Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2012eng
PURPOSE: The main purpose of this study was to evaluate the accuracy of preoperative magnetic resonance imaging (MRI) in detecting subscapularis tears identified during the gold standard of arthroscopy and determine whether MRI can reliably predict which patients have subscapularis tears. A second purpose was to determine whether magnetic resonance (MR) arthrograms could better identify a subscapularis tear than conventional MRI.
METHODS: This was a retrospective study evaluating 39 consecutive patients (40 shoulders) who had a preoperative 1.5-T MRI study and underwent an arthroscopic subsca

Anatomic factors, such as a hooked acromion, have been associated with rotator cuff disorders. Orientation of the glenoid relative to the scapula has been implicated in shoulder instability, but it has not been linked to rotator cuff disorders. The purpose of the current study was to test the hypothesis that superior inclination of the glenoid is associated with full-thickness rotator cuff tears. Glenoid inclination angles were measured from 16 shoulder radiographs of a convenience sample of eight cadavers in which one shoulder had an intact rotator cuff and the other shoulder had a full-thick

A cadaver study was performed to determine the effect of arm position and capsular release on rotator cuff repair. Artificial defects were made in the rotator cuff to include only the supraspinatus (small) or both supraspinatus and infraspinatus (large). The defects were repaired in a standard manner with the shoulder abducted 30 degrees at the glenohumeral joint. Strain gauges were placed on the lateral cortex of the greater tuberosity and measurements were recorded in 36 different combinations of abduction, flexion/extension, and medial/lateral rotation. Readings were obtained before and aft

Overuse injuries are a major cause of pain, performance decline, and time loss in professional athletes. Unlike acute traumatic injuries, these conditions develop gradually because of repetitive submaximal loading and inadequate recovery. Recurrent mechanical stress may lead to microstructural damage involving tendons, bones, cartilage, ligaments, and surrounding soft tissues. Overhead athletes, runners, and jumping athletes are particularly susceptible to chronic overload syndromes, including rotator cuff tendinopathy, patellar tendinopathy, Achilles tendinopathy, and stress fractures. This study reports on 9 patients with obstetrical brachial plexus injury who underwent a latissimus dorsi (LD) transfer to reconstruct external rotation of the upper extremity. Transfer of the LD to the rotator cuff is widely used for restoring shoulder abduction and external rotation in patients with obstetrical brachial plexus injury. Patients were classified according to type of paralysis, age of surgery, and shoulder function. This retrospective article evaluated the results of the LD transfer in a group of 9 patients. The study shows that in the near future, profit can be made in active a

This review explores the impact of osteoporosis on arthroscopic rotator cuff repair and subsequent tendon healing, focusing on challenges such as anchor fixation failure, intraoperative fractures, and limited surgical visibility. It examines how osteoporosis disrupts the tendon healing microenvironment post-surgery through mechanisms involving bone metabolism, growth factors, the immune system, sex hormones, oxidative stress, and adipose infiltration. Effective surgical planning is crucial to mitigate the adverse effects of osteoporosis on rotator cuff repair. This review offers recommendation

UNLABELLED: In some patients with persistent pain around the Long Head of Biceps Tendon (LHB), lesions of the "biceps reflection pulley", associated with biceps tendinitis, can be found. The aim of this study is to describe the anatomy of the reflection pulley and the arthroscopic appearance of the pulley lesion. Therapeutic options will be discussed.
METHOD: The results of a former histo-anatomic study done by the authors in 13 cadaver shoulders are compared with the intra-operative findings in patients with persistent tendinitis of the LHB together with a lesion of the reflection pulley and

Multiple factors influence rehabilitation strategies after rotator cuff repair. These variables may also impact the overall success of the surgical intervention. Physicians and rehabilitation specialists should be aware of prognostic indicators that can provide therapeutic guidance and offer insights into eventual clinical outcomes. The success of surgical and rehabilitative interventions is often evaluated in terms of patient-reported outcome measures, return to activity, and pain. Although these factors are somewhat interdependent, each of them independently influences the final result. This

