Geospatial mapping can be used to identify geographic areas and social factors associated with intentional injury as targets for prevention efforts distinct to a given community.

BACKGROUND: Geographic information systems (GIS) have proven effective in studying intentional injury in various communities; however, GIS is not implemented widely for use by Level I trauma centers in understanding patient populations. Our study of intentional injury combines the capabilities of GIS with a Level I trauma center registry to determine the spatial distribution of victims and correlated socioeconomic factors. METHODS: One thousand ninety-nine of 3,109 total incidents of intentional trauma in the trauma registry from 2005 to 2015 had sufficient street address information to be ma…

Venous thromboembolism in transfer trauma patients: A global problem.

BACKGROUND: As air travel and immobility are risk factors for venous thromboembolism, we aimed to test the hypothesis that internationally transferred trauma patients have a high incidence of venous thromboembolism on arrival. METHODS: A prospectively maintained registry of all international transferred trauma patients who presented to our level I trauma center from January 2023 to June 2024 was retrospectively reviewed. Patients with either lower extremity venous duplex ultrasound or computed tomography scan of the chest with contrast on arrival were included. The primary outcome was venous

Placental Histology After Minor Trauma in Pregnancy: A Pilot Study.

OBJECTIVE: Trauma in pregnancy may cause placental abruption. Consequences of moderate placental injury on neurodevelopment are unknown. The aim was to evaluate placental histology after maternal trauma. METHODS: A prospective study was conducted at 2 tertiary medical centers in the Netherlands. Placentas from women exposed to maternal trauma ≥ 20 weeks' gestational age were histologically examined. Neurological follow-up of the infants was performed at 1 year of age by means of Alberta Infant Motor Scale. Histological findings were compared to placentas from pregnancies without trauma. PRIN

Modern approach to management of patients following major trauma.

Trauma remains one of the three main causes of death all over the world. In Poland severe trauma is also an important public health problem which should be fully recognised in the changing system of our health service. There is a growing understanding and interest in the integration of pre-hospital, in-hospital and rehabilitation services taking part in the management of trauma patients. The importance of emergency medicine departments has been found valuable in Western countries and should be promoted by all means in our country. Certain recommendations of treatment of patients following seve

Extension of the fluidics NTF-Biobank of polytraumatized patients with extracellular vesicles: improved vesicle preservation through local processing.

PURPOSE: Extracellular vesicles (EVs) show diagnostic relevance in trauma, prompting the development of the NTF‑EV‑Biobank as an extension of the established Network Trauma Research (NTF) plasma/serum biobank. After optimizing the EV isolation and characterization standard operating procedure (SOP) at a single center, this multicenter study evaluates its transferability and examines how sample transport affects EV quality before large‑scale implementation in a nationwide biobank setting. METHODS: Plasma and serum from polytrauma patients and healthy controls (n=10) were processed under the st

Development and validation of a prediction model for respiratory complications and in-hospital mortality in trauma patients.

Respiratory complications are major contributors to morbidity and mortality in trauma patients, yet conventional predictive models remain limited in scope and performance. We developed and validated an ensemble machine learning model that integrates both prehospital and in-hospital clinical variables to predict respiratory complications in trauma patients. We developed and internally validated machine learning models using data from the Korea Trauma Data Bank, comprising records from 19 major trauma centers in Korea between 2017 and 2022 (discovery; n = 48,376). For external validation, data f…

Trauma Team Activation: Not Just for Trauma Patients.

Specialized trauma teams have been shown to improve outcomes in critically injured patients. At our institution, an the American College of Surgeons Committee on trauma level I Trauma center, the trauma team activation (TTA) criteria includes both physiologic and anatomic criteria, but any attending physician can activate the trauma team at their discretion outside criteria. As a result, the trauma team has been activated for noninjured patients meeting physiologic criteria secondary to nontraumatic hemorrhage. We present two cases in which the trauma team was activated for noninjured patients

Use of resuscitative endovascular balloon occlusion of the aorta (REBOA) for trauma and its performance in Japan over the past 18 years: a nationwide descriptive study.

BACKGROUND: Resuscitative endovascular balloon occlusion of the aorta (REBOA) has been used to control massive hemorrhages. Although there is no consensus on the efficacy of REBOA, it remains an option as a bridging therapy in non-trauma centers where trauma surgeons are not available. To better understand the current landscape of REBOA application, we examined changes in its usage, target population, and treatment outcomes in Japan, where immediate hemostasis procedures sometimes cannot be performed. METHODS: This retrospective observational study used the Japan Trauma Data Bank data. All ca…

Emergency department versus operating room intubation of patients undergoing immediate hemorrhage control surgery.

