Navigating the Perils of Pregnancy and Childbirth Among Youth in La Pointe, Haiti: Insights Captured Through Community Photovoice Research.

In Haiti, surviving pregnancy and childbirth is especially tenuous for young women, who are already marginalized. One of the four leading causes of death for women 20-24 years old is pregnancy- or childbirth-related. An important gap remains regarding youth perspectives of maternal health and well-being within their communities. We conducted a community-based participatory research study using Photovoice to understand the lived experiences of 17 young women from La Pointe, Haiti, and its surrounding rural communities, who survived a maternal near-miss, a grave obstetric event that one narrowly

Navigating neurosurgical care in low- and middle-income countries from Africa: a narrative review of barriers and facilitators.

BACKGROUND: Neurosurgical care is vital for conditions such as traumatic brain injury, brain tumors, and hydrocephalus. In African low- and middle-income countries (LMICs), access remains limited due to systemic and structural challenges. Despite bearing 15% of the global neurosurgical burden, the region faces a massive workforce deficit and underdeveloped infrastructure. OBJECTIVE: To synthesize existing evidence on barriers impeding and facilitators improving access to neurosurgical care in African LMICs. METHODS: A narrative review was conducted through a targeted search of PubMed, Africa

Rethinking malaria elimination: a perspective on challenges and solutions in Angola.

BACKGROUND: Multiple challenges hinder malaria control in Angola, including climatic variability, ineffective vector control, population displacement, socioeconomic inequalities, and increasing resistance to anti-malarial drugs and insecticides. These barriers have been further exacerbated by the COVID-19 pandemic, disrupting healthcare services and reversing prior gains. Despite a 36% reduction in malaria mortality since 2000, Angola remains off track to meet the Global Technical Strategy (GTS) targets for 2025, with no significant progress recorded in reducing malaria mortality between 2015

Problems of equity in US HIV integrated planning, 2015-2021: Enacting a bounded justice continuum.

The US HIV/AIDS response is notably worse than those of other high-income countries. The country's epidemic is marked by low viral suppression rates, high incidence, lacking coordination, and entrenched disparities along lines of sexuality, race/ethnicity, gender, class, and other factors. In 2010, the National HIV/AIDS Strategy for the United States (NHAS) was launched, centering an equity-oriented vision that prioritized marginalized groups. NHAS implementation required states to create HIV integrated plans to better coordinate services and meet populations' needs. We used Carol Bacchi's "Wh

Macro-level determinants of gender differences in the prevalence of major depression and alcohol use disorder in the United States and across Europe.

BACKGROUND: Major depressive disorder (MDD) and alcohol use disorder (AUD) are leading psychiatric causes of burden of disease. Although research has found pronounced gender differences in these disorders, the magnitude of these differences varies substantially between settings. However, integrated analyses of both disorders are lacking, limiting the comparability of findings. Moreover, few studies conceptualize political and socio-cultural characteristics as separate macro-level determinants of gender differences. This paper analyzes the association between gender differences in the prevalenc

Understanding the impact of the COVID-19 pandemic and its control measures on women and children: A Zimbabwe case study.

COVID-19 presented countries with unprecedented health policy challenges. For low-income countries in particular, policymakers had to contend with both the direct threats posed by COVID-19 as well as the social, educational, and economic harms associated with lockdown and other infection prevention and control measures. We present a holistic and contextualised case study of the direct and indirect impacts of COVID-19 on women and children, with some assessment of their uneven distribution across socio-economic, age and gender groups. We used different types of primary and secondary data from m

Factors predicting primary and booster COVID-19 vaccination in a community sample of African American men and women in the United States Midwest.

COVID-19 has disproportionately burdened impoverished minority communities. This study recruited an age- and gender-diverse community sample of 541 Black adults in a United States Midwestern city with large racial health disparities, with the aim of examining factors associated with COVID-19 vaccination. All participants completed measures assessing their COVID-19 vaccination status (unvaccinated, received primary vaccination, or received primary plus booster vaccination) as well as demographic characteristics, socioeconomic factors, health and health system factors, and health behavior theory

Navigating Homelessness Assistance While Pregnant: A Rapid Qualitative Research-to-Policy Collaboration in Washington, DC.

BACKGROUND: Homelessness during pregnancy contributes to adverse pregnancy and infant outcomes from birth through early childhood. Washington, DC, a microcosm of structural inequities in the United States, has persistent racial disparities in perinatal outcomes and housing insecurity. METHODS: Grounded in a reproductive justice framework, we explored the lived experience of navigating homelessness assistance while pregnant to inform recommendations for a collaborative policy and practice change effort. We conducted 20 individual interviews with DC residents who experienced homelessness during

AOA Critical Issues Symposium: Gun Violence as a Public Health Crisis.

Gun violence is an epidemic throughout the United States and is increasing around the world-it is a public health crisis. The impact of gun violence is not limited to the victims (our patients); it also extends to the physicians and caregivers who are taking care of these patients every day. Even more broadly, gun violence affects those living and going to work in potentially dangerous environments. The "vicarious trauma" that is experienced in these situations can have long-term effects on physicians, nurses, and communities. Importantly, socioeconomic disparities and community deprivation st

Determinants of adherence towards tuberculosis guidelines among Indonesian private practitioners: a qualitative study.

