Rouleaux mimicking anti-H in molecularly confirmed para-Bombay siblings: a diagnostic pitfall.

The para-Bombay phenotype is a rare H-deficient blood group variant characterized by markedly reduced or absent expression of H on red blood cells (RBCs) because of mutations in FUT1, with variable secretor status influenced by FUT2. The Bombay phenotype is relatively more common in the Indian population, but the para-Bombay phenotype remains exceedingly rare and is frequently underrecognized because of diagnostic challenges during routine serologic testing. We report molecularly confirmed para-Bombay siblings from India in whom rouleaux-induced pseudoagglutination mimicked anti-H reactiv ity.…

Familial isolated 17,20-lyase deficiency in three siblings caused by a CYP17A1 mutation.

SUMMARY: 17α-hydroxylase/17,20-lyase deficiency (17OHD) is a rare form of congenital adrenal hyperplasia, inherited in an autosomal recessive manner. The CYP17A1 gene on chromosome 10 encodes cytochrome P450c17, a single bifunctional enzyme with both 17α-hydroxylase activity (essential for cortisol synthesis) and 17,20-lyase activity (essential for sex steroid synthesis). Mutations in CYP17A1 can impair either or both activities, resulting in a clinical spectrum ranging from combined 17α-hydroxylase/17,20-lyase deficiency to isolated 17,20-lyase deficiency. We report a case of three siblings, …

Compound Heterozygous COA7 Variants Presenting With Childhood-Onset Axonal Neuropathy in 2 Siblings.

OBJECTIVES: Variants in COA7 (cytochrome c oxidase assembly factor 7) are a rare cause of mitochondrial disease, with limited clinical descriptions and phenotypic variability. We describe 2 siblings carrying compound heterozygous COA7 variants, one of which (c.457C>T; p.Leu153Phe) is novel. Both presented with early-onset, slowly progressive axonal sensorimotor neuropathy, with differences in severity and associated features. METHODS: Patients were identified based on clinical presentation and evaluated through longitudinal neurologic, neurophysiologic, genetic, biochemical, and neuroimaging …

Three Yemeni Siblings With Johanson-Blizzard Syndrome: A Case Report and Literature Review.

Johanson-Blizzard syndrome (JBS), also known as UBR1-related disorder, is a very rare autosomal recessive disorder caused by pathogenic variants in the UBR1 gene and characterized by significant phenotypic variability. The condition is known to be mainly characterized by craniofacial abnormalities, exocrine pancreatic insufficiency, growth retardation, and sensorineural hearing loss. We describe three affected siblings from a consanguineous Yemeni family with JBS. Two brothers suffered from profound symptoms resulting in infant death, which included failure to thrive, exocrine pancreatic dysfu…

Severe SPTBN4-Related Neurodevelopmental Disorder: Case Report of Two Siblings with Lethal Outcomes.

Spectrin beta non-erythrocytic 4 (SPTBN4) disorder is a rare yet impactful condition that is also known as neurodevelopmental disorder with hypotonia, neuropathy, and deafness (NEDHND). It is caused by homozygous or compound heterozygous mutations in the SPTBN4 gene, which encodes beta IV (βIV) spectrin primarily found in neurons and pancreatic islets. Here, we report two male siblings from a Syrian non-consanguineous (possibly distantly related) family diagnosed with SPTBN4 disorder. Both patients exhibited severe failure to thrive, hypotonia, dysphagia, and global developmental delay, initia…

Unseen Care, Unequal Worlds: Photovoice Narratives of Siblings Supporting Deafblind Adolescents in Indonesia.

BACKGROUND: Congenital deafblindness is a rare and complex condition characterised by combined hearing and vision impairment, frequently accompanied by additional intellectual, developmental, and psychosocial disabilities. Adolescents with deafblindness often encounter substantial challenges during the transition from school to adulthood, particularly in low-resource settings where formal disability services are limited. While existing research has predominantly focused on parental experiences, the perspectives of siblings-who frequently assume significant caregiving and advocacy responsibilit…

Brooke-Spiegler Syndrome in Japanese Siblings with an R758X Mutation: A Case Report.

INTRODUCTION: Brooke-Spiegler syndrome is a rare hereditary skin disorder characterized by the development of multiple adnexal tumors, such as trichoepitheliomas and cylindromas, with a higher prevalence in females. CASE PRESENTATION: We report a rare case involving two male siblings who developed facial lesions consistent with this condition. Both patients presented with tumors on the nasal ala, which were successfully treated with carbon dioxide (CO2) laser therapy. Genetic testing confirmed the diagnosis of Brooke-Spiegler syndrome, consistent with autosomal dominant inheritance. CONCLUSI…

GNE Mutation-related Congenital Thrombocytopenia in 2 Siblings: Case Reports and Literature Review.

BACKGROUND: GNE mutations are rare pathologic conditions that can cause severe thrombocytopenia and bleeding tendency from the neonatal period. The clinical presentation of patients with GNE mutations varies from mild skin and mucosal bleeding to life-threatening bleeding. CASE PRESENTATION: This study reported two siblings with hereditary thrombocytopenia. The 2 patients exhibited severe thrombocytopenia (platelet [PLT] count: <15,000/mm 3 ) since the neonatal period and did not respond to intravenous immunoglobulin (IVIG) and steroids. The patients required PLT transfusions once every 1 to …

Biallelic Variant in NRDC Gene in Two Siblings With Developmental Delay and Seizures.

We report a biallelic likely pathogenic variant in the NRDC gene in two Iranian siblings with developmental delay, microcephaly, hypotonia, seizures, and absent speech. Exome sequencing (ES) identified a frameshift deletion in exon 15 of NRDC (NM_001101662.2): c.1702_1703del (p.Met568Valfs*2), confirmed to segregate with disease in the family. This is the second report implicating biallelic NRDC gene variants in neurodevelopmental disorders. Our findings expand the phenotypic spectrum and support a potential role for NRDC in severe neurodevelopmental delay.

Novel PNPLA6 Variants: Hypogonadotropic Hypogonadism, Pituitary Hypoplasia, Anosmia, and Cerebellar Atrophy in Siblings.

Hypogonadotropic hypogonadism can stem from many different genetic factors, and the list of implicated genes keeps expanding. PNPLA6-related disorders encompass a spectrum of neuroendocrine and neurodegenerative conditions arising from pathogenic variants of PNPLA6. Two male siblings, aged 22 and 21 years, presented with delayed puberty and absent secondary sexual characteristics. Both had low serum gonadotropin levels and testosterone levels, consistent with hypogonadotropic hypogonadism. Olfactory testing confirmed severe hyposmia. Magnetic resonance imaging (MRI) of the brain showed severel…
