huntington's disease.

Huntington's disease is caused by a known genetic mutation and so potentially can be diagnosed many years before the onset of symptoms. Neuropathological changes have been found in both striatum and frontal cortex in the pre-symptomatic stage. Disruption of cortico-striatal white matter fibre tracts is therefore likely to contribute to the first clinical signs of the disease. We analysed diffusion tensor MR image (DTI) data from 25 pre-symptomatic gene carriers (PSCs) and 20 matched controls using a multivariate support vector machine to identify patterns of changes in fractional anisotropy (F

Classification of Volatile Organic Compounds Using a Novel High-Frequency Quartz Crystal Microbalance Sensor Array Based on Ethyl Cellulose Microstructures.

Detecting volatile organic compounds (VOCs) is essential because they affect air quality and can serve as biomarkers for non-invasive early disease detection. VOCs can be identified using electronic noses combining cross-reactive sensors, pattern recognition, and classification algorithms. Here, a single-polymer 12-sensor array based on 30 MHz quartz crystal microbalances was developed using three ethyl cellulose (EC) microstructures: casting films (CFs), anti-solvent microparticles (ASMs), and electrospray microparticles (ESMs). The array detected ethanol, ethyl acetate, and heptane at ppm co

Revolutionizing Pancreatic Cancer Detection: The Role of AI-Integrated Biosensors in Early Diagnostics.

Pancreatic cancer, particularly pancreatic ductal adenocarcinoma (PDAC), is among the most aggressive malignancies, often progresses silently until advanced stages, which significantly contributes to high mortality rates. The development of biosensor technologies, when augmented by artificial intelligence (AI), represents a paradigm shift in early pancreatic cancer detection by enhancing analytical sensitivity and specificity, while enabling real-time data processing. This review critically evaluates AI-driven biosensors, including enzyme-based, immunosensor, nucleic acid, electrochemical, and

Artificial Intelligence and the Clinical Gaze: Visual Practices of AI-Assisted Colonoscopy.

This article examines how commercial artificial intelligence (AI) systems are integrated into the visual practice of colonoscopy and how they reshape the clinical gaze as a sociotechnical and situated mode of perception. Based on ethnographic observations of colonoscopies and in-depth interviews with gastroenterologists, this study analyses the real-time use of AI-based pattern recognition systems during diagnostic procedures. Unlike retrospective image analysis in radiology, AI in colonoscopy operates in vivo and in real time, requiring practitioners to engage with algorithmic markings within

Artificial intelligence in biomarker discovery for diseases: diagnostic and therapeutic prospects.

Biomarkers are central to modern diagnostics and therapeutics, yet traditional discovery approaches suffer from single-modality analyses, weak mechanistic foundations, and low reproducibility. Artificial intelligence (AI) enables the integration of complex multimodal biomedical data, but the translation of AI-derived biomarkers into clinical applications remains inconsistent. This review examines how AI reshapes biomarker discovery across diseases, focusing on biological mechanisms, validation requirements, and therapeutic integration. We synthesize evidence across molecular, cellular, imaging

Towards the Probabilistic Analysis of Small Bowel Capsule Endoscopy Features to Predict Severity of Duodenal Histology in Patients with Villous Atrophy.

Small bowel capsule endoscopy (SBCE) can be complementary to histological assessment of celiac disease (CD) and serology negative villous atrophy (SNVA). Determining the severity of disease on SBCE using statistical machine learning methods can be useful in the follow up of patients. SBCE can play an additional role in differentiating between CD and SNVA. De-identified SBCEs of patients with CD and SNVA were included. Probabilistic analysis of features on SBCE were used to predict severity of duodenal histology and to distinguish between CD and SNVA. Patients with higher Marsh scores were more

Electrocardiographic changes predate Parkinson's disease onset.

Autonomic nervous system involvement precedes the motor features of Parkinson's disease (PD). Our goal was to develop a proof-of-concept model for identifying subjects at high risk of developing PD by analysis of cardiac electrical activity. We used standard 10-s electrocardiogram (ECG) recordings of 60 subjects from the Honolulu Asia Aging Study including 10 with prevalent PD, 25 with prodromal PD, and 25 controls who never developed PD. Various methods were implemented to extract features from ECGs including simple heart rate variability (HRV) metrics, commonly used signal processing methods

An iPhone application using a novel stool color detection algorithm for biliary atresia screening.

BACKGROUND: The stool color card has been the primary tool for identifying acholic stools in infants with biliary atresia (BA), in several countries. However, BA stools are not always acholic, as obliteration of the bile duct occurs gradually. This study aims to introduce Baby Poop (Baby unchi in Japanese), a free iPhone application, employing a detection algorithm to capture subtle differences in colors, even with non-acholic BA stools. METHODS: The application is designed for use by caregivers of infants aged approximately 2 weeks-1 month. Baseline analysis to determine optimal color parame
