Implementing pharmacist-led home medicines reviews within fracture liaison services for better post-fracture care: a qualitative study of patient and practitioner experience.

UNLABELLED: Home medicines reviews present a novel intervention supporting continuity of care between fracture liaison services and primary care. Home medicines review pharmacists deliver multifaceted, collaborative interventions that can bridge gaps in care, promote patient empowerment, facilitate a GP-patient discussion about bone health, and improve coordination of care. PURPOSE: Medication review has potential to reduce the risk of falls and fractures and improve transition of care from a specialist fracture liaison service to primary care. This study was designed to examine and evaluate

Association of Anesthesia Type and Vasoconstrictor Use with Intranodal Lymphangiography Duration: A Multicenter Retrospective Cohort Study.

PURPOSE: To investigate whether anesthesia type (sedation vs general anesthesia [GA]) or α1-adrenergic agonist vasoconstrictor administration is associated with faster intralymphatic contrast agent transit during intranodal lymphangiography in an effort to identify modifiable nonprocedural factors that may reduce procedure duration. MATERIALS AND METHODS: This retrospective observational study reviewed intranodal lymphangiography from 2 academic hospitals (2015-2022). A multiple linear regression model assessed the association of anesthesia type and vasoconstrictor administration with intraly

A Novel Dyadic Psychological Intervention for Depression Initiated During Short-Term Skilled Nursing Facility Stays.

Depressive symptoms are prevalent among short-stay skilled nursing facility (SNF) residents and often persist after discharge, contributing to poor clinical outcomes. Given the central role of informal care partners in post-acute care transitions, a dyadic approach to psychological care may be optimal, but it remains untested in the SNF settings. Intervening during Stressful Transitions in Later Life (InSTILL) is a novel, contextually sensitive, dyadic psychological intervention designed to reduce depression during post-acute care. In a single-arm proof-of-concept study, 6 SNF resident-care pa

HIV-related stigma among young people living with HIV transitioning to an adult clinic in a public hospital in Rio de Janeiro, Brazil.

This study analyzes how experiences of HIV-related stigma are expressed among HIV-positive young people transitioning to an adult clinic, the health service, the family, the affective-sexual interactions, and their relationship with inequalities and social hierarchies. This research included 31 young people (median age 21) transitioning to an adult clinic (G1) and 12 young people (median age 30) who had already made this transition (G2), both monitored at a health service in Rio de Janeiro. Seventy percent of the 43 young people were women and 65% were infected by mother-to-child transmission.

Prenatal exercise interventions result in subtle changes in the human breast milk metabolome and lipidome 7-14 days postpartum: exploratory analyses of the FitMum randomized controlled trial.

BACKGROUND: Prenatal exercise has been linked to positive effects on offspring health in animal and human studies, with changes in breast milk composition suggested as a possible explanatory mechanism. This exploratory study investigated whether prenatal exercise interventions affected the metabolome and lipidome of transitional human breast milk in healthy pregnant women compared to standard care. METHODS: The present study is based on the FitMum randomized controlled trial (Copenhagen University Hospital - North Zealand, 2018-2021), which included in total 220 healthy inactive pregnant wome

Transition mentorship for spina bifida patients with the JUMP program: a pilot study.

OBJECTIVE: The transition from pediatric to adult care is challenging for patients and families with spina bifida (SB). Lifelong care relationships yield to new care environments that are typically larger, less personal, and less engaged with the nuances of SB care. Adolescence and young adulthood are often characterized by personal and psychological stresses due to factors independent of illness or chronic medical complexity. Surveys have demonstrated that transition is associated with uncertainty, anxiety, and elevated risk of adverse events for many SB patients. To help mitigate this, the a

Bridging the Gap Between the ED and Home: The Community Paramedic-Led Transitions Intervention for Persons Living With Dementia.

