BACKGROUND: Since the introduction of megavoltage radiation therapy radiation osteitis has become a rare event and may be easily mistaken for bone metastases. A case of radiation osteitis is reported and diagnostic features are discussed.
CASE REPORT: A 70 year-old female patient underwent rectum resection for rectum cancer and was given standard adjuvant therapy consisting of irradiation of the tumor site and lymph nodes in the pelvis with 18 MeV photons, boxfield technique up to 50.4 Gy and chemotherapy with 5-FU. Two hundred patients with well-differentiated adenocarcinomas of the rectum stages T1, T2 (113 T1, 87 T2) were accrued in two French institutions (Lyon-Sud and Dijon). Treatment consisted of intracavitary 50 kV X-rays (90-120 Gy in 3-4 fractions and 4-6 weeks). A 20-30 Gy interstitial brachytherapy boost was given after 2-3 fractions when a limited infiltration of the rectal wall was present. A study of regional lymph node metastases was performed using the clearing method in 322 patients with carcinoma of the colon and rectum (140 with carcinoma of the colon and 182 with carcinoma of the rectum) who had undergone surgical resection. One hundred and nineteen operative specimens of carcinoma of the rectum were prospectively studied to evaluate distant microscopic intramural spread with respect to gross tumoral margins and to determine if the degree of spread was related to involvement of the lymph nodes. Sophisticated therapies for rectal carcinoma, such as sphincter preserving operations, pouch-anal anastomosis, preoperative irradiation or adjuvant chemotherapy, require an exact pretherapy assessment of the tumour and its lymph node involvement. Reported is a case of a 73-year-old male with a history of ulcerative colitis that had started at the age of 57. In 1985, on receiving a barium enema, a polypoid lesion was found in his rectum. In 1986, the results of a colonoscopy showed that the polypoid lesion had reached an IIa-aggregated elevation, and biopsies of this lesion were diagnosed as an adenoma or a hyperplastic polyp. Two hundred seventy-six patients with positive findings on a stool guaiac test, hematochezia, a change in bowel habits, or a family history of colorectal carcinoma were referred to the colorectal clinic for further investigation. Colorectal mucosa from patients with colorectal carcinoma was compared with rectal mucosa from a control group. It was found that the LDH isoenzyme pattern of uninvolved mucosa proximal and distal to the carcinoma differed from that of normal tissue and resembled that of carcinoma tissue except in the case of the uninvolved mucosa proximal to the carcinoma of the sigmoid colon, which did not differ from normal mucosa.
