Management of acute soft tissue injury to the auricle.

The external ear is commonly involved in facial trauma. Injuries to the ear can range from simple lacerations to complete avulsions. We review the normal auricular anatomy and vascular supply, as well as the initial management of any auricular injury. Furthermore, we review the literature on soft tissue injuries of the ear and present a simple algorithm for classifying injuries. The classification is based on whether or not cartilage is involved. Injuries to the lobule do not involve cartilage and thus are more easily repaired by simple closure or Z-plasty. Injuries involving cartilage are fur

Sourcing and development of tissue for transplantation in reconstructive surgery: A narrative review.

The wealth of allogeneic and xenogeneic tissue products available to plastic and reconstructive surgeons has allowed for the development of novel surgical solutions to challenging clinical problems, often obviating the need to inflict donor site morbidity. Allogeneic tissue used for reconstructive surgery enters the tissue industry through whole body donation or reproductive tissue donation and has been regulated by the FDA as human cells, tissues, and cellular and tissue-based products (HCT/Ps) since 1997. Tissue banks offering allogeneic tissue can also undergo voluntary regulation by the Am

A New Technique Using Modified Dehaan's Method for Median Cleft Lip Closure: A Case Report.

OBJECTIVE: Median cleft lip is an extremely rare congenital abnormality that can be classified into "true" (characterized by true tissue separation) and "false" (caused by holoprosencephaly). We report a patient with true median cleft lip who underwent cheiloplasty using the surgical procedure for bilateral cleft lip. METHODS: The subject was a girl, who presented with mild orbital hypertelorism. Alveolar cleft with midline cleft lip extending slightly to the white lip, and anterior basal meningocele was observed. The patient was in good general condition and had undergone cheiloplasty using

A new perspective in oculoplastic surgical management of symptomatic distichiasis in lymphedema-distichiasis syndrome.

Lymphedema-distichiasis syndrome (LDS) is an autosomal dominant condition associated with FOXC2 mutations. Patients with distichiasis are mostly symptomatic, and efforts to deal with their ocular complaints comprise of electrolysis, cryotherapy and a variety of surgical techniques. We describe an enhanced surgical technique for a case of symptomatic distichiasis of the right eye with scarred, irregular eyelid margins secondary to initial cryotherapy, whereby the distorted tarsus was excised to remove the aberrant hair follicles, the levator palpebrae superioris was released to extend the upper

What is the current Dutch nerve management during amputation surgery?

Peripheral nerve management during limb amputation is essential, as neuromas frequently form after nerve transection. Some of which are painful and impair patient quality of life. Selecting the appropriate technique for nerve management is important as it may impact the development of painful neuromas. Diagnosis is often complex, and treatments may yield limited success, highlighting the importance of careful nerve handling at the time of surgery. In this study, we questioned Dutch plastic surgeons about their knowledge and preferred technique(s) in managing nerves during amputation surgery. A

Supply and Demand: Pediatric Surgical Specialties Fellowship Match Trends.

INTRODUCTION: Surgical specialization is critical in medical training, with fellowships as key pathways for career advancement and shaping the pediatric surgical workforce. Given the surgical condition and procedural overlap between subspecialties, skills often complement each other to enhance comprehensive pediatric care. This study examines the balance of supply and demand across pediatric surgical fellowship positions, evaluating trends in individual fields and their collective impact on pediatric surgical care. METHODS: We analyzed available fellowship match data from 2004 to 2024 across

Loss to follow-up after direct-to-implant breast reconstruction.

Loss to follow-up is inevitable in retrospective cohort studies, and patients are lost to follow-up after direct-to-implant reconstruction despite annual follow-up recommendation. We analyzed more than 500 patients to analyze the rate of loss to follow-up to plastic surgery and to investigate the factors affecting it. A retrospective review of patients who underwent direct-to-implant reconstruction between July 2008 and August 2016 was performed. Loss to follow-up to plastic surgery was defined as a difference of ≥24 months between the total and plastic surgery follow-up. The rate of loss to f

Mucogingival Treatment of a Gingival Invagination After Tooth Extraction: A 3-Year Follow-Up After a Perio-Ortho Multidisciplinary Approach.

