A female patient with end-stage chronic obstructive pulmonary disease received a double-lung transplant from a donor with serologic evidence of past syphilis. The recipient showed a Treponema pallidum-specific IgG seroconversion 8 days after transplantation with increasing titers at follow-up. In Month 3 post-transplantation the immunoglobulin G (IgG) antibody titer decreased. Retrospectively, we argue that the reactive syphilis serology in the recipient was due to a transmission of immunocompetent B cells in the graft rather than a transmission of T pallidum.

Analysis of the stigmata of congenital syphilis in two study populations indicates that the imprints of the disease are the same, whether the patient population is of Caribbean or of North American origin. If the Columbian theory of origin of syphilis were correct, the longer syphilization of the West Indies might have tended to lessen the effects of the disease by attenuating Treponema pallidum. The presence of yaws and pinta in the West Indies may even have had an immunizing effect. The similar findings in the two population groups suggests that the events postulated by the Columbian theory …

Serum or spleen cells from hamsters immune to infection with Treponema pallidum strain Bosnia A conferred partial protection against syphilitic infection by the same strain on recipient hamsters. Cutaneous lesions did not develop, and in the lymph nodes the average weight and number of treponemes detected were significantly lower than in normal and control hamsters. Treatment of the immune spleen cells with antithymocyte serum and complement abolished their ability to transfer resistance. This is the first direct evidence that thymus-derived cells are involved in resistance to syphilitic infec…

Syphilis is a sexually transmitted infection caused by Treponema pallidum It progresses through three clinical stages and shows various oral symptoms, mainly during the secondary stage. The disease can resemble other common oral mucosal conditions, such as infections with nonspecific pharyngitis, tonsillitis and laryngitis, as well as neoplastic and immune-mediated ulcers. This report discusses a heterosexual couple with rare manifestations of primary and secondary syphilis involving oropharyngeal lesions and an extragenital chancre, highlighting the diagnostic challenge and the importance of …

We report a case of a 59-year-old man presenting with a widespread follicular-based papular rash with a several-month history of myalgias, lymphadenopathy, fatigue, and weight loss who was diagnosed with acute syphilitic folliculitis by tissue biopsy analysis with immunohistochemical demonstration of spirochetes in hair follicle epithelium. Serologic analysis also showed evidence of Treponema sp. infection. Owing to the rising number of syphilis cases in the last decade, it is important to recognize classic cutaneous findings of syphilis in addition to unusual presentations such as syphilitic …

Syphilis is an infection caused by Treponema pallidum. It is most commonly acquired through sexual transmission, although it can also be transmitted vertically across the placenta, resulting in congenital syphilis. Even with improved public health measures, testing, and treatment capabilities, primary, secondary, and congenital syphilis have all surged since 2012. Given this marked increase in both incidence and prevalence, here we present a comprehensive review of the clinical presentation, treatment, and management of congenital syphilis, with particular consideration given to the mucocutane…

Syphilis is an infection caused by the bacterium Treponema pallidum. Patients with syphilis can present with a wide range of symptoms depending on the stage of disease. An underappreciated manifestation of syphilis is ocular involvement, which has been increasingly reported in the last several years. Delay in diagnosis and treatment of ocular syphilis can increase the risk of irreversible vision loss. Syphilis can affect almost every structure in the eye; however, most commonly presents with panuveitis. Ocular syphilis is treated similarly to neurosyphilis with prompt initiation of intravenous…

Syphilis is caused by infection with the spirochetal bacterium Treponema pallidum subsp. pallidum. It was first recognized in the late 15th century. Since 2000, the incidence of sexually acquired syphilis has increased substantially in the developed world, with men who have sex with men and persons living with HIV infection disproportionately affected. Clinical manifestations of syphilis are protean and often include mucocutaneous manifestations. The first article in this continuing medical education series reviews historical aspects, microbiology, epidemiology, and clinical manifestations of …

Syphilis is resurging worldwide. Here, we present the case of a 33-year-old heterosexual man who presented with a 3-week history of epigastric pain, nausea, emesis, and 8 kg weight loss. He was subsequently diagnosed with gastric syphilis, based on reactive syphilis serological testing and Treponema pallidum found in gastric biopsy specimens. Gastric syphilis is a rare presentation observed in 1% of cases and usually develops in secondary syphilis. Given the nonspecific manifestation and findings, a high index of suspicion is required for diagnosis of gastric syphilis.