INTRODUCTION: In high tuberculosis (TB) burden countries, the private sector manages a large proportion of initial visits by presumptive patients with TB. In Indonesia, the second largest contributor of TB cases globally, private practitioners (PPs) often do not adhere to national TB guidelines. A district public-private mix programme to mitigate this issue was started in 2019, yet engagement remains low. This study examines what affects the decisions of Indonesian PPs regarding TB management. METHODS: We performed semistructured in-depth interviews and focus group discussions (FGDs) with 24

[Informal care and occupation: Factors influencing the willingness to provide care].

INTRODUCTION: The proportion of people in need of care will continue to rise in Germany. In 2019, the majority of people in need of care was cared for at home. Reconciling caregiving and work poses a heavy burden for many caregivers. Therefore, a monetary compensation for care is being discussed politically in order to facilitate the reconciliation of work and care. The aim of this study was to investigate whether and under which circumstances a sample of the German population is willing to care for a close relative. A particular focus was placed on the willingness to reduce working hours, the

Trapped in housing insecurity: Socioecological barriers to housing access experienced by intimate partner violence survivors from marginalized communities.

Homelessness and intimate partner violence (IPV) do not impact all communities equally. Survivors from marginalized communities-that is communities that have been historically and structurally excluded from social, economic, and political resources-face additional challenges weathering IPV and housing crises. Understanding the housing experiences of marginalized survivors is necessary to achieve housing equity for all survivors. Community-based participatory research methods were utilized to convene 14 listening sessions (7 primary/7 validation) with Black and Latinx IPV survivors with interse

The Use of Five Public Health Themes in Understanding the Roles of Misinformation and Education Toward Disparities in Racial and Ethnic Distribution of COVID-19.

The distribution of coronavirus disease 2019 (COVID-19) infection across the historically marginalized populations in the United States (US) has consistently been inequitable. In addition, systemic racism and prejudice, which have existed for decades, have caused a lack of faith in public health and medical experts and have resulted in the epidemic of misinformation. To counteract the COVID-19 pandemic and widespread misinformation, the political establishment and public health experts must work collaboratively. And because they are closely associated, there had been a significant increase in

Bayes Lines Tool (BLT): a SQL-script for analyzing diagnostic test results with an application to SARS-CoV-2-testing.

The performance of diagnostic tests crucially depends on the disease prevalence, test sensitivity, and test specificity. However, these quantities are often not well known when tests are performed outside defined routine lab procedures which make the rating of the test results somewhat problematic. A current example is the mass testing taking place within the context of the world-wide SARS-CoV-2 crisis. Here, for the first time in history, laboratory test results have a dramatic impact on political decisions. Therefore, transparent, comprehensible, and reliable data is mandatory. It is in the

Hypertension in Low- and Middle-Income Countries.

In recent decades low- and middle-income countries (LMICs) have been witnessing a significant shift toward raised blood pressure; yet in LMICs, only 1 in 3 are aware of their hypertension status, and ≈8% have their blood pressure controlled. This rising burden widens the inequality gap, contributes to massive economic hardships of patients and carers, and increases costs to the health system, facing challenges such as low physician-to-patient ratios and lack of access to medicines. Established risk factors include unhealthy diet (high salt and low fruit and vegetable intake), physical inactivi

Effects of socio-economic factors on research over systemic sclerosis: an analysis based on long time series of bibliometric data.

BACKGROUND: Systemic sclerosis (SSc) is a rare detrimental disease warranting global research efforts. Evaluating how socio-economic factors impact country research output on SSc could help to identify solutions advancing research. METHODS: Publication production on SSc during 1969-2018 and data for structural and policy factors for WHO member countries were collected from public sources. Associations between SSc research output and country-level factors were investigated through panel regression. Difference-in-differences analysis further assessed the causal effects of rare disease legislati

The 4 Youth by Youth (4YBY) pragmatic trial to enhance HIV self-testing uptake and sustainability: Study protocol in Nigeria.

BACKGROUND: The World Health Organization recommends HIV self-testing (HIVST) as an additional approach to HIV testing and the Nigerian government is supportive of this policy recommendation. However, effectively increasing uptake and sustainability among Nigerian youth is unknown. The goal of this study is to conduct a full-powered type I hybrid effectiveness-implementation trial to test the effectiveness of youth-friendly implementation science strategies in increasing uptake and sustainability of HIVST led by and for Nigerian youth. METHODS: Our 4 Youth by Youth (4YBY) strategy combines fo

Impact of risk assessment on judges' fairness in sentencing relatively poor defendants.

OBJECTIVE: Use of risk assessment instruments in the criminal justice system is controversial. Advocates emphasize that risk assessments are more transparent, consistent, and accurate in predicting re-offending than judicial intuition. Skeptics worry that risk assessments will increase socioeconomic disparities in incarceration. Ultimately, judges make decisions-not risk assessments. This study tests whether providing risk assessment information interacts with a defendant's socioeconomic class to influence judges' sentencing decisions. HYPOTHESES: Tentatively, socioeconomic status was expecte

The rise and fall of viticulture in the Late Antique Negev Highlands reconstructed from archaeobotanical and ceramic data.

The international scope of the Mediterranean wine trade in Late Antiquity raises important questions concerning sustainability in an ancient international economy and offers a valuable historical precedent to modern globalization. Such questions involve the role of intercontinental commerce in maintaining sustainable production within important supply regions and the vulnerability of peripheral regions believed to have been especially sensitive to environmental and political disturbances. We provide archaeobotanical evidence from trash mounds at three sites in the central Negev Desert, Israel,