More than 6 million persons living with dementia (PLWD) in the United States rely on the emergency department (ED) for unscheduled care, with up to half discharged home after treatment. The ED-to-home transition poses significant challenges for PLWD and their care partners (referred to as "dyads"), contributing to high rates of ED revisits and adverse outcomes. The Community Paramedic-led Transitions Intervention (CPTI) was developed to address these challenges by adapting the validated Care Transitions Intervention for the ED setting. Delivered by trained community paramedics, CPTI is a short

Trajectories of Stroke Care in Ontario: Which Path to Best Care?

BACKGROUND: Few studies have tracked stroke survivors through transitions across the health system and identified the most common trajectories and quality of care received. The objectives of our study were to examine the trajectories that incident stroke patients experience and to quantify the extent to which their care adhered to the best practices for stroke care. METHODS: A population-based cohort of first-ever stroke/transient ischemic attack (TIA) patients from the 2012/13 Ontario Stroke Audit was linked to administrative databases using an encrypted health card number to identify domina

Transition Readiness Assessment in Adolescents and Young Adults with Neurofibromatosis Type 1 (NF1).

Neurofibromatosis type 1 (NF1) conveys significant disease morbidity and lower quality of life compared to the general population. Research has shown that decreased positive health outcomes are directly correlated with inadequate development of health-related self-management skills among similar patient populations, and among these populations a healthcare transition (HCT) intervention improves provision of care and health outcomes. Thus, HCT intervention may improve care and outcomes in NF1. To design a future informed NF1 HCT intervention, baseline transition readiness must be assessed. A su

The Evolution of Diapsid Reproductive Strategy with Inferences about Extinct Taxa.

Diapsids show an extremely wide range of reproductive strategies. Offspring may receive no parental care, care from only one sex, care from both parents, or care under more complex regimes. Young may vary from independent, super-precocial hatchlings to altricial neonates needing much care before leaving the nest. Parents can invest heavily in a few young, or less so in a larger number. Here we examine the evolution of these traits across a composite phylogeny spanning the extant diapsids and including the limited number of extinct taxa for which reproductive strategies can be well constrained.

Paroxysmal short runs of irregular narrow QRS tachycardia: What is the mechanism?

The present case that showed the frequent transition between left atrial tachycardia and AV nodal re-entrant tachycardia has never been reported. This instructive case highlights the need for careful interpretation of intracardiac electrocardiogram and suggests that differential pacing maneuvers are not feasible in transitional tachycardia with atrial fusion.

Reducing Recurrence and Complications Related to Clostridioides difficile Infection: A Panel Discussion Summary.

BACKGROUND: The Centers for Disease Control and Prevention identifies Clostridioides difficile infection (CDI) as an urgent threat to people and health care systems. CDI leads to high health care utilizations and results in significantly reduced quality of life for patients. The high burden of disease is seen across all health care settings, outside of the hospital, in the community, and in younger people. Individuals with CDI transition from hospitals to long-term care facilities to the community, and management of these transitions can reduce the incidence of recurrence and rehospitalization

Care Navigation Increases Initiation of Hepatitis C Treatment After Release From Prison in a Prospective Randomized Controlled Trial: The C-LINK Study.

BACKGROUND: Prison-based hepatitis C treatment is safe and effective; however, many individuals are released untreated due to time or resource constraints. On community re-entry, individuals face a number of immediate competing priorities, and in this context, linkage to hepatitis C care is low. Interventions targeted at improving healthcare continuity after prison release have yielded positive outcomes for other health diagnoses; however, data regarding hepatitis C transitional care are limited. METHODS: We conducted a prospective randomized controlled trial comparing a hepatitis C care navi…

Housing Attainment With the Psychiatric Assistance for Transitional Homelessness in Shelters Program.

IMPORTANCE: The prevalence of mental illness is high among homeless populations, yet traditional clinic-based care is often inaccessible. For urban areas where housing-first models are difficult to implement because of high costs, integrated on-site psychiatric services in shelters may offer a feasible alternative. OBJECTIVE: To examine clinical and demographic factors associated with housing attainment among individuals experiencing homelessness with mental illness participating in an on-site psychiatric service program.
