More evidence or stronger political will: exploring the feasibility of needle and syringe programs in Ukrainian prisons.

INTRODUCTION: In 2007, the World Health Organization (WHO) recommended for prison authorities to introduce prison needle and syringe programs (PNSP) if they have any evidence that injecting drug use is taking place in prisons. This article presents descriptive evidence that injecting drug use takes place in Ukrainian prisons, it discusses how (denial of) access to injection equipment is regulated in the current system and what changes should be considered in order to implement PNSP. BACKGROUND: Ukrainian prisons still live by the laws and policies adopted in the Soviet Union. Besides laws and…

Identifying research priorities to improve the health of incarcerated populations: results of citizens' juries in Australian prisons.

Health disparities in incarcerated populations should guide investment in the health care and research of these communities. Although users of health-care services are important in providing input into decisions about research, the voices of people in prison are absent regarding research into their health. In this Health Policy paper, we present priorities for research into the health of people in prison according to people in prison themselves. By use of a deliberative research approach, citizens' juries were conducted in six prisons (three men's and three women's prisons) in Australia. Parti…

An opt-out approach to hepatitis C virus testing in English prisons: the literature underpinning the policy.

BACKGROUND: The World Health Organization's aim to eliminate hepatitis C virus (HCV) infection as a public health threat by 2030 is dependent on testing people. HCV prevalence is higher in prisons, so to increase test uptake an 'opt-out' approach to blood-borne virus testing in English and Welsh prisons was introduced. AIMS: This literature review examines the evidence behind the introduction of this public health policy. METHODS: Four healthcare databases were searched for publications between January 2000 and February 2020 on the opt-out approach to blood-borne virus testing in prisons. F…

Decisional considerations for methadone uptake in Kyrgyz prisons: The importance of understanding context and providing accurate information.

BACKGROUND: The Kyrgyz Republic (Kyrgyzstan) is one of few countries in Eastern Europe and Central Asia to provide methadone within its prisons, but uptake of this program has been suboptimal, in part because methadone uptake may have personal or social risks and consequences. Decision aids are evidence-based strategies that are designed to inform the patient's choice by objectively providing information that incorporates patient preferences. METHODS: We conducted qualitative interviews in Kyrgyz and Russian with currently and formerly incarcerated people (n = 36) in Kyrgyzstan from October 2…

Medical Isolation and Solitary Confinement: Balancing Health and Humanity in US Jails and Prisons During COVID-19.

In the face of the continually worsening COVID-19 pandemic, jails and prisons have become the greatest vectors of community transmission and are a point of heightened crisis and fear within the global crisis. Critical public health tools to mitigate the spread of COVID-19 are medical isolation and quarantine, but use of these tools is complicated in prisons and jails where decades of overuse of punitive solitary confinement is the norm. This has resulted in advocates denouncing the use of any form of isolation and attorneys litigating to end its use. It is essential to clarify the critical dif…

[Health care delivery and psychiatric hospitalizations in the prisons of the North of France: An observational study].

BACKGROUND: In French prisons, psychiatric care for inmates is organized into three levels: ambulatory care within each jail in "unités sanitaires en milieu pénitentiaire" (USMP: sanitary units in correctional settings), day hospitalizations in the 28  services médico-psychologiques régionaux (SMPR, "regional medical-psychological services") and full-time hospitalizations in one of the nine "unités d'hospitalisation spécialement aménagées" (UHSA: specially equipped hospital units). Despite high prevalence of mental disorders among French prisoners, the efficiency of these specialized psychiatr…

Early Effects of COVID-19 on Programs Providing Medications for Opioid Use Disorder in Jails and Prisons.

OBJECTIVES: To describe how the novel coronavirus (COVID-19) pandemic has affected opioid agonist treatment (OAT) programs in jails and prisons. METHODS: In May 2020, we conducted an online survey of 19 carceral systems that provided methadone and/or buprenorphine treatment for incarcerated populations before COVID-19. Eleven survey items examined challenges and changes to these programs as a result of the pandemic. Sixteen of 19 programs (84%) responded to the survey. RESULTS: Ten out of 16 systems reported downsizing their OAT programs. Seven of 16 systems made changes to medication dispen…

The use of 'tie down' in New Zealand prisons-what is the role of the health sector?

We draw upon two recent reports from the Chief Ombudsman that describe the prison management of people assessed at risk of self-harm or suic … a 100-bed mental health facility. We call for the use of 'tie down' to be abolished in New Zealand prisons, and for all health professionals to refuse to participate in this practice. Government must make provision for sufficient forensic mental health capacity and capability in the health sector, and ensure timely, equitable access to high-quality, trau …

A Qualitative Inquiry About Pruno, an Illicit Alcoholic Beverage Linked to Botulism Outbreaks in United States Prisons.

OBJECTIVES: Since 2011, 3 outbreaks of botulism in US prisons have been attributed to pruno, which is an alcoholic beverage made by inmates. Following 1 outbreak, we conducted a qualitative inquiry to understand pruno brewing and its social context to inform outbreak prevention measures. METHODS: We interviewed staff, inmates, and parolees from 1 prison about pruno production methods, the social aspects of pruno, and strategies for communicating the association between botulism and pruno. RESULTS: Twenty-seven inmates and parolees and 13 staff completed interviews. Pruno is fermented from wa…

Exploring Medication Safety in Transitions From Prison to Community: A Qualitative Study.

BACKGROUND: The prison population in England and Wales exceeds 88,000, with a high turnover - 47% of sentenced admissions in 2023 served less than 12 months. Transitions from prison to the community are recognised as high-risk periods for medication-related harm, driven by complex health needs, short custodial stays, and fragmented healthcare systems. While national and international guidance exists to support safe medication management, implementation during prison-community transitions remains inconsistent, and evidence on both the drivers of unsafe medication practices and potential solutio

Public Support for Incarceration Declines With Exposure to Evidence That Prison Increases Reoffending.

How might public sentiment respond to information that incarceration may not always achieve one of its core goals of deterring crime or that it could even, perversely, increase reoffending? Across three experiments, participants provided prison sentence length recommendations for a violent or nonviolent criminal case vignette. Before doing so, they were randomized to view either criminogenic evidence (incarceration increases reoffending), deterrent evidence (incarceration decreases reoffending), "no-effect" evidence (incarceration has no effect on reoffending), or no evidence at all. We predic

Promoting Prison Reform through Collaborative Research: A Case Study in Chile.

Prison systems in Latin America currently present various problems that include severe overcrowding, high levels of violence, scarce access to reintegration programs, and high recidivism, among others. In this precarious context, the effective reintegration of incarcerated people becomes very problematic and unlikely since, upon leaving prison, people face various barriers that are combined with a series of accumulated disadvantages that were amplified by the experience of incarceration. However, a somewhat forgotten prerequisite for effective reintegration processes is the development of evid