BACKGROUND: Hemorrhage control surgery is an essential trauma center function. Airway management of the unstable bleeding patient in the emergency department (ED) presents a challenge. Premature intubation in the ED can exacerbate shock and precipitate extremis. We hypothesized that ED versus operating room intubation of patients requiring urgent hemorrhage control surgery is associated with adverse outcomes at the patient and hospital-levels. METHODS: Patients who underwent hemorrhage control within 60 minutes of arrival at level 1 or 2 trauma centers were identified (National Trauma Data Ba…

[Diagnostic assessment and treatment concepts for thoracic trauma].

Most patients with chest trauma can be successfully treated with tube thoracostomy and appropriate pain medication. Initial care of these patients is usually straightforward and performed by an emergency doctor or an emergency room surgeon, e.g. a general surgeon. If more extensive therapy of these polytraumatized patients appears to be required, tertiary care should be done in specialized centers or clinics with network structures. An appropriate structured network of surgical centers guarantees sufficient and efficient care of patients with severe chest trauma. In a best-case scenario the sp

Guideline adherence in helicopter versus ground emergency medical services: an indication-based analysis of 129,783 patients from the TraumaRegister DGU®.

BACKGROUND: Helicopter emergency medical services (HEMS) are associated with higher intervention rates and improved access to specialized trauma centers compared with ground emergency medical services (GEMS). However, it remains unclear whether these differences reflect increased intervention intensity alone or more consistent implementation of guideline-recommended care. This study aimed to compare rates of selected guideline-recommended prehospital trauma interventions between HEMS and GEMS using a nationwide trauma registry. METHODS: This retrospective multicenter cohort study was based on…

Prehospital Extended Focused Assessment with Sonography in Trauma with 5G-enabled Real-time Telesupervision for the Early Detection of Intra-abdominal Hemorrhage and Trauma Team Activation: A Case Report and Feasibility Insight.

Prehospital ultrasound, particularly the Extended Focused Assessment with Sonography in Trauma (EFAST), has emerged as a valuable tool in the early identification of life-threatening conditions, including hemoperitoneum and pneumothorax. Current evidence supports the role of prehospital ultrasound in improving diagnostic accuracy, facilitating triage decisions, and enhancing communication with receiving trauma centers. However, its application in detecting intra-abdominal hemorrhage and expediting trauma team activation (TTA) remains underutilized in many emergency medical systems. This case h…

The "Slash Pack" as a Temporary Method for Initial Hemorrhage Control of Penetrating Neck Trauma in the Emergency Department: A Report of Two Cases.

Penetrating trauma to the neck poses a critical challenge in the emergency department. These wounds have the potential to lead to rapid blood loss, hemorrhagic shock, and an increase in mortality. Large academic tertiary centers typically have vast resources and consultants to allow for rapid assessment and treatment of penetrating neck trauma. In community emergency departments, this is not typically the case, as surgical backup is usually not readily available and lacks the additional subspecialty coverage to aid in the management of the trauma patient. This situation often necessitates inno

Are We Imaging Gently in Indiana? A System-Wide Population-Based Study of Chest CT Use in the Pediatric Trauma Population.

OBJECTIVES: Over the past decade, there has been a significant effort to decrease radiation exposure in pediatric trauma patients. The objective of this study was to determine if trauma centers (TCs) and nontrauma centers (non-TCs) are practicing in line with this effort. We hypothesized that TCs would demonstrate a significant decrease in the use of chest computed tomography (CT) during the study period, whereas non-TC would show no change in chest CT use. METHODS: We queried a state-wide database from 2010 to 2020 for pediatric trauma encounters at TCs and non-TCs within a single large heal…

Helical and multislice CTA following penetrating trauma to the subclavian and axillary arteries (pictorial essay).

Penetrating injuries with resultant trauma to the subclavian and axillary arteries have traditionally been evaluated with direct contrast angiography. Physical examination has a low sensitivity for detection of vascular injuries in stable patients and surgical exploration is challenging. With advancements in CT technology, a less invasive and more rapid approach in evaluating arterial injury is now being utilized in many trauma centers. This article will depict the CT angiographic signs of subclavian and axillary artery injuries.

The evaluation of blunt abdominal trauma: the evolving role of ultrasound.

There is a growing body of literature pertaining to the use of ultrasound in the evaluation of patients with blunt abdominal trauma. Multiple studies have looked at the use of this modality as a screening examination for the detection of intra-abdominal fluid and as a means of diagnosing specific organ injuries. Although ultrasound has been used extensively in Europe and Asia, it has only recently been used in the United States. In many centers, ultrasound is now being performed by emergency medicine physicians and trauma surgeons as part of the initial trauma evaluation. This paper summarizes
