Unenhanced Dual-Layer Spectral-Detector CT for Characterizing Indeterminate Adrenal Lesions.

Background Unenhanced dual-layer spectral-detector CT may facilitate adrenal lesion characterization; however, no studies have evaluated its incremental diagnostic yield for indeterminate lesions (unenhanced attenuation >10 HU) in comparison to that with conventional unenhanced CT. Purpose To determine whether spectral attenuation analysis improves characterization of lipid-poor adrenal adenomas from nonadenomas compared to that with mean attenuation and histogram analysis of conventional CT images. Materials and Methods This retrospective study included patients with indeterminate adrenal les

COVID-19 pneumonia on chest X-rays: Performance of a deep learning-based computer-aided detection system.

Chest X-rays (CXRs) can help triage for Coronavirus disease (COVID-19) patients in resource-constrained environments, and a computer-aided detection system (CAD) that can identify pneumonia on CXR may help the triage of patients in those environment where expert radiologists are not available. However, the performance of existing CAD for identifying COVID-19 and associated pneumonia on CXRs has been scarcely investigated. In this study, CXRs of patients with and without COVID-19 confirmed by reverse transcriptase polymerase chain reaction (RT-PCR) were retrospectively collected from four and o

Cerebral attenuation on single-phase CT angiography source images: Automated ischemia detection and morphologic outcome prediction after thrombectomy in patients with ischemic stroke.

OBJECTIVES: Stroke triage using CT perfusion (CTP) or MRI gained importance after successful application in recent trials on late-window thrombectomy but is often unavailable and time-consuming. We tested the clinical value of software-based analysis of cerebral attenuation on Single-phase CT angiography source images (CTASI) as CTP surrogate in stroke patients. METHODS: Software-based automated segmentation and Hounsfield unit (HU) measurements for all regions of the Alberta Stroke Program Early CT Score (ASPECTS) on CTASI were performed in patients with large vessel occlusion stroke who und

Combined Qualitative and Quantitative Assessment of Low-Attenuation Renal Lesions Improves Identification of Renal Malignancy on Noncontrast Computed Tomography.

OBJECTIVE: The objective of this study was to assess renal lesions measuring less than 20 Hounsfield units (HU) on noncontrast computed tomography (NCT). METHODS: Twenty-one (18.1%) of 116 consecutive pathologically proven renal cell carcinomas measured less than 20 HU on NCT and were compared with 40 confirmed benign cysts also measuring less than 20 HU. All lesions were assessed qualitatively (heterogeneous or homogenous) by 3 blinded readers and quantitatively with commercially available textural analysis software. Finally, a combined assessment was performed. RESULTS: Qualitative assessm

Anterior convex lateral orbital wall: distinctive morphology in Apert syndrome.

Bony malformations of the orbit and alterations to the soft tissue in Apert syndrome contribute to ophthalmic dysfunction. Recognised structural malformation of the sphenoid and ethmoid sinuses, together with corresponding deformities in the anterior and middle cranial base, are characteristic. Our aim was to explore the underlying structural components of disfigurement and the consequent development of the orbit in patients with Apert syndrome over time by studying 18 preoperative computed tomographic (CT) scans of affected patients and 36 scans from controls. Cephalometric measurements relat

Comparison of true unenhanced and virtual unenhanced (VUE) attenuation values in abdominopelvic single-source rapid kilovoltage-switching spectral CT.

OBJECTIVE: To assess the agreement between the true non-contrast (TNC) attenuation values of intra-abdominal structures and attenuation values obtained on virtual-unenhanced (VUE) images based on rapid kVp-switching dual-energy CT. The effects of contrast phase and patient characteristics (e.g., BMI, hematocrit, hemoglobin content) on VUE values were also investigated. METHODS: Ninety four patients who underwent triphasic abdominal CT (liver mass protocol, n = 47; pancreas mass protocol, n = 47) between August 2014 and May 2015 were retrospectively reviewed. Unenhanced series was performed us

Automatic quantification of the myocardial extracellular volume by cardiac computed tomography: Synthetic ECV by CCT.

BACKGROUND: The quantification of extracellular volume fraction (ECV) by Cardiac Computed Tomography (CCT) can identify changes in the myocardial interstitium due to fibrosis or infiltration. Current methodologies require laboratory blood hematocrit (Hct) measurement - which complicates the technique. The attenuation of blood (HUblood) is known to change with anemia. We hypothesized that the relationship between Hct and HUblood could be calibrated to rapidly generate a synthetic ECV without formally measuring Hct. METHODS: The association between Hct and HUblood was derived from forty non-con

Sporadic insulinomas on volume perfusion CT: dynamic enhancement patterns and timing of optimal tumour-parenchyma contrast.

OBJECTIVES: To assess enhancement patterns of sporadic insulinomas on volume perfusion CT (VPCT), and to identify timing of optimal tumour-parenchyma contrast. METHODS: Consecutive patients who underwent VPCT for clinically suspected insulinomas were retrospectively identified. Patients with insulinomas confirmed by surgery were included, and patients with familial syndromes were excluded. Two radiologists evaluated VPCT images in consensus. Tumour-parenchyma contrast at each time point was measured, and timing of optimal contrast was determined. Time duration of hyperenhancement (tumour-pare

Effect of Nasal Obstruction on Continuous Positive Airway Pressure Treatment: Computational Fluid Dynamics Analyses.

