Mineral-targeted alkaline phosphatase improves bone graft-mediated alveolar bone defect healing in mice.

BACKGROUND: Alveolar bone loss challenges tooth retention and implant placement. Freeze-dried bone allograft (FDBA) is widely used for alveolar ridge preservation but has limitations in resorption rate and healing time. In this proof-of-concept study, we tested whether pro-mineralization enzyme, tissue-nonspecific alkaline phosphatase (TNAP) enhances bone healing. METHODS: Seven-week-old mice (n = 4-6/group; equal numbers of males and females) underwent maxillary molar extraction and standardized alveolar defect creation. Mice were assigned to sham, FDBA with saline (BGS) or FDBA with mineral

A comparison of five statistical methods used to analyze longitudinal EORTC QLQ-C30 quality of life scores in randomized controlled trials: a simulation study.

BACKGROUND AND OBJECTIVES: Quality of life (QoL) is an important component of cancer care. The European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 30 (EORTC QLQ-C30) is widely used to evaluate patient-reported QoL over time. A scoping review found a wide range of statistical methods are used to analyze longitudinal QLQ-C30 scores in randomized controlled trials (RCTs), and many of the methods ignore the longitudinal nature of the data, potentially impacting the conclusions drawn. We planned a simulation study to evaluate the performance of five recomme

Periodontal plastic surgery for cases with fixed "hygienic" lingual retainer: Case study.

BACKGROUND: The fixed "hygienic" lingual retainer (FHLiR)'s design is consistently observed as a contributing factor to plaque accumulation, possible minor undesired tooth movement, resulting in buccal bone loss, and gingival recession (GR). This case study aimed to describe the clinical outcomes of a combined periodontal treatment and periodontal plastic surgical approach for the treatment of GR in patients using FHLiR. METHODS: This case study involves three systemically healthy, non-smoking patients with localized GRs (RT1 and/or RT2) in the mandibular central incisors associated with FHLi

Rotated pedicled palatal flaps for primary closure in immediate post-extractive guided bone regeneration at maxillary molar locations: A case report of a new technique.

BACKGROUND: Replacing severely compromised teeth with endo-periodontal lesions continues to pose a clinical challenge because of accompanying deficiencies in hard and soft tissues. While traditional staged protocols, involving vertical ridge augmentation and soft tissue grafting, are effective, they are complex, time-intensive, and often not well-received by patients. This case report presents a simplified approach that combines immediate guided bone regeneration (GBR) using a rotated pedicled palatal flap to achieve primary closure without requiring buccal flap advancement, thereby preserving

Theory of clinical therapeutic progress: reconciling equipoise with fat-tailed (skewed) outcomes.

OBJECTIVE: Randomized controlled trials (RCTs) embody a paradox: they require equipoise-genuine uncertainty about treatment effects-to ethically protect participants, yet this very requirement can slow therapeutic innovation by discouraging trials of promising new treatments. We sought to determine which statistical distribution best reflects the balance between protecting trial participants and advancing therapeutic discoveries. STUDY DESIGN AND SETTING: We performed a systematic review to analyze 716 cancer RCTs (1955-2018; published by 2022), encompassing 984 experimental vs standard treat

Anti-inflammatory and antiresorptive activities of tanshinone-IIA mitigate alveolar bone destruction in mice with experimental periodontitis.

BACKGROUND: Periodontitis is a chronic inflammatory condition that leads to progressive destruction of the alveolar bone. Currently, there is a lack of effective adjuvant treatments to nonsurgical periodontal therapy offering strong antiresorptive properties. Tanshinone IIA (T-IIA), a lipophilic compound derived from Salvia miltiorrhiza, exhibits various biological properties. This study investigates the antiresorptive activity of tanshinone as an adjuvant therapy for periodontitis. METHODS: Forty 8-week-old male C57BL6/J mice were randomly assigned to four experimental groups: control (C), p

Systematic Review and Meta-Analysis of Individual Participant Data: Randomized, Placebo-Controlled Trials of Selective Serotonin Reuptake Inhibitors for Pediatric Obsessive-Compulsive Disorder.

OBJECTIVE: Selective serotonin reuptake inhibitors (SSRIs) are the first choice in pharmacotherapy for children and adolescents with obsessive-compulsive disorder (OCD). SSRI trials for pediatric OCD have not been investigated using individual participant data (IPD), which is crucial for detecting patient-level effect modifiers. This study performed an IPD meta-analysis of efficacy of SSRIs compared with placebo and a meta-regression on baseline patient characteristics that might modify efficacy. METHOD: Crude participant data from short-term, randomized, placebo-controlled SSRI trials for pe

Vitamin C mesotherapy with microneedling for gingival depigmentation: A case study.

BACKGROUND: Surgical methods of gingival depigmentation can be challenging, particularly if the gingival phenotype is thin due to the risk of gingival recession and bone exposure. Thus, exploring alternative, non-surgical, minimally invasive treatment modalities is warranted. In dermatology, vitamin C is extensively used for depigmentation and microneedling for collagen induction, with limited literature about its usage for improving gingival esthetics. The present preliminary case study aims to explore the synergistic use of vitamin C mesotherapy with microneedling for the esthetic management