At the end of the 15th century syphilis appeared in Europe as a devastating epidemic. For nearly four centuries mercury was regarded as a remedy of first-rate importance along with guaiac and potassium iodide. In the early 20th century, two new substances were added to syphilis therapeutics, namely arsenobenzol and bismuth. The absence of treponemal serum tests and the erroneous attribution of the positive Wassermann reaction exclusively to Treponema pallidum infection resulted in prolonged and non-proven effective treatments. In 1943 John Mahoney introduced penicillin, revolutionizing the tre…

Infectious agents have been implicated in the pathogenesis of many autoimmune diseases via various pathogenic mechanisms, such as molecular mimicry, resulting in modulation of the host's immune tolerance. In the following article we examine the association between serological evidence of past infection with Toxoplasma gondii, rubella virus, cytomegalovirus, Treponema pallidum, and Epstein-Barr virus, and the co-existence of celiac disease. Our results imply that certain infections may generate an immunological environment that disfavors future appearance of certain autoimmune conditions such a…

Optometrists provide an important service to the community, often acting as the first health care provider a patient consults. Many patients will present with ocular complaints that have systemic associations. In addition to diabetes, hypertension, and rheumatologic disorders, optometrists can encounter the ocular manifestations of sexually transmitted diseases such as syphilis. Syphilis is a chronic systemic infection caused by the spirochete Treponema pallidum, which is transmitted primarily through sexual contact. A recent rise in the prevalence and clinical manifestation of syphilis increa…

Treponema pallidum and HIV are both sexually transmitted agents of infectious diseases with epidemiological similarities. Ulcerous genital diseases such as primary syphilis facilitate transmission of HIV. An increasing number of case reports gives evidence that in HIV-infected patients secondary syphilis runs a more severe course and may present as syphilis maligna and that neurological complications of secondary and tertiary syphilis seem to occur more frequently and at an earlier stage. The clinical evaluation, interpretation of serology and choice of treatment schedules must take these cons…

An infant girl acquired syphilis after exchange transfusion with fresh whole blood in 1977 in Rotterdam. She showed typical features of early infectious syphilis three and a half months after the transfusion. The parents had no histories or clinical or serologic signs indicative of syphilis. Although the blood donor had negative serologic tests for syphilis shortly before giving the blood, later examination indicated that he must have become infected with Treponema pallidum shortly before donation of the blood used in the transfusion of the infant.

A 32-year-old man developed a recurrent nonrhegmatogenous retinal detachment, uveal effusion, and visual loss as a result of latent secondary syphilis. Treponemes were found in the subretinal fluid and the Treponema pallidum hemagglutination test demonstrated substantially higher titers in the subretinal fluid than in the serum (1: 2,569 vs 1: 16). Despite scleral dissection and a scleral implant and treatment with penicillin, the patient's visual loss persisted and the last examination showed a thickened choroid, a flat nonrhegmatogenous retinal detachment, and reaccumulation of subretinal fl…

Recent developments in syphilis serology are set down. The new specific tests for treponema pallidum are described. Reactivity in different stages of the disease including congenital and neurosyphilis is discussed. An outline is given of the possibilities of the IgM-FTA-ABS and 19 S IgM-FTA-ABS tests. Syphilis serology today requires only 3 tests: a screening procedure, where the specific AMHA-TP is the method of choice, FTA-ABS test should be used for confirmation, and the quantitative VDRL in order to follow up the effectiveness of treatment.

Experiments are reported in which it was shown, first, that the effectiveness of the reaction to experimental syphilis was increased in rabbits immune to vaccine virus, the ensuing disease being less severe than in control animals. It was further shown that a comparable modification of the syphilitic reaction occurred in rabbits inoculated intratesticularly with a mixture of Treponema pallidum and vaccine virus, and it was suggested that this result was due to the influence of a vaccinal immunity and to the local effect of the vaccinal reaction. It was pointed out that the manner in which two …

There is a rising incidence of several sexually transmitted infections (STIs), many of which can present with proctitis. Causative organisms include Neisseria gonorrhoeae, Chlamydia trachomatis, herpes simplex virus, Treponema pallidum (syphilis), Giardia lamblia (giardiasis) and Entamoeba histolytica (amoebiasis). This paper outlines important clinical discriminators and key investigations to distinguish these organisms from non-infective pathology that include inflammatory bowel disease, solitary rectal ulcer syndrome and Behçet's syndrome. Management of these infections is described and sug…

Syphilis is a systemic infection caused by the spirochete Treponema pallidum. It is transmitted during pregnancy or through sexual contact. Signs and symptoms vary depending on the clinical stage of the disease. Syphilis has been well-studied, and the introduction of penicillin has resulted in a decline in the number of new cases and deaths. Recently, however, there has been a surge in new cases in young people, especially those with human immunodeficiency virus (HIV). This epidemiological shift has been attributed to increased sexual activity, risky sexual behaviors, and immunodeficiency. The…