OBJECTIVE: Nasal obstruction is a common problem in continuous positive airway pressure (CPAP) therapy for obstructive sleep apnea and limits treatment compliance. The purpose of this study is to model the effects of nasal obstruction on airflow parameters under CPAP using computational fluid dynamics (CFD), and to clarify quantitatively the relation between airflow velocity and pressure loss coefficient in subjects with and without nasal obstruction. METHODS: We conducted an observational cross-sectional study of 16 Japanese adult subjects, of whom 9 had nasal obstruction and 7 did not (cont

On the Automated Segmentation of Epicardial and Mediastinal Cardiac Adipose Tissues Using Classification Algorithms.

The quantification of fat depots on the surroundings of the heart is an accurate procedure for evaluating health risk factors correlated with several diseases. However, this type of evaluation is not widely employed in clinical practice due to the required human workload. This work proposes a novel technique for the automatic segmentation of cardiac fat pads. The technique is based on applying classification algorithms to the segmentation of cardiac CT images. Furthermore, we extensively evaluate the performance of several algorithms on this task and discuss which provided better predictive mo

Portosystemic collateral vessels in liver cirrhosis: a three-dimensional MDCT pictorial review.

PURPOSE: Portosystemic collateral vessels (PSCV) are a consequence of the portal hypertension that occurs in chronic liver diseases. Their prognosis is strongly marked by the risk of digestive hemorrhage and hepatic encephalopathy. MATERIALS AND METHODS: CT was performed with a 16-MDCT scanner. Maximum intensity projection and volume rendering were systematically performed on a workstation to analyze PSCV. RESULTS: We describe the PSCV according to their drainage into either the superior or the inferior vena cava. In the superior vena cave group, we found gastric veins, gastric varices, esop

Trapped on the "whirl": diagnostic sign on emergency CT.

The "whirl sign" is an uncommon finding on emergency CT. However, it is easy to overlook if not kept in mind. Its recognition is of capital importance, being most of its causes potentially lethal. Surgical treatment is also mandatory when signs of complication are found. The whirl sign is usually found associated to midgut, cecal and sigmoid volvulus, small-bowel volvulus and closed-loop obstructions, and post-surgical mesenteric windows (including retroanastomotic hernias). CT is an optimal imaging technique to depict the so-called sign and associated CT features suggesting complication (circ

CT perfusion-derived mean transit time predicts early mortality and delayed vasospasm after experimental subarachnoid hemorrhage.

BACKGROUND AND PURPOSE: There are limited indicators available to predict cerebral vasospasm in patients with subarachnoid hemorrhage (SAH). The purpose of this study was to determine if CT perfusion-derived hemodynamic parameters are predictors of vasospasm severity and outcome after experimental SAH. MATERIALS AND METHODS: SAH was induced in 25 New Zealand white rabbits. Cerebral blood flow (CBF), cerebral blood volume (CBV), and mean transit time (MTT) were measured with CT perfusion before SAH, within 1 hour after SAH, and on days 2, 4, 7, 9, and 16 after SAH. Basilar artery diameter, mea

Lumbar spinal canal morphometry from computed tomography scans: reproducibility, results and clinical implications.

The reproducibility of measurements of the lumbar spine taken from computed tomography (CT) scans was assessed. The overall variaton in 53 patients scanned on more than one occasion was only 2.8%. Factors which reduced the reproducibility were increasing angle of scan from the mid-discal plane, measurement of oblique structures and measurement of dimensions with soft-tissue boundaries. Measurements from 142 patients are presented and compared with both other published series and the results from 17 patients with spinal stenosis. Whereas the sagittal diameter of the bony canal and the interpedi

Phonation Analysis Combined with 3D Reconstruction of the Thyroarytenoid Muscle in Aged Ovine Ex Vivo Larynx Models.

OBJECTIVE: The aim of the study was to establish a basic data set of combined functional and anatomical measures of aged sheep larynges using ex vivo models. Combining these two approaches in one and the same larynx is an unmet goal so far yet is important as newer treatment strategies aim to preserve the organ structure and new assessment tools are required. Ovine larynges were used as their dimensions, and muscle fiber type distribution highly resemble the human larynx. STUDY DESIGN: Ex vivo animal study. METHODS: Larynges of six sheep (~9 years of age) were subjected to ex vivo functional

MR imaging of the uterus and cervix in healthy women: determination of normal values.

The purpose of this study was to establish normal values for the volume of the uterus and cervix in MRI based on age and the menstrual cycle phase. We performed MRI of the pelvis in 100 healthy women. For the uterus, they were further divided into two groups: one with myomas and/or adenomyosis and one without either. The volume of the uterus and cervix and thickness of the uterine wall layers were analysed by age and the menstrual cycle phase. The mean volume of the uterus in both groups and the cervix significantly increased with age to reach its peak at 41-50 years, and then dropped. Likewis
