From Caries to Periodontal Breakdown: A Biological and Clinical Continuum Linking Cariology, Operative Dentistry, Endodontics, and Periodontology.

Dental diseases have long been taught and treated as separate entities: cariology, operative dentistry, endodontics, and periodontology, each working within its own boundaries. However, increasing biological and clinical evidence suggests that this classified view does not fully reflect how disease progresses in the mouth. Instead, dental disease should be understood as a continuum within the interconnected tooth-pulp-periodontium complex. This review provides current evidence showing how dental caries can serve as the starting point of a process that can progress through pulpitis and apical p

Occurrence of two newly named oral treponemes - Treponema parvum and Treponema putidum - in primary endodontic infections.

BACKGROUND/AIMS: Recent evidence from molecular genetic studies has revealed that oral Treponema species are involved in infections of endodontic origin. This study assessed the occurrence of two newly named oral treponemes - Treponema parvum and Treponema putidum - in primary endodontic infections using a culture-independent identification technique. METHODS: Genomic DNA was isolated directly from clinical samples, and a 16S rRNA gene-based nested polymerase chain reaction (PCR) assay was used to determine the presence of T. parvum and T. putidum. Species-specific primer pairs were developed

Tissue response following CO2 laser application in apical surgery: light microscopic assessment in dogs.

The potential advantages of CO2 laser in apical surgery have not been established histologically. Therefore, the long-term effects of CO2 laser on the apical and periapical tissues were examined histologically in dogs 6 months after apical surgery. Lased specimens and unlased controls showed periapical inflammatory and osteogenic reactions. Lased root surfaces revealed craters with a superficial charred layer closely associated with new cementum-like matrix. The subjacent dentin appeared tubule-free and eosinophilic. Lased bone trabeculae showed a charred layer with a deeper osteocyte-free zon

Monospecies Bacteria-Induced Chronic Apical Periodontitis Triggers the Aortic Inflammatory Response Via Modulation of Systemic Inflammation and Lipid Metabolism.

Cardiovascular disease (CVD) is the leading cause of death worldwide and has been confirmed to be associated with a common oral bacterial infection-chronic apical periodontitis (CAP). However, the detailed mechanisms remain controversial. CAP can potentially alter systemic inflammation, lipid metabolism, and gut microbiota, all of which contribute to the progression of the aortic inflammatory response. This study aimed to explore the differential effects between Enterococcus faecalis and Porphyromonas gingivalis-CAP on the aortic inflammatory response, which focused on changes in systemic infl

Identification of cuproptosis-related genes in chronic apical periodontitis based on bulk and single-cell RNA sequencing analyses and experimental validation.

BACKGROUND: Chronic apical periodontitis (CAP) is a prevalent oral inflammatory disease, yet the complex mechanisms underlying its etiology remain unclear. A recently identified cell death pathway known as cuproptosis may be linked to this condition. METHODS: Differentially expressed cuproptosis-related genes (DE-CRGs) were identified by integrating human CAP dataset (GSE237398) with health control (HC) dataset (GSE223924) from the Gene Expression Omnibus (GEO) database. Subsequently, single-cell RNA sequencing (scRNA-seq) data from clinical samples with CAP (n=3) and HC (n=3) from the GSE171

Histologic and Electron Microscopic Characterization of a Human Immature Permanent Premolar with Chronic Apical Abscess 16 Years after Regenerative Endodontic Procedures.

Previous studies have reported successful clinical outcomes after regenerative endodontic procedures (REPs) for immature permanent teeth with pulpal infection. However, it remains unclear whether the procedures promote true regeneration or repair. This case report describes the histologic and electron microscopic characteristics of a human immature permanent premolar with a chronic apical abscess that was treated with an REP. Tooth #20 of a 9-year-old girl underwent an REP. At the 6-year follow-up, the patient was asymptomatic, and closure of the apex and thickening of the dentinal walls were

Factors that influence the outcomes of surgical endodontic treatment.

Surgical endodontic treatment encompasses a broad spectrum of procedures, amongst which root-end cavity preparation and filling, retrograde root canal treatment and through-and-through endodontic surgery, may be classified under the umbrella term 'Root-end surgery'. This narrative review considers the available data on periapical healing, soft tissue healing, tooth survival and oral health-related quality of life (OHRQoL), following root-end surgery and the factors that affect its outcomes. The pooled periapical healed rate for the studies published up to 2021 was 69% (95% CI: 65%, 73%) but in

RGS10 negatively regulates apical periodontitis via TFEB-mediated autophagy in BABL/c mice model and in vitro.

AIM: Apical periodontitis is a prevalent oral inflammatory disease that has recently been linked to transcription factor EB (TFEB)-mediated autophagy. Regulator of G-protein signalling 10 (RGS10) is reported to be an effective regulator of the immune system and inflammation. This study aimed to investigate the involvement of RGS10 during the development of apical periodontitis through the TFEB-mediated autophagy signalling pathway. METHODOLOGY: Sixty BALB/c mice were randomly divided into four groups of 15 mice for the in vivo experiment. Rgs10 was locally overexpressed through eight injectio

Outcome measures to assess the effectiveness of endodontic treatment for pulpitis and apical periodontitis for use in the development of European Society of Endodontology (ESE) S3 level clinical practice guidelines: a protocol.

The European Society of Endodontology (ESE) is in the process of developing S3Level Clinical Practice Guidelines for the treatment of pulpal and apical disease for the benefit of clinicians and patients. In order to ensure a homogenous review process in the development of the clinical practice guidelines, it is essential that the core outcomes for all endodontic treatments are standardized and recommendations are made regarding minimum follow-up time specific to each outcome measure. In the absence of a recognized core outcome set in Endodontics, the current project aimed to follow an establis

The importance of the initial comprehensive dental examination.

New diagnostic technology that improves patient care is becoming more common in dental practices at both the generalist and specialist levels. However, new diagnostic tests may be ordered prematurely or given more importance than warranted, resulting in misdiagnoses or delay of appropriate treatment. In this report, we describe a case in which several oral health care professionals failed to diagnose persistent facial pain correctly, in part because of failure to conduct a comprehensive dental examination and over-reliance on new technology. During 13 months of persistent mandibular pain, vari

Endodontic treatment and hypotheses on an unusual case of dens invaginatus.

This work describes a case of "dens invaginatus" and analyzes the possible aspects of this malformation. An unusual type of dens invaginatus was detected in a young patient corresponding to the maxillary lateral incisor and showing extensive periradicular radiolucency and a vestibular fistula. The radiographic and tomographic examination revealed two apices: one wide open in the distal part of the root and the other normally formed in the mesial. Nonsurgical endodontic treatment was performed by using the "one-step apexification technique," filling both apexes with mineral trioxide aggregate f
