Paediatric multisystem inflammatory syndrome associated with COVID-19: filling the gap between myocarditis and Kawasaki?

Myocarditis and Kawasaki disease are common but usually distinct diseases in children. During the coronavirus pandemic (COVID-19), reports of a new form of myocarditis with clinical features of Kawasaki appeared. We investigated the place of this new disease in the spectrum encompassing Kawasaki disease and myocarditis.Thirty two consecutive children referred to our centre for a suspicion of Kawasaki or a diagnosis of myocarditis were included and eventually divided into four groups: 11 Kawasaki diseases, 6 Kawasaki syndromes (children with another diagnosis), 7 myocarditis without Kawasaki cl

Reversal of Refractory Ulcerative Colitis and Severe Chronic Fatigue Syndrome Symptoms Arising from Immune Disturbance in an HLA-DR/DQ Genetically Susceptible Individual with Multiple Biotoxin Exposures.

BACKGROUND: Patients with multisymptom chronic conditions, such as refractory ulcerative colitis (RUC) and chronic fatigue syndrome (CFS), present diagnostic and management challenges for clinicians, as well as the opportunity to recognize and treat emerging disease entities. In the current case we report reversal of co-existing RUC and CFS symptoms arising from biotoxin exposures in a genetically susceptible individual. CASE REPORT: A 25-year-old previously healthy male with new-onset refractory ulcerative colitis (RUC) and chronic fatigue syndrome (CFS) tested negative for autoimmune diseas

The significance of fetal inflammatory response syndrome in early and later adaptation of premature infants.

PURPOSE: Intrauterine infection may induce the fetal inflammatory response syndrome (FIRS) and may cause cerebral palsy and bronchopulmonary dysplasia in newborns. The aim of the study was to evaluate the importance of FIRS for the early and later adaptation of preterms. METHODS: Hundred and fifty-eight preterm infants, born at 22-34 weeks of gestation, were investigated at Vilnius University Children hospital in 2007-2009. The data were evaluated at the first week after birth and at 36-37 weeks of post conceptual age. The levels of IL-6, tTNF-α (total), tVEGF-A (total), aTNF-α (active) and a

Monocyte human leukocyte antigen-DR expression correlates with intrapulmonary shunting after major trauma.

BACKGROUND: Severe injury is often complicated by the development of sepsis and the adult respiratory distress syndrome. Since the outcome from severe injury also correlates with changes in monocyte human leukocyte antigen (HLA)-DR expression in such patients, the present study aimed to determine whether or not there was a relationship between monocyte HLA-DR expression and indicators of early pulmonary dysfunction. METHODS: Monocyte HLA-DR expression and serum interleukin (IL)-6 were measured on admission and then again on days 1, 3, 5, and 7 after major injury in 29 patients admitted for th

Epstein-Barr virus dacryoadenitis resulting in keratoconjunctivitis sicca in a child.

PURPOSE: To describe a case of severe dry eye syndrome in a child. METHODS: Observational case report. The authors describe a 10-year-old male with severe dry eyes who was profoundly disabled by pain and photophobia despite aggressive conventional therapy. Lacrimal gland histology was consistent with the primary Sjögren syndrome, and serologic and immunohistologic evidence supported the hypothesis of Epstein-Barr virus causality. RESULTS: Treatment with systemic acyclovir and cyclosporin A resulted in dramatic and rapid reversal of the profound sicca syndrome and enabled the patient to resum

[Cerebrospinal fluid in infection of the central nervous system by yeasts of the genus Candida: analysis of 11 cases].

This study is based on the analysis of 44 cerebrospinal fluid (CSF) samples from 11 patients with central nervous system (CNS) Candida infection. Risk factors for CNS fungal infection were present in all patients. Five had a chronic meningitis syndrome; two had acquired immunodeficiency syndrome (AIDS); two had cranial trauma followed by chronic meningities; one had intravascular disseminated coagulation syndrome and sepsis; and one had systemic candidiasis after kidney transplant. Etiological diagnosis was made in all by the CSF examination. Nine cases had positive CSF culture for Candida. Tw

Systemic inflammatory response drives post-cardiac arrest multi-organ injury via IL-17 signaling in murine and porcine models.

BACKGROUND: Systemic inflammation plays a central role in the development of multi-organ diseases. Post-cardiac arrest syndrome (PCAS) is increasingly recognized as a systemic inflammatory condition characterized by myocardial dysfunction, brain injury, and multi-organ dysfunction following resuscitation. Interleukin-17 signaling is an important proinflammatory pathway, but its specific role in the pathogenesis of PCAS remains unclear. METHODS AND RESULTS: Using transcriptomic profiling of both cardiac and cerebral tissues after cardiac arrest/cardiopulmonary resuscitation (CA/CPR), we found

[Current view on the risks of artificial pulmonary ventilation].

