Rethinking neurophysiology of delayed onset muscle soreness: updated concepts and mechanisms for mitigation across endurance, resistance, and plyometric exercise.

UNLABELLED: Delayed-onset muscle soreness (DOMS) is a physiological response to intense eccentric exercise across endurance, resistance, and … training causes strength loss with Group III/IV nociceptor activation. Plyometric exercise disrupts proprioception alongside C-fiber sensitization. Recovery interventions show DOMS reduction: compression garments SMD = -0.44, acupuncture SMD = -0.89, foam rolling SMD = -0.34, but majority of studies lack extractable effect sizes preventing meta-analysis. Interve …

Prosthodontic Rehabilitation of a Patient with Missing Teeth and Loss of Vertical Dimension Using Telescopic Overdentures.

Overdentures are one of the most sought-after treatment options for patients with few remaining natural teeth. They are desired as they tend to preserve the natural teeth and its intact periodontium. This helps in reducing the residual ridge resorption and preserve proprioception. These characteristics offer better masticatory ability in patients rehabilitated with overdentures when compared to conventional complete dentures. Enhancements such as telescopic denture for better retention prove to be efficient clinically. Telescopic dentures enable frictional type of retention. This is a case rep…

Progressive Myelomalacia in a Pomeranian Following Spinal Fracture and Surgical Stabilization-A Case Report.

A 4-year-old female spayed Pomeranian presented for being unable to use its pelvic limbs after a fall. The dog was paraplegic with absent pelvic limb nociception and a Schiff-Sherrington posture. Radiographs and Computed tomography showed a T11-T12 luxation. Spinal stabilization was performed for pain control. Twenty-four hours following surgery the patient lost pelvic limb reflexes. Twelve hours later she lost cutaneous trunci and forelimb proprioception and had increased respiratory effort. A diagnosis of progressive myelomalacia was made and the patient was euthanized. Most cases of progres…

[Complaints of imbalance in elderly people].

Balance disorders in the elderly often have several contributing causes. The search for these causes focuses on vision, proprioception, coordination and medication. The peripheral vestibular system is often overlooked. This is probably due to the fact that most clinicians overlook the vestibular system, when complaints of vertigo are missing. However, dysfunction of the vestibular system may cause imbalance without vertigo. Three cases are presented. One case illustrates several contributing causes leading to imbalance. Two other cases illustrate causes of vestibular dysfunction resulting in i…

Treatment options for patellofemoral instability in sports traumatology.

Patellofemoral instability not only involves lateral patellar dislocation, patellar mal-tracking or subluxation but can also cause a limiting disability for sports activities. Its underlying causes are known as morphological anomalies of the patellofemoral joint or the mechanical axis, femorotibial malrotation, variants of the knee extensor apparatus, and ligamentous insufficiencies often accompanied by poor proprioception. Athletes with such predisposing factors are either suffering from unspecific anterior knee pain or from slightly traumatic or recurrent lateral patellar dislocation Treatme…

Intracranial haemorrhage in a dobermann puppy with von Willebrand's disease.

Neurological examination of a lethargic, ataxic 12-week-old dobermann revealed decreased conscious proprioception in all its limbs. Haematological examination revealed a low platelet count. Cytological examination of a sample of cerebrospinal fluid revealed evidence of haemorrhage and chronic inflammation. The levels of von Willebrand's factor antigen were extremely low. Skull radiographs were consistent with mild hydrocephalus. Treatment resulted in little clinical improvement and the animal was euthanased. Post mortem examination of the brain revealed an internal hydrocephalus with haemorrha…

Brown-Sequard syndrome following heroin injection.

Reported is a case of Brown-Sequard syndrome following attempted heroin injection into the right external jugular vein. A right-sided hemiparalysis with a contralateral sensory loss of touch, pain, proprioception, and temperature developed over several hours to the C3 dermatome level. A myelogram showed a vasculitis pattern in the lower cervical region. Treatment was with high-dose dexamethasone for ten days. After six weeks of inpatient physical therapy, only minimal motor and sensory return was seen. Although this syndrome is usually due to lateral hemisection of the spinal cord by a stab wo…

A topodiagnostic investigation on body lateropulsion in medullary infarcts.

Body lateropulsion may occur without signs of vestibular dysfunction and vestibular nucleus involvement. The authors examined 10 such patients with three-dimensional brainstem mapping. Body lateropulsion without limb ataxia reflected an impairment of vestibulospinal postural control caused by a lesion of the descending lateral vestibulospinal tract, whereas body lateropulsion with limb ataxia was probably the consequence of impaired or absent proprioceptive information caused by a lesion of the ascending dorsal spino-cerebellar tract.

