BACKGROUND: Strongyloides stercoralis is a world wide distributed small intestinal nematode parasite. In immunocompetent individuals S stercoralis can produce asymptomatic infections or a moderate clinical picture of diarrhea, some cases become chronic. In immunocompromised patients, a disseminated disease may appear, sometimes fatal. In Chile, there is little epidemiological information about S stercoralis infections and appropriate diagnostic techniques are usually not used.
AIM: To evaluate the yield of an ELISA test for the diagnosis of strongyloidiasis in Chilean patients.

The effects of benzimidazole anthelmintics in murine strongyloidiasis were examined. Thiabendazole 50 mg/kg daily produced a 91% reduction in the numbers of Strongyloides ratti larvae in the feces. A similar suppression was seen when thiabendazole was given during the intestinal phase, but no effect was noted when the drug was administered during the phase of larval migration. Thiabendazole had no effect on larvae in the skin or lungs, did not inhibit maturation of worms, and did not expel adult worms from the gut, but did reduce fecundity of adult worms in the intestines by 84%. Mebendazole a…

Strongyloides stercoralis is a soil-transmitted helminth with an unusual life cycle, causing canine and human strongyloidiasis, mainly endemic in tropical and subtropical areas. Following percutaneous or oral transmission of infective third-stage larvae in the vertebrate host, the parasite can cause autoinfection, leading to life-long infection. At present, the transmammary transmission was only assessed in experimentally infested dogs. Here, we provide observational evidence of S. stercoralis transmammary transmission in puppies suckling from a truffle dog from Central Italy, from where its p…

A 58-year-old, human immunodeficiency virus (HIV) negative, male farmer presented with a right-sided hemorrhagic, exudative, eosinophilic, rapidly reaccumulating pleural effusion. Pleural biopsy revealed secondary deposits from a squamous cell carcinoma. Wet mount film of pleural fluid unexpectedly showed filariform larvae of Strongyloides stercoralis. Our observation suggests that strongyloidiasis should be considered as the etiological cause for pleural effusion of obscure etiology. The threshold for suspicion should be high if the pleural effusion is exudative and eosinophil rich.

Eighteen cases of strongyloidiasis have been diagnosed in Geneva, Switzerland, since the laboratory method of Baërmann was introduced. Digestive disorders were reported by 15 patients, and pulmonary symptoms were present in 4. Six also had various neurological symptoms. Peripheric eosinophilia was above 5% in all cases and above 10% in 14. A single course of treatment with thiabendazoled 17 patients, while the last was cured after a second course. Three of our 18 patients were infected in Geneva and had never travelled abroad.

Strongyloidiasis is a neglected infectious disease with diverse clinical manifestations that may lead to severe complications, including death in immunocompromised individuals. Uncomplicated infections can be diagnosed early using serological testing. Regarding treatment, the World Health Organization recommends ivermectin, thiabendazole, and albendazole as effective therapies against Strongyloides stercoralis. The objective of this study was to evaluate the therapeutic effectiveness of ivermectin 200 μg/kg/day for 2 consecutive days in patients infected with S. stercoralis, with the aim of op…

Human strongyloidiasis is an important neglected tropical disease primarily caused by the nematode Strongyloides stercoralis, and to a lesser extent Strongyloides fuelleborni which mainly infects non-human primates. Zoonotic sources of infection have important implications for control and prevention of morbidity and mortality caused by strongyloidiasis. Recent molecular evidence suggests that for S. fuelleborni, primate host specificity is variable among genotypes across the Old World, and consequently that these types likely vary in their capacity for human spillover infections. Populations o…

OBJECTIVE: The Strongyloides stercoralis hyperinfection syndrome (SHS) may develop in individuals with asymptomatic infection receiving immunosuppressive treatment. This report summarizes current knowledge regarding SHS in patients with systemic lupus erythematosus (SLE) and associated antiphospholipid syndrome (APS).
METHODS: Two patients with active SLE and associated APS presenting with SHS are reported. Additional cases of strongyloidiasis in SLE were identified and reviewed.
RESULTS: Patient 1: A 34-year-old woman with SLE and APS characterized by active glomerulonephritis, stroke, and …

Strongyloides is a unique nematode in its ability to cause a secondary hyperinfection and disseminated disease several years following initial contact. The prevalence of Strongyloides infection has been rising; it is currently considered a global disease, which presents with a broad spectrum of clinical signs and symptoms among patients. This case report focuses on a 67-year-old Caribbean female presenting with severe weight loss, vomiting, early satiety, and mild anemia who was subsequently diagnosed with strongyloidiasis on the basis of a duodenal biopsy pathology report obtained via esophag…

Strongyloidiasis is a neglected disease in Latin America. Gastrointestinal manifestations are nonspecific and duodenal obstruction is a rare complication. Here we present the case of a 31-year-old male from the central jungle of Peru, admitted due to a high intestinal obstruction, with duodenal ulcers and stenosis evidenced in the upper endoscopy. The histopathological report revealed presence of larvae of Strongyloides stercoralis. Clinical and endoscopic follow up were favorable with ivermectin treatment. There are near 20 reported cases of duodenal obstruction due to S. stercoralis. Additio…

Strongyloides stercoralis is a soil-transmitted helminth that is mainly found in the tropical and subtropical regions and affects approximately 600 million people globally. The medical importance of strongyloidiasis lies in its capacity to remain asymptomatic and chronically unnoticed until the host is immunocompromised. Additionally, in severe strongyloidiasis, hyperinfection syndrome and larva dissemination to various organs can occur. Parasitological techniques such as Baermann-Moraes and agar plate culture to detect larvae in stool samples are the current gold standard. However, the sensit…

Strongyloidiasis is prevalent in Thailand, yet its prevalence in the south is lower than in other parts of the country. This might be due to the long rainy season in the south resulting in stool submersion in water inhibiting worm development. In this study, the effect of water submersion of fecal samples on development of Strongyloides stercoralis was investigated. Ten ml of a 1 ∶ 5 fecal suspension were placed in 15-ml tubes, 35-mm dishes, and 90-mm dishes producing the depths of 80 mm, 11 mm and 2 mm-suspensions, respectively. The worm development was followed at 1/6, 4, 6, 8, 10, 12, 14, 1…

A 61-year-old diabetic man had fever and a significant exudative type of eosinophilic pleural effusion. Microscopic examination of stained specimens of bronchial washings revealed filariform larvae of Strongyloides stercoralis. This intestinal nematode is endemic in some areas of the world. Pulmonary symptoms are usually mild, though severe pulmonary involvement may be occasionally seen. Although serohemorrhagic pleural effusion has been well reported in the literature, we found no report of such an unusual case of significant exudative eosinophilic pleural effusion due to strongyloidiasis.

Over the past two decades a wide range of immunological studies have been carried out on organisms responsible for causing serious bacterial and parasitic diseases in Thailand. Particular emphasis has been placed on cholera, where investigations have been directed towards vaccine development and rapid diagnosis and on diagnosis of typhoid and enterotoxigenic Escherichia coli. In the area of parasitic diseases emphasis has been directed especially to immunodiagnosis of helminthic diseases, including gnathostomiasis, paragonimiasis, opisthorchiasis, strongyloidiasis and trichinellosis. These stu…
