Sleepiness and safety at work among night shift NHS nurses.

BACKGROUND: Sleepiness associated with night shift working (NSW) is known to adversely affect workers' health and well-being. It has been associated with adverse safety outcomes and is a recognized workplace hazard among healthcare workers. AIMS: This study was aimed to assess the prevalence of self-reported sleepiness in NSW nurses and midwives. This study also explored the consequences of sleepiness on safety at work and driving. METHODS: A cross-sectional study of NSW nurses and midwives was conducted at an National Health Service (NHS) hospital trust from 16 March 2020 to 1 June 2020. Da

Injection route and TLR9 agonist addition significantly impact heroin vaccine efficacy.

Active immunization is an effective means of blocking the pharmacodynamic effects of drugs and holds promise as a treatment for heroin addiction. Previously, we demonstrated the efficacy of our first-generation vaccine in blocking heroin self-administration in rats, however, many vaccine components can be modified to further improve performance. Herein we examine the effects of varying heroin vaccine injection route and adjuvant formulation. Mice immunized via subcutaneous (sc) injection exhibited inferior anti-heroin titers compared to intraperitoneal (ip) and sc/ip coadministration injection

Bright-light effects on cognitive performance in elderly persons working simulated night shifts: psychological well-being as a mediator?

PURPOSE: The present study examined whether the relationship between light exposure and cognitive functioning is mediated by psychological well-being in elderly persons working night shifts. The role of psychological well-being has been neglected so far in the relationship between bright light and cognitive performance. Sleepiness and mood were applied as indicators of psychological well-being. Cognitive functioning was examined in terms of concentration, working memory, and divided attention. METHODS: A total of thirty-two test persons worked in three consecutive simulated night shifts, 16 u

Attentional shifting and the role of the dorsal pathway in visual word recognition.

A substantial amount of evidence has been collected to propose an exclusive role for the dorsal visual pathway in the control of guided visual search mechanisms, specifically in the preattentive direction of spatial selection [Vidyasagar, T. R. (1999). A neuronal model of attentional spotlight: Parietal guiding the temporal. Brain Research and Reviews, 30, 66-76; Vidyasagar, T. R. (2001). From attentional gating in macaque primary visual cortex to dyslexia in humans. Progress in Brain Research, 134, 297-312]. Moreover, it has been suggested recently that the dorsal visual pathway is specifical

A pictorial sleepiness scale based on cartoon faces.

STUDY OBJECTIVES: To develop a sleepiness scale devoid of semantic or geometric elements. DESIGN: Subjects were asked to rank in order 7 cartoon faces representing degrees of sleepiness. We used Thurstone's scaling procedure to transform these rankings into an interval scale, which allowed us to eliminate 2 of the faces. The remaining 5 faces were ranked again using other subjects. In a validation study, subjects rated their perceived level of sleepiness using our scale and other sleepiness scales. Employed shiftworkers and school-going children used our scale to assess its practical applicab

Decision for self and other modulates risk attitude and electrophysiological processing: evidence from a behavioral and electrophysiological experiment.

Understanding how people make risky decisions for others compared to themselves is central to decision neuroscience. However, the cognitive and neural underpinnings of such self-other shifts in risk preference-and the mechanisms driving individual differences-remain unclear. To address this, we employed a mixed gambling task with feedback in which participants made risky decisions for themselves and for others while electroencephalography was recorded. Although individuals generally exhibited similar patterns across agents, decisions made for others were associated with a higher degree of risk

The locus of recognition memory signals in human cortex depends on the complexity of the memory representations.

According to a "Swiss Army Knife" model of the brain, cognitive functions such as episodic memory and face perception map onto distinct neural substrates. In contrast, representational accounts propose that each brain region is best explained not by which specialized function it performs, but by the type of information it represents with its neural firing. In a functional magnetic resonance imaging study, we asked whether the neural signals supporting recognition memory fall mandatorily within the medial temporal lobes (MTL), traditionally thought the seat of declarative memory, or whether the

[Health promotion and obesity in the workplace among health care workers of a hospital in Catania (Italy)].

BACKGROUND: Italian Legislative Decree No. 81/2008 foresees the involvement of a "specialised physician" in activities for the promotion of health at the workplace. OBJECTIVES: These activities are aimed at modifying erroneous lifestyles (such as smoking, drinking, bad diet, etc.) which are responsible for serious illnesses and also for repercussions on the assessment of fitness for a specific task. Obesity, as we know, represents the major risk factor in the onset of metabolic, neoplastic, cardiovascular and respiratory diseases and also in the increased incidence of accidents at the workpla

Imaging cognitive deficits in drug abuse.

The neuropsychological network is a complex structure. To identify processes location and network capacity the brain imaging techniques together and in combination with other neuropsychological techniques and the expanding of well elaborated designs provide us with a multidimensional understanding, and contributes to the understanding of each illicit drug's character, which is of importance in designing of new treatment programs and clinical practice. Cannabis, MDMA, amphetamine, cocaine, and heroin abusers display both acute effects and chronic effects, deficits in attention, memory, and exec

Localisation and stimulation of the parietal eye field.