BACKGROUND: Gingival invagination is a relatively common occurrence following orthodontic closure of extraction sites. The present paper reports a combined periodontal and orthodontic treatment in a patient after lower incisor tooth extraction, in which periodontal plastic surgery was performed prior to the orthodontic space closure. CASE REPORT: A 28-year-old female patient was treated for orthodontic concerns. The comprehensive treatment plan encompassed periodontal supportive therapy, tooth extraction, and periodontal plastic surgery utilizing the "connective tissue platform technique." Po

[New surgical techniques for the treatment of auricular atresia].

The rehabilitation of patients having severe malformation of the auricle with congenital auricular atresia requires plastic surgery and audiologic treatment. After classifying these malformations, various options involving hearing aids and surgery are presented. The main focus is on our new technique of combined auricular reconstruction with construction of the external ear canal and tympanoplasty, the so-called atresia-operation. It is characterised by three steps combining plastic and otologic surgery with prefabrication of the tympanic membrane and the external ear canal. Finally, our curre

Acute plastic bronchitis after Ross procedure treated with veno-venous extracorporeal membrane oxygenation.

PURPOSE: Plastic bronchitis is a rare condition characterized by the formation of airway casts occluding the lower respiratory tract. It is described more commonly in children, especially following correction of congenital heart disease. It involves lymphatic abnormalities leading to endobronchial lymph precipitating airway cast formation. When it presents acutely, it can lead to acute airway obstruction, which can be life-threatening. Plastic bronchitis has been rarely described in adults and is potentially underdiagnosed. The purpose of this case report is to emphasize, for the adult anesthe

Surgical therapy of recurrent vulvar cancer.

OBJECTIVE: The success of various surgical interventions in 201 cases with recurrent vulvar carcinoma was examined in the light of patients' pretreatment, surgical therapy, plastic reconstruction, and postoperative disease course. STUDY DESIGN: A databank of standardized clinical data was analyzed using statistical procedures. RESULTS: Therapy was selected on an individual basis according to tumor status. Recurrence at a site distant from the primary tumor, particularly in the inguinal region, indicated a markedly unfavorable prognosis. In contrast, tumors recurring locally did not exhibit a

The team approach in orofacial management.

Successful treatment of head and neck deformities by the plastic and maxillofacial surgeon often involves participation of the speech pathologist and orthodontist in planning therapy and helping the patient adapt to the surgery performed. Patients are presented demonstrating the communication in treatment planning and implementation recommended between surgeon, speech pathologist, and orthodontist. We describe the tools of examination used in the Division of Plastic and Maxillofacial Surgery at Duke University Medical Center.

Sartorius Muscle Flaps: Perioperative Outcomes Based on Surgical Specialty.

BACKGROUND: Complicated groin wounds often require repair by sartorius muscle flap (SMF). Operating surgical specialty differs based on SMF indication, hospital, and operating surgeon preference. We aim to assess the effect of operating surgical specialty, indication for SMF, and other patient-level factors on 30-day outcomes. METHODS: We collected data on all patients undergoing SMF performed at our institution from 2005 to 2015, including age, sex, body mass index, comorbidity index (hypertension, diabetes, dyslipidemia, peripheral arterial disease, coronary artery disease), smoking status,

[Giant abdomino-pubic melanoma. A case report].

Cutaneous melanoma patients are usually young adults. The evolution into a nodular form darkens the prognosis because it increases the risk of metastasis occurrence. The only curative treatment is wide surgical excision when extension check is negative. We report a case of abdomino-pubic melanoma original by its amazing size, its spreading, both superficial and nodular, without metastasis, and its late occurrence to a senior patient. Curative management was performed with a plastic surgery technique: the abdominal plasty. The melanoma was removed with free margins and the patient was free of d

The Hand That Gives Form to an Idea.

A visit to the Cartier exhibition at the National Gallery of Victoria prompted an unexpected reflection on plastic surgery. Cartier's philosophy of "constant reinvention" revealed a creative process that moves from an idea to design and finally to the skilled hand that gives form to the idea. A similar progression can be found in plastic surgery, where innovation begins with a surgical concept, is translated into operative design, and ultimately becomes reality through the surgeon's hands. The familiar jeweler's forceps displayed in the exhibition also recalled the delicate instruments used in
