PURPOSE: To review the functional outcome of AO type C distal humeral fractures (bicondylar fractures) managed with dual orthogonal plate fixation.
METHOD: Twenty-three patients were reviewed retrospectively at a mean of 45.1 months. Patient-rated outcomes (Disabilities of the Arm Shoulder and Hand [DASH], Patient Rated Ulnar Nerve Evaluation [PRUNE], American Shoulder and Elbow Surgeons Elbow form [ASES-e], and Short Form-36 [SF-36]), clinical, radiographic, and objective evaluations were used to assess outcomes.
RESULTS: Almost 40% of patients required a third plate for adequate fixation.

BACKGROUND: Proximal humerus fractures (PHFs) are among the most common and challenging fractures due to complex anatomy and variable fracture patterns. While anatomical reduction is critical for successful outcomes, no tool currently predicts the intraoperative difficulty of achieving it. Despite advances in surgical techniques, complications persist. Existing classification systems, such as Codman's, Neer's, AO/OTA, and Mayo-FJD, focus on fracture morphology and displacement but do not address intraoperative complexity. This study aims to develop a new score for the early prediction of intra

The treatment of fracture sequelae following proximal humeral fractures presents a significant challenge, as they range from necrosis and pseudarthrosis to malunions. Classifications such as Boileau's facilitate diagnosis and treatment planning. Type 1 sequelae, characterized by necrosis, can be treated with either anatomical or reverse prostheses depending on the condition of the rotator cuff. Type 2 sequelae involve locked dislocations and may require bony reconstructions. Type 3 pseudarthrosis is managed either with joint-preserving techniques or prosthetic replacement, depending on the deg

INTRODUCTION: While locking plates have markedly improved fixation of proximal humerus fractures, a cohort of fractures remains difficult to treat. This cohort has been identified as fractures with marked medial comminution and varus deformity. Loss of reduction and fixation failure are the most frequently reported complications for this cohort. We report the use of an orthogonal 1/3 tubular plate to augment the proximal humerus locking plate.
METHODS: The subject underwent osteosynthesis for a four-part proximal humerus fracture with medial comminution. Fixation was performed within 24 h of

A technique of removing proximal humerus locking plates using an arthroscopic technique is described. Though technically demanding, benefits include smaller incisions, decreased risk of infection, and the ability to visualize and protect the axillary nerve throughout the course of the procedure. Furthermore, arthroscopy affords the surgeon with the ability to address concomitant intraarticular pathology at the time of surgery.

At three institutions, 71 humeral intramedullary nails were inserted into the shoulder; 67 were reviewed at 6 months and at completion of treatment. Fifty-one utilized the anterior acromial approach and 16 were inserted lateral to the acromion. Shoulder motion was rated as: excellent (asymptomatic and within 15 degrees of normal); good (normal daily function within normal motion); and poor. Nails were also inserted into the humeral diaphysis of eight cadaver shoulders. Fifty-one nails were inserted via the anterior acromial incision; 48 were graded as excellent, one as good, and two with traum

Isolated fractures of the greater tuberosity are uncommon, comprising a minority of proximal humerus fractures. Their management depends primarily on the degree and direction of displacement. Posterior-superior displacement, in particular, is associated with subacromial impingement and compromised rotator cuff function, often necessitating surgical intervention. This case report aims to present the management of an isolated greater tuberosity fracture with posterior-superior displacement in an elderly patient, emphasizing successful fixation with interfragmentary screws, without rotator cuff r

A 28-year-old woman presented with right shoulder pain and, after a delay, a diagnosis of clavicular stress fracture was made. The patient gave no history of repetitive abnormal loading of the upper limbs which could have assisted the diagnosis. The authors wish to raise awareness of this rare diagnosis such that it can be considered in the differential for shoulder pain of unclear origin.

The Neer four-part classification for displaced proximal humeral fractures is the most useful classification so far devised for consistent diagnosis and treatment. Nevertheless, there are several points in the Neer classification that need changing. Group II fractures, i.e., displaced fractures of the anatomical neck have a severe prognosis, excluding a mild graduation as in group II. Displaced four-segment fractures can be classified in group IV as well as in group V, which means the classification less than sharp. Without disregarding Neer's criteria, we use our own simplified classification

PURPOSE: For fractures requiring operative fixation, the "tripod technique" using headless compression screws has recently been described as a less invasive alternative to open reduction and internal fixation with plate and screws. The purpose of this study was to evaluate the clinical and radiographic outcomes of the tripod technique for the treatment of radial head and neck fractures.
METHODS: We performed a retrospective chart review of all radial head and neck fractures treated with the tripod technique at our institution over a 10-year period. Patients with less than 6 months of follow-u