The subscapularis muscle is the largest and strongest muscle of the rotator cuff. It plays an essential role in the stability and function of the shoulder. Tears of the subscapularis tendon are more frequent than previously estimated. The worldwide use of arthroscopy in rotator cuff surgery has allowed to recognize the true prevalence of subscapularis lesions, as it permits to visualize the articular side where partial tears are usually localized. Subscapularis tears are generally non-traumatic, arising from intrinsic degeneration, subcoracoid and/or anterosuperior impingement. Clinical presen

Tears of the subscapularis were previously believed to be rare, but are now recognized to be very common. As maintenance of the subscapularis footprint is integral to normal biomechanical function of the shoulder it is important for the shoulder surgeon to be adept at recognizing and treating these tears. A combination of physical examination tests can be used to determine both the presence and size of a subscapularis tear. Compared with posterosuperior rotator cuff tears, magnetic resonance imaging detection of subscapularis is less reliable and therefore requires a high index of suspicion. A

BACKGROUND: The purpose of the present study was to evaluate the indications for, and outcomes of, reverse shoulder arthroplasty in patients with massive rotator cuff tears but without glenohumeral arthritis.
METHODS: From December 1998 to December 2006, sixty-nine patients (seventy-two shoulders) were managed with reverse shoulder arthroplasty for the treatment of irreparable rotator cuff dysfunction without glenohumeral arthritis. The indications for reverse shoulder arthroplasty were persistent shoulder pain and dysfunction despite a minimum of six months of nonoperative treatment, the pre

INTRODUCTION: The impact of sex hormones on connective tissue is controversial. Previous investigations found that female estrogen contraceptive therapy use may have simultaneous beneficial and detrimental musculoskeletal health effects, whereas males receiving androgen therapy had an increased Odds Ratio (OR) (3.6) of rotator cuff tears and higher Achilles Tendon injury rates. Furthermore, males receiving androgen therapy had an increased OR (26.7) for rotator cuff revision within a year of primary surgery.
MATERIALS AND METHODS: Our design involves an observational, retrospective study usin

BACKGROUND: Literature regarding surfing injuries is scarce and most studies report a high number of minor injuries. Recent literature suggests that musculoskeletal injuries are more common than previously reported. In the mid 2000s, competitive surfing has seen an increase in aerial maneuvers, as they have become more highly rewarded by the judging panel, which may be contributing to a change in injury patterns. Our goal was to evaluate orthopedic injuries in professional surfers.
HYPOTHESIS: Orthopedic injuries in professional surfers are diverse and have evolved with skill and technology.

HYPOTHESIS: The purpose of this study is to evaluate the muscle activity with surface electromyography (EMG) and the clinical outcome of the latissimus dorsi transfer. It remains unclear whether the clinical results of the latissimus dorsi transfer for massive posterosuperior rotator cuff tears are achieved either by active muscle contractions or by a passive tenodesis effect of the transfer.
METHODS: Eight patients were evaluated preoperatively and at 1 year (SD, 0.1) after the latissimus dorsi transfer. Clinical evaluation of outcomes included active range of motion, Constant score, and vis

Rotator cuff tears (RCTs) often require surgical intervention; however, surgical outcome is frequently insufficient due to poor healing of the tendon-bone enthesis, leading to high retear rates. Previously, the recombinant human amelogenin protein (rHAM+) regenerated injured or diseased musculoskeletal tissues including ligaments and osteochondral injuries. We therefore hypothesized that rHAM+ can also induce regeneration of the biomechanical and structural properties of the enthesis. The right shoulder infraspinatus tendon of 75 rats was fully-transected, and treated either with 0.5 mg/mL rHA

OBJECTIVE: This study aimed to clarify the relationship between the directions of humeral head translation, the presence of acromial or coracoid spurs, and the locations of tendon tears in massive rotator cuff tears. Thirty shoulders from thirty patients with massive rotator cuff tears who underwent reverse shoulder arthroplasty were included. Preoperative 3DCT classified humeral head translation into three groups: minimal type, posterosuperior type, and anterosuperior type. The presence of acromial or coracoid spurs was also assessed. Preoperative MRI and intraoperative findings determined th