There is no doubt that artificial ventilation of lungs seems to be proven as life-saving manoeuvre, whereas a growing amount of evidence is presented and published recently that artificial ventilation cause damage to patient by so far unexplained mechanism--by conversion of mechanical stress to biochemical signals inducing local and systemic inflammatory response, which is wide spreading and causing multiple organ dysfunction syndrome (MODS). Higher mortality of patients with acute lung injury on MODS rather than acute respiratory failure itself can be explained just by these findings. The art

Tumor Microenvironment Autophagic Processes and Cachexia: The Missing Link?

Cachexia is a syndrome that affects the entire organism and presents a variable plethora of symptoms in patients, always associated with continuous and involuntary degradation of skeletal muscle mass and function loss. In cancer, this syndrome occurs in 50% of all patients, while prevalence increases to 80% as the disease worsens, reducing quality of life, treatment tolerance, therapeutic response, and survival. Both chronic systemic inflammation and immunosuppression, paradoxically, correspond to important features in cachexia patients. Systemic inflammation in cachexia is fueled by the inter

[High molecular weight kininogen in inflammation and angiogenesis: a review of its properties and therapeutic applications].

The plasma kallikrein-kinin system (KKS) participates in the pathogenesis of inflammatory reactions involved in cellular injury, coagulation, fibrinolysis, kinin formation, complement activation, cytokine secretion and release of proteases. It has been shown that KKS activation in the systemic inflammatory response syndrome results in decrease of its component plasma proteins. Similar changes have been documented in diabetes, sepsis, children with vasculitis, allograft rejection, disseminated intravascular coagulation, patients with recurrent pregnancy losses, hereditary angioedema, adult resp…

Potential of extracellular vesicles for early prediction of severity and potential risk stratification in critical inflammatory diseases.

Some acute inflammatory diseases are often exacerbated during or after hospitalization, leading to some severe manifestations like systemic inflammatory response syndrome, multiple organ failure, and high mortality. Early clinical predictors of disease severity are urgently needed to optimize patient management for better prognosis. The existing clinical scoring system and laboratory tests cannot circumvent the problems of low sensitivity and limited specificity. Extracellular vesicles (EVs) are heterogeneous nanosecretory vesicles containing various biomolecules related to immune regulation, …

Role of complement and CD14 in meconium-induced cytokine formation.

OBJECTIVE: Meconium aspiration syndrome has a complex, poorly defined pathophysiology. Meconium is a potent activator of complement in vitro and in vivo; the latter is associated with a systemic inflammatory response. The complement system and Toll-like receptors are 2 important upstream components of the innate immune system that act partly independently in the inflammatory network. The aim of this study was to investigate the relative role of complement and CD14 in meconium-induced cytokine production. METHODS: Human adult (n = 6) and cord whole blood (n = 6) anticoagulated with lepirudin w

New management strategies in ARDS. Immunomodulation.

Acute respiratory distress syndrome is a common condition among the critically ill and is associated with high morbidity and mortality [table: see text] rates. Improved understanding of the underlying inflammatory pathogenetics has encouraged the search for strategies that, by modifying this immune response, can improve outcome for this group of patients. Some agents are obviously anti-inflammatory. Others have been used primarily for other purposes; their immune effects are incidental, but no less important. Although immunomodulatory strategies have been discussed for many years, they now are

The Inhibitory Effect of Curcumin on Virus-Induced Cytokine Storm and Its Potential Use in the Associated Severe Pneumonia.

Coronavirus infection, including SARS-CoV, MERS-CoV, and SARS-CoV2, causes daunting diseases that can be fatal because of lung failure and systemic cytokine storm. The development of coronavirus-evoked pneumonia is associated with excessive inflammatory responses in the lung, known as "cytokine storms," which results in pulmonary edema, atelectasis, and acute lung injury (ALI) or fatal acute respiratory distress syndrome (ARDS). No drugs are available to suppress overly immune response-mediated lung injury effectively. In light of the low toxicity and its antioxidant, anti-inflammatory, and an

Respiratory transmission potential of severe fever with thrombocytopenia syndrome bunyavirus: evidence from intranasal exposure in a humanized mouse model.

Severe Fever with Thrombocytopenia Syndrome Bunyavirus (SFTSV) is a highly lethal pathogen with expanding endemic regions in Asia. While primarily transmitted by ticks, recent evidence suggests potential airborne transmission, raising significant public health concerns. This study investigates the potential for respiratory transmission and pathogenesis using humanized NCG mice inoculated with SFTSV via subcutaneous injection challenge (SIC) or intranasal drop challenge (IDC). Both groups demonstrated rapid systemic dissemination, marked by viremia, weight loss, and multi-organ injury, with hem

[Various approaches to surgical treatment of infectious aneurysms of the abdominal aorta and iliac arteries (clinical case)].

The article presents clinical observations of surgical treatment of patients with infectious aneurysms of the aorta and iliac arteries. Initially, the patients sought medical help for pain in the lumbar region. The examination revealed aneurysms of various localization, accompanied by a syndrome of systemic inflammatory response. An open surgery was chosen as the treatment strategy. One patient required reintervation after the first operation due to the formation of a pseudoaneurysm of the distal anastomosis. Both patients were discharged for outpatient treatment after massive antibacterial th