BACKGROUND: Lateral end of clavicle fractures can be a challenge, with a 20% to 30% non-union rate if treated non-operatively. Several operative options exist, each having their own merits and some having potential disadvantages. The Minimally Invasive Acromioclavicular Joint Reconstruction (MINAR®) (Storz, Tutlingen, Germany) set uses an Orthocord (Depuy Synthes Mitek, Leeds, UK) suture and two Flip Tacks (Storz) via a transclavicular-coracoid approach to reconstruct the coracoclavicular ligaments.
METHODS: Referrals were made to two senior surgeons at separate institutions regarding Robinso

BACKGROUND: Fractures of the distal radius are common. As the population of the western world ages, their incidence is set to increase further. There are various methods of treating these fractures, but optimal management remains controversial. In the United Kingdom, the most common surgical treatment of closed distal radius fractures is by Kirschner-wires (K-wires) or volar locking plate. In this study, we compared long-term functional outcomes of volar locking plates with those of K-wires.
METHODS: A retrospective comparative study of 71 patients with dorsally displaced distal radius fractu

Displaced proximal humeral fractures warrant surgical fixation for early rehabilitation and better functional outcome. These fractures are traditionally fixed by delto pectoral surgical approach. Direct lateral approach has recently gained interest as it involves less soft tissue dissection and is particularly helpful in certain fracture patterns. However, there have been concerns of axillary nerve damage with this approach. We report a case series of proximal humerus fractures fixed by direct lateral approach from our institution. All displaced Type 2 and 3 fractures were included in our stud

Pediatric avulsion fractures most commonly occur at sites of secondary ossification and are often associated with chronic stress from repetitive movements. Because of a variety of risk factors, youth athletes place higher stress on ossification centers, and their activities may predispose them to injury. This case report describes a 12-year-old female softball player who presented with pain at the distal acromion, worsened by her overhead throwing motion. Further questioning revealed improper throwing mechanics placing extenuated stress on the shoulder. Plain radiograph imaging showed an avuls

The Disability of the Arm, Shoulder and Hand (DASH) and the Patient-Rated Wrist Evaluation (PRWE) scores appear most frequently in the literature when assessing functional outcome after distal radius fracture. We aimed to evaluate if the two questionnaires correlate. We reviewed 258 cases of adults who sustained a distal radius fracture over a one year period. At mean follow-up of 17 months the disability of the arm, shoulder and hand (DASH) and the patient-rated wrist evaluation (PRWE) scores were recorded. The outcome scores for each group were not statistically different (DASH p = 0.86, PRW

We treated 47 patients with a mean age of 57 years (22 to 88) who had a proximal humeral fracture in which there was a severe varus deformity, using a standard operative protocol of anatomical reduction, fixation with a locking plate and supplementation by structural allografts in unstable fractures. The functional and radiological outcomes were reviewed. At two years after operation the median Constant score was 86 points and the median Disabilities of the Arm, Shoulder and Hand score 17 points. Seven of the patients underwent further surgery, two for failure of fixation, three for dysfunctio

Greater tuberosity (GT) fractures with rotator cuff injuries are challenging due to risks of nonunion and functional loss. We report a case treated with a distal radius locking plate combined with suture anchors. A female patient presented with shoulder pain and immobility 4 weeks after trauma, diagnosed with humeral head dislocation and rotator cuff tear. She underwent minimally invasive fixation using this combined technique, which provides stable fracture reduction while repairing the rotator cuff. At the 18-month follow-up, shoulder function was fully restored with no complications. This c

The rising incidence of proximal humeral fractures has made this an important topic in the treatment reality of trauma surgery. Given the variety of treatment options available, the question arises as to what extent patient age influences the choice of treatment and, consequently, the functional outcome. In our approach the morphological features of the fracture and individual patient characteristics form the foundation for determining whether surgical or conservative treatment should be pursued. Whenever possible, joint-preserving reconstruction should be the goal in younger patients. In olde

This article describes a multifragmentary glenoid fracture in a 65-year-old patient after falling from a height of 2 m. The accident led to complete Y‑shaped destruction of the glenohumeral joint with a large posteroinferior fragment and a large anteroinferior glenoid fragment. The treatment consisted of a two-stage surgical procedure and a combination of open and arthroscopic techniques. First, open surgery of one fragment from the dorsal side was performed using screw osteosynthesis. Arthroscopic refixation of the anterior glenoid fragment (bony Bankart lesion) followed 4 weeks later using a