Localisation of the human parietal eye fields (PEF) has not been as well studied as the human frontal eye fields (FEF). Stimulation studies in rhesus monkeys have suggested the localisation of the PEF to be within the intraparietal sulcus. Functional MRI studies have demonstrated this region to be highly active and potentially connected in saccadic and gaze shifting tasks. Here, we present a case of a patient with left versive seizures evaluated with SEEG, in whom electrical stimulation within the right intraparietal sulcus resulted in horizontal and downward conjugate eye movements contralate

Detained and Dying: Ethical Issues Surrounding End-of-Life Care in Prison.

Prisons are increasingly being called upon to provide end-of-life (EOL) care within the restrictive correctional environment. Several relatively recent phenomena have brought medical ethics to the forefront of prison EOL care-including aging behind bars, a paradigm shift in prison culture, the increasing rate of in-prison deaths, and the corresponding prison hospice movement. This article examines prominent ethical issues that emerge for prison personnel who are tasked with providing care to terminally ill offenders by presenting three offender composite characters that exemplify dying offende

Reprint of: Object-based attentional facilitation and inhibition are neuropsychologically dissociated.

Salient peripheral cues produce a transient shift of attention which is superseded by a sustained inhibitory effect. Cueing part of an object produces an inhibitory cueing effect (ICE) that spreads throughout the object. In dynamic scenes the ICE stays with objects as they move. We examined object-centred attentional facilitation and inhibition in a patient with visual form agnosia. There was no evidence of object-centred attentional facilitation. In contrast, object-centred ICE was observed in 3 out of 4 tasks. These inhibitory effects were strongest where cues to objecthood were highly salie

Infection of male rats with Toxoplasma gondii results in enhanced delay aversion and neural changes in the nucleus accumbens core.

Rats infected with the protozoan parasite Toxoplasma gondii exhibit reduced avoidance of predator odours. This behavioural change is likely to increase transmission of the parasite from rats to cats. Here, we show that infection with T. gondii increases the propensity of the infected rats to make more impulsive choices, manifested as delay aversion in an intertemporal choice task. Concomitantly, T. gondii infection causes reduction in dopamine content and neuronal spine density of the nucleus accumbens core, but not of the nucleus accumbens shell. These results are consistent with a role of th

Decision-making in Blackjack: an electrophysiological analysis.

Previous studies have identified a negative potential in the event-related potential (ERP), the error-related negativity (ERN), which is claimed to be triggered by a deviation from a reward expectation. Furthermore, this negativity is related to shifts in risk taking, strategic behavioral adjustments, and inhibition. We used a computer Blackjack gambling task to further examine the process associated with the ERN. Our findings are in line with the view that the ERN process is related to the degree of reward expectation. Furthermore, increased ERN amplitude is associated with the negative evalu

A physiological correlate of the 'spotlight' of visual attention.

Here we identify a neural correlate of the ability to precisely direct visual attention to locations other than the center of gaze. Human subjects performed a task requiring shifts of visual attention (but not of gaze) from one location to the next within a dense array of targets and distracters while functional MRI was used to map corresponding displacements of neural activation within visual cortex. The cortical topography of the purely attention-driven activity precisely matched the topography of activity evoked by the cued targets when presented in isolation. Such retinotopic mapping of at

Checklists in community care: reducing differences in care delivery between regular and relief staff to improve consistency and client experience.

BACKGROUND: Today, healthcare is more complex than just ensuring clients receive quality care; it also involves consistently delivering excellent client experience. A non-profit community support services agency conducted an extensive diagnostic journey to determine root causes of inconsistent care delivery between regular and relief frontline staff. LOCAL PROBLEM: Clients and family caregivers noted lower satisfaction in care delivery when a relief staff (ie, internal staff or an external agency that is covering a shift) provided service in comparison with their regular staff. The diagnostic

Occupational metal exposure and systemic inflammation among recycling workers engaged in domestic waste sorting, shredding, and material extraction.

INTRODUCTION: The amount of metal-containing waste being recycled and the number of employees in these recycling industries, an important part of the green transition, are increasing. It is essential to ensure that the workers are not exposed to harmful levels of metals during work. This study aims to document current biomarker exposure levels of metals and investigate the association between metals and inflammatory biomarkers among recycling workers. METHODS: In this cross-sectional study, concentrations of lead, mercury, copper, cobalt, cadmium, chromium, manganese, nickel, and selenium wer

Higher order aberrations and retinal image quality during short-term accommodation in myopic and non-myopic children.

INTRODUCTION: Despite the known associations between near work and myopia, and retinal image quality and eye growth, accommodation-induced changes in higher order aberrations (HOA's) and retinal image quality in children with different refractive errors are poorly understood. METHODS: Ocular HOA's were measured using a Hartmann-Shack wavefront sensor (COAS-HD, Wavefront Sciences) in 18 myopic and 18 age- and sex-matched non-myopic children during short-term accommodation tasks (four demands of 0, 3, 6 and 9 D) presented using a Badal optometer. Functional compensation of motor function in pre-symptomatic Huntington's disease.

Involuntary choreiform movements are a clinical hallmark of Huntington's disease. Studies in clinically affected patients suggest a shift of motor activations to parietal cortices in response to progressive neurodegeneration. Here, we studied pre-symptomatic gene carriers to examine the compensatory mechanisms that underlie the phenomenon of retained motor function in the presence of degenerative change. Fifteen pre-symptomatic gene carriers and 12 matched controls performed button presses paced by a metronome at either 0.5 or 2 Hz with four fingers of the right hand whilst being scanned with
