[Clinically relevant breath tests in gastroenterological diagnostics--recommendations of the German Society for Neurogastroenterology and Motility as well as the German Society for Digestive and Metabolic Diseases].

H (2)- and (13)C-breath tests are valuable non-invasive diagnostic tools for gastroenterological diseases. H (2)-breath tests are clinically established for the diagnosis of carbohydrate intolerance resulting from malabsorption (H (2)-breath tests with lactose, fructose, saccharose, sorbitol), of bacterial overgrowth (glucose H (2)-breath test) and for measurement of orcoceal transit time (lactulose H (2)-breath test). The (13)C-urea breath test is regarded as the "gold standard" procedure for the diagnosis of Helicobacter pylori infection. Moreover, (13)C-breath tests for measurement of gastr

[Adult Refsum disease. A retinal dystrophy with therapeutic options].

Adult Refsum disease is one of the few forms of tapetoretinal degenerations accessible for therapy. The disease is characterized by an elevated plasma phytanic acid level and high concentrations of phytanic acid in a variety tissues. Beside tapetoretinal degeneration, additional symptoms are chronic polyneuropathy, cerebellar ataxia, sensorineural hearing loss, anosmia, ichthyosis, skeletal malformations, and cardiac abnormalities. A diet low in phytanic acid leads to an amelioration of polyneuropathy and ataxia and slows or even stops the other manifestations. This beneficial effect of dietar

[Adjuvants and additives in vaccines--medical relevance].

Side effects of vaccinations can have different causes. Substances admixed to vaccines may produce allergic or toxic reactions. The significance and importance of causal associations is discussed in this paper. A table is added listing the most important substances in vaccines, such as inactivating substances, preservatives, stabilizers, adjuvants, and residual substances derived from production processes. Among possible allergic reactions, type I reactions, as the most undesirable ones, should be avoided. In this respect, vaccines against yellow fever are the most important ones. With respect

Treatment of inflammatory dilated cardiomyopathy and (peri)myocarditis with immunosuppression and i.v. immunoglobulins.

OBJECTIVES: Treatment objectives in inflammatory dilated cardiomyopathy (DCMi), myocarditis (M) and peri(myo)carditis are 1) the elimination of inflammatory cells from the myocardium and pericardium, 2) the elimination or (second best) mitigation of B-cell products such as antibodies and immuncomplexes directed against cardiac epitopes such as sarcolemmal, fibrillary and mitochondrial epitopes, and 3) the eradication of the causative viral or microbial agent, if present. ANTIPHLOGISTIC TREATMENT: A "non-specific" anti-inflammatory treatment in peri(myo)carditis can be carried out with antiphl

Survey of the use of selective culture for Neisseria gonorrhoeae in specimens from the female genital tract sent by general practitioners to a microbiology laboratory.

A retrospective survey of the number of cultures found to be positive for Neisseria gonorrhoeae in genital specimens from female patients sent by general practitioners (GPs) over a three-year period was carried out. The organism could be detected in only four specimens out of over 28,000 specimens sent. Specifically, additional selective culture for N gonorrhoeae had been carried out in 8529 of these specimens. An estimate of the cost savings achievable if this laboratory was no longer to culture routinely for N gonorrhoeae was made. GPs should be aware of their local laboratory's normal pract

An analysis of decisions by European general practitioners to admit to hospital patients with lower respiratory tract infections. The European Study Group of Community Acquired Pneumonia (ESOCAP) of the European Respiratory Society.

BACKGROUND: The purpose of this study was to identify factors on which European general practitioners (GPs) base their decisions to admit to hospital patients with lower respiratory tract infections (LRTI). METHODS: A survey was carried out from December 1993 to January 1994 to identify factors that affect GPs' decisions to admit to hospital patients with LRTI by collecting data on 2056 patients from 605 GPs in France, Germany, Italy, Spain, and the UK. RESULTS: Only 93 (4.5%) of the patients included in the study were admitted to hospital. Univariate analysis showed that age > 60 years, ins

Open-access upper alimentary endoscopy.