The neck-tongue syndrome.

The neck-tongue syndrome, consisting of pain in the neck and altered sensation in the ipsilateral half of the tongue aggravated by neck movement, has been attributed to damage to lingual afferent fibres travelling in the hypoglossal nerve to the C2 spinal roots. The lingual afferents in the hypoglossal nerve are thought to be proprioceptive. Two further cases of the neck-tongue syndrome are described, the spectrum of its clinical manifestations is explored, and the phenomenon of lingual pseudoathetosis is illustrated as a result of the presumed lingual deafferentation.

Ventral decompression for caudal cervical disk herniation in large- and giant-breed dogs.

Ventral decompression was evaluated as a treatment for cervical disk protrusion in large- and giant-breed dogs. All of the dogs improved after ventral decompression. After a follow-up period averaging 16 months, half of the dogs were clinically normal and the other half were functional pets despite minor residual proprioceptive deficits. Ventral decompression alone appeared to be acceptable treatment for dogs not having other radiographic signs of caudal cervical spondylopathy.

[Diagnosis, prevention and treatment of acute lateral ankle sprains].

Acute lateral ankle sprain is a frequent musculoskeletal injury, especially in sports. Most people recover within weeks, however, some have persistent symptoms for years. The Ottawa Ankle Rules can be used for initial investigation of a fracture in the ankle. Five days after the injury, a clinical examination should be completed. Both proprioceptive exercise and mechanical support can prevent ankle injuries and are also effective treatment modalities. Only few require surgery.

Lateral ankle instability and revision surgery alternatives in the athlete.

Ankle instability in the athlete is a common problem that is routinely treated non-operatively, with a 90% success rate. With proprioceptive training, preventive equipment (bracing/taping), and closed kinetic chain strengthening, surgery for ankle instability is uncommon. Nonetheless, some athletes present with recurrent ankle instability that, despite work-up and conservative treatment, requires surgical correction. The use of a primary ligament repair (Brostrom procedure) versus augmented (anatomic) reconstructions is discussed in detail in this article.

Neural correlates of kinematic features of passive finger movement revealed by univariate and multivariate fMRI analyses.

Finger movements are associated with a relatively large neural representation. Passive finger movement-externally generated movement without voluntary muscle activity-is a robust approach to investigate the neural representation of kinesthesia and proprioception. While some studies have characterized the neural correlates of passive finger movement, they have relied solely on mass univariate analysis, potentially limiting sensitivity. Additionally, limited consideration has been given to stimulus duration, a factor closely tied to kinematic features such as amplitude and velocity, which more r…

Beyond distal arthrogryposis: refining the phenotypic landscape of PIEZO2-related disorders.

Piezo2 channels are mechanosensors expressed in dorsal root ganglia and Merkel cells, and involved in proprioception and touch sensation. PIEZO2 pathogenic variants cause rare autosomal dominant and recessive disorders. Dominant forms include distal arthrogryposis type 3, 5, and Marden-Walker syndrome, whereas the recessive form corresponds to distal arthrogryposis with impaired proprioception and touch (DAIPT). Given the rarity of these conditions, additional reports are essential to refine their phenotypic characterization. We conducted a multicenter, retrospective study to identify patients…

[Retrograde intramedullary nail fixation for Charcot neuroarthropathy].

UNLABELLED: Charcot arthropathy is defined as an articular degenerative, chronic, progressive disease that affects one or more peripheral joints, develops as a result of a failure in the normal sensory perception (pain and proprioception) in the innervation of joints located in the foot and ankle, is characterized by destructive bone lesions without the presence of infection. Neuropathy is directly associated with diabetes mellitus type 1 and 2. Surgery is indicated when there is severe involvement of soft tissue, foot joints are unstable, at the presence of chronic or recurrent ulcers or when…

C3 and CD47 mediate sensory-motor circuit refinement during spinal cord development.

Overground movement in mammals requires the assembly and refinement of sensory-motor circuits to ensure proper motor control. Although, supernumerary synapses are formed and subsequently pruned in the brain, whether this occurs within spinal sensory-motor circuits remains unclear. Moreover, it is unknown what molecules are involved. Here, we demonstrate the presence of proprioceptive supernumerary synapses forming inappropriate contacts with motor neurons, resulting in miswired immature circuits. Using mouse genetics, neuronal circuit mapping, electrophysiology, and behavioral studies, we demo
