OBJECT: Basilar impression (BI) secondary to osteogenesis imperfecta (OI) is a rare but debilitating condition that is often progressive unless it is halted. More recently, ventral decompression surgery has been advocated for this condition. This study is a retrospective review of the 21-year experience of ventral decompression surgery and dorsal occipitocervical fixation in patients with BI secondary to OI and is the largest patient series reported to date.
METHODS: Twenty patients treated between 1982 and 2003 by the senior author at the authors' institution were included in this study. All

STUDY DESIGN: This study prospectively analyzed gait in 21 patients with flatback and reviewed radiographs and charts.
OBJECTIVE: To analyze the effect of sagittal imbalance on gait and hip and knee joints.
SUMMARY OF BACKGROUND DATA: Loss of lumbar lordosis causes anterior displacement of the center of gravity, which creates instability and increases the work of gait. Several compensatory changes occur in response. The long-term effect of these changes on extra-axial joints has not been reported, nor have many studies analyzed the gait deviations in patients with flatback. Over time, as pat

This is a retrospective review of 12 patients treated for severe postlaminectomy and postirradiation kyphosis by one surgeon from 1977 to 1994. The average age of the patients was 15 years with a range from 2 to 35 years. The duration of followup ranged from 24 months to 156 months with an average of 65 months. All patients had undergone multilevel laminectomies or irradiation of the thoracic or lumbar spine for an intraspinal lesion or trauma. The average preoperative kyphosis was 84 degrees and this was reduced to an average of 39 degrees after surgery. There were no pseudarthroses and there

BACKGROUND: We explored the surgical technique of reducing the humeral head and repairing the fractures through a combined approach in the treatment of this complex injury.
METHODS: Six patients with posterior shoulder dislocations associated with proximal humerus fractures were enrolled in this study. The posteriorly dislocated head was first reduced through a shoulder posterior incision and the ruptured posterior capsular tissues were repaired simultaneously using a suture anchor. The fractures were then reduced and fixed with a PHILOS through a deltopectoral approach. The affected shoulder

BACKGROUND: We evaluated treatment of osteomyelitis in the foot in the presence of Charcot neuroarthropathy, a devastating condition with progressive degeneration and joint destruction. We hypothesized that there was a difference in (1) amputation rate, (2) amputation level, (3) duration of antibiotic therapy, and (4) duration of immobilization for treatment of osteomyelitis within versus outside the Charcot zone.
METHODS: Forty patients (43 ft) diagnosed with Charcot neuroarthropathy and osteomyelitis of the same foot were retrospectively analyzed. Some patients were successfully treated for

OBJECTIVES: The purpose of this article is to discuss the spectrum of failed spondylolisthesis cases for which either anterior column support or iliac screw fixation or both are useful in salvaging failed spondylolisthesis surgeries.
SUMMARY OF BACKGROUND DATA: Past studies and experience have suggested that there is a relatively high rate of sacral screw failure both in long constructs to the sacrum in the adult population and also with treatment of both high-grade and adult spondylolisthesis at L5-S1. It has been noted that anteri

Objective Total knee replacement after previous open reduction and internal fixation for tibial plateau fracture (conversion total knee) increases the complexity of the procedure and the complication rate. However, very little research exists to report on opioid use and cost associated with total knee arthroplasty (TKA) following tibial plateau fracture fixation as compared to primary TKA patients with no history of tibial plateau fracture. The aim of this study is to compare the differences in opioid use, reimbursements, and complication rates between patients with and without a history of ti

OBJECTIVES: Active middle ear implants (AMEI) have been used to treat hearing loss in patients for whom conventional hearing aids are unsuccessful for varied biologic or personal reasons. Several studies have discussed feedback as a potential complication of AMEI usage, though the feedback pathway is not well understood. While reverse propagation of an acoustic signal through the ossicular chain and tympanic membrane constitutes an air-conducted source of feedback, the implanted nature of the device microphone near the mastoid cortex suggests that bone conduction pathways may potentially be an

OBJECTIVES: Hip Osteoarthritis (OA) affects a significant component of the adult population, placing itself among the most important causes of disability and need for total hip replacement. Femoroacetabular impingement (FAI) is an anatomic alteration of the proximal femur and/or acetabulum leading to chondro-labral damage and playing a prominent role in OA pathogenesis. Thus, treating FAI is fundamental to relieve hip pain and further joint tissue deterioration. Labral reconstruction is considered the treatment of choice, in particular using tendon allografts or autografts which, however, have

INTRODUCTION: Lateral fractures of proximal femur are the most frequent fractures in elderly people. Internal fixation using medullary nails is the gold standard of treatment (Gamma 3 nail is the most implanted device) due to reduced incidence of complications than other devices. We report our experience in treating this kind of fractures with Gamma 3 nail, between January 2015 and December 2021.
METHODS: We performed a retrospective cohort study of patients treated in our orthopaedic department; level of clinical care is III: 559 patients (431 females and 128 males, with an average age of 85

A wide variety of disease processes produce alteration of midfacial skeletal growth, resulting in moderate-to-severe midface deficiency presenting as retrusion associated with Angle's class III malocclusion. Le Fort III osteotomies with advancement can provide an excellent tool for correction of this deformity. Recently, the corrective procedure of choice for advancement of midfacial segments has been distraction osteogenesis after osteotomy. Straight linear advancement is the most common choice for corrective movement of the midfacial segment, whether accomplished through acute surgical advan

INTRODUCTION: Charcot diabetic arthropathy is one of the most serious complications in patients with diabetes mellitus with sensorimotor neuropathy of the lower extremities, accompanied by metabolic disturbances and subsequent destruction of bones and joints. There are many methods of surgical treatment of diabetic charcot's foot described in the literature. A review of the literature found no publications describing the surgical treatment of Charcot arthropathy patients using the method of two-stage replacement of total and subtotal foot bone defects with heterotopic allografts using 3D model

Total knee arthroplasty (TKA) is still the gold-standard operative procedure for treating end-stage knee osteoarthritis, a disease impacting millions of patients worldwide and a key contributor to disability. As prevalence rates rise, the number of TKA procedures continues to increase. This review covers surgical procedure history, implant choices, perioperative guidelines, and technologies shaping TKA's future. We performed a comprehensive narrative review of published literature on several aspects of TKA. Significant areas of comparison include cemented versus cementless fixation, mobile ver

INTRODUCTION: Anterior cervical decompression and fusion (ACDF) is the standard surgical procedure for cervical radiculopathy and myelopathy, although ACDF includes risks of adjacent segment disease (ASD) and subsequent revision procedures. Various interbody cage, plate, and screw options can be utilized. Stand-alone devices were designed to overcome undesired complications of hardware prominence and associated dysphagia, soft tissue violation, and adjacent level encroachment. Implants include biomechanical structural support (cage) composed of various materials (polyetheretherketone (PEEK)/ti

Prostate cancer is the second most common cancer in men worldwide. Radical prostatectomy and radiation beam therapy are the most common treatment options for localized prostate cancer and have different associated complications. The etiology of post prostatectomy incontinence is multifactorial. There is evidence in the literature that anatomic support and pelvic innervation are important factors in the etiology of post-prostatectomy incontinence. Among the many surgical and technical factors proposed in the literature, extensive dissection during surgery, damage to the neurovascular bundle and