During 11 years of an open-access endoscopy service for general practitioners the proportion of positive diagnoses has remained relatively constant. There has been a progressive fall in the incidence of duodenal ulcers but an increase in oesophagitis which is now the single most commonly detected abnormality. The number of barium meal requests has fallen by approximately 50%. A study of cimetidine prescribing suggests that use of the open-access service results in fewer inappropriate prescriptions.

Examining variations in the prevalence of hazardous opioid prescribing across general practices in England: a cross-sectional study.

BACKGROUND: Prescribed opioids are potent analgesics associated with high safety risks due to their adverse effects, drug-drug and drug-disease interactions and potential for dependency. To support the implementation of prescribing indicators for further interventions, this study examined the prevalence of different types of potentially hazardous opioid prescribing (PHOP) in general practices across England and investigated underlying factors and variation between practices. METHODS: We conducted a cross-sectional study focusing on adults (aged ≥18 years) at risk of triggering 17 PHOP indicat

Development of institutional guidelines for menstrual suppression and abnormal uterine bleeding in patients undergoing intensive chemotherapy for hematologic malignancies.

PURPOSE: Bleeding complications significantly contribute to morbidity and mortality of patients with hematologic malignancies. Although there is some evidence for treatment and prevention of uterine bleeding in patients with cancer, there is no specific guidance used universally for hematologic malignancies. This study evaluated prescribing patterns of agents used for treatment and prevention of uterine bleeding in patients receiving intensive chemotherapy for hematologic malignancies to provide guidance for a standardized approach for treatment and prevention of uterine bleeding within a sing

Global onychomycosis management and the challenges of antifungal stewardship.

PURPOSE: Onychomycosis, in contrast to other routine superficial dermatophyte infections, is difficult to treat and poor outcomes are not unusual. Few guidelines for onychomycosis management have been published, and these vary considerably from region to region. To meet global antifungal stewardship (AFS) goals, there must be global management guidelines for clinicians to follow in their onychomycosis practice. We aimed to survey current practices for onychomycosis management across a variety of global regions and compare practices to published treatment guidelines, as well as to what degree t

Teaching Opioid Tapering Through Guided Instruction.

BACKGROUND AND OBJECTIVES: Given the current opioid epidemic, national practice guidelines and many state laws are shifting the treatment paradigm for chronic, noncancer pain to a judicious use of opioids. This has prompted a need to teach family medicine residents how to appropriately taper opioids. We created a multifaceted approach to integrate teaching of opioid tapering into a family medicine curriculum with an emphasis on guided instruction. We assessed the degree to which this curriculum affected guideline-concordant opioid prescribing within the family medicine practice. METHODS: A re

Home oxygen therapy: re-thinking the role of devices.

A range of devices are available for delivering and monitoring home oxygen therapy (HOT). Guidelines do not give indications for the choice of the delivery device but recommend the use of an ambulatory system in subjects on HOT whilst walking. Areas covered: We provide a clinical overview of HOT and review traditional and newer delivery and monitoring devices for HOT. Despite relevant technology advancements, clinicians, faced with many challenges when they prescribe oxygen therapy, often remain familiar to traditional devices and continuous flow delivery of oxygen. Some self-filling delivery-

Improving Guideline-Based Streptococcal Pharyngitis Testing: A Quality Improvement Initiative.

BACKGROUND AND OBJECTIVES: Acute pharyngitis is a common diagnosis in ambulatory pediatrics. The Infectious Diseases Society of America (IDSA) clinical practice guideline for group A streptococcal (GAS) pharyngitis recommends strict criteria for GAS testing to avoid misdiagnosis and unnecessary treatment of children who are colonized with group A Streptococcus. We sought to improve adherence to the IDSA guideline for testing and treatment of GAS pharyngitis in a large community pediatrics practice. METHODS: The Model for Improvement was used, and iterative Plan-Do-Study-Act cycles were comple
