Prospective evaluation of 2nd generation rapid diagnostic tests for the serological diagnosis of gambiense human African trypanosomiasis in the Democratic Republic of the Congo.

BACKGROUND: Accurate diagnosis is essential for achieving the 2030 goal of eliminating gambiense human African trypanosomiasis (gHAT) transmission. In recent years, novel screening and confirmatory laboratory tests, including rapid diagnostic tests incorporating recombinant antigens (2nd generation RDTs), an inhibition ELISA (iELISA) and RT-qPCR, have been developed. However, data on their diagnostic performance remain limited. This study aimed to evaluate the specificity of these newly developed tests, with particular emphasis on 2nd generation RDTs. METHODS: During routine mobile team scree…

Mental distress and health-related quality of life in gambiense human African trypanosomiasis: a case-control study in the Democratic Republic of Congo.

BACKGROUND: The extent to which neuropsychiatric sequelae affects the mental health status and quality of life of former gambiense human African trypanosomiasis (gHAT) patients is not known. METHODS: We assessed anxiety, depression and health-related quality of life (HRQoL) in 93 patients and their age- and sex-matched controls using the Hospital Anxiety and Depression Scale, Becks Depression Inventory and the 36-item Short Form Health Survey in structured interviews in the Vanga health zone in the Democratic Republic of Congo. Data were analysed using Stata version 14.0. The degree of associ…

Predicting the impact of COVID-19 interruptions on transmission of gambiense human African trypanosomiasis in two health zones of the Democratic Republic of Congo.

Many control programmes against neglected tropical diseases have been interrupted due to the coronavirus disease 2019 (COVID-19) pandemic, including those that rely on active case finding. In this study we focus on gambiense human African trypanosomiasis (gHAT), where active screening was suspended in the Democratic Republic of Congo (DRC) due to the pandemic. We use two independent mathematical models to predict the impact of COVID-19 interruptions on transmission and reporting and achievement of the 2030 elimination of transmission (EOT) goal for gHAT in two moderate-risk regions of the DRC.…

Cost-effectiveness modelling to optimise active screening strategy for gambiense human African trypanosomiasis in endemic areas of the Democratic Republic of Congo.

BACKGROUND: Gambiense human African trypanosomiasis (gHAT) has been brought under control recently with village-based active screening playing a major role in case reduction. In the approach to elimination, we investigate how to optimise active screening in villages in the Democratic Republic of Congo, such that the expenses of screening programmes can be efficiently allocated whilst continuing to avert morbidity and mortality. METHODS: We implement a cost-effectiveness analysis using a stochastic gHAT infection model for a range of active screening strategies and, in conjunction with a cost …

Feasibility of a dried blood spot strategy for serological screening and surveillance to monitor elimination of Human African Trypanosomiasis in the Democratic Republic of the Congo.

In recent years, the number of reported Human African Trypanosomiasis (HAT) cases caused by Trypanosoma brucei (T.b.) gambiense has been markedly declining, and the goal of 'elimination as a public health problem' is within reach. For the next stage, i.e. interruption of HAT transmission by 2030, intensive screening and surveillance will need to be maintained, but with tools and strategies more efficiently tailored to the very low prevalence. We assessed the sequential use of ELISA and Immune Trypanolysis (ITL) on dried blood spot (DBS) samples as an alternative to the traditional HAT field te…

Food and pathogen adaptations in the Angolan Namib desert: Tracing the spread of lactase persistence and human African trypanosomiasis resistance into southwestern Africa.

OBJECTIVES: We investigated the frequency distribution and haplotype diversity of human African trypanosomiasis (HAT) resistance and lactase persistence (LP) variants in populations from the Angolan Namib to trace the spread of these genetic adaptations into southwestern Africa. MATERIALS AND METHODS: We resequenced two fragments of the LCT enhancer and the APOL1 gene and genotyped flanking short tandem repeat loci in six groups with different subsistence traditions living in the Angolan Namib, and in a comparative dataset including other populations from Africa and Europe. LP in the Angolan …

In vivo SPECT imaging of [¹²³I]-labeled pentamidine pro-drugs for the treatment of human African trypanosomiasis, pharmacokinetics, and bioavailability studies in rats.

UNLABELLED: Pentamidine is an effective antiparasitic agent and approved drug for the treatment of African trypanosomiasis (sleeping sickness). However, pentamidine suffers from poor orally bioavailability and lacks central nervous system (CNS) delivery. Therefore its applicability is limited to intravenous or intramuscular treatment of the first stage of the African trypanosomiasis. For this reason, several new pentamidine pro-drugs have been developed with the aim of providing improved orally availability and CNS penetration. AIM: this work aims to measure and to compare the distribution, b…

A mixed methods study of a health worker training intervention to increase syndromic referral for gambiense human African trypanosomiasis in South Sudan.

BACKGROUND: Active screening by mobile teams is considered the most effective method for detecting gambiense-type human African trypanosomiasis (HAT) but constrained funding in many post-conflict countries limits this approach. Non-specialist health care workers (HCWs) in peripheral health facilities could be trained to identify potential cases for testing based on symptoms. We tested a training intervention for HCWs in peripheral facilities in Nimule, South Sudan to increase knowledge of HAT symptomatology and the rate of syndromic referrals to a central screening and treatment centre. METHO…

The use of Loop-mediated Isothermal Amplification (LAMP) to detect the re-emerging Human African Trypanosomiasis (HAT) in the Luangwa and Zambezi valleys.

BACKGROUND: Loop-mediated isothermal amplification (LAMP) is a novel strategy which amplifies DNA with high sensitivity and rapidity under isothermal conditions. In the present study, the performance of the repetitive insertion mobile element (RIME)-LAMP and human serum resistance-associated gene (SRA)-LAMP assays were evaluated using clinical specimens obtained from four male patients from Luangwa and Zambezi valleys in Zambia and Zimbabwe, respectively. FINDINGS: The cases reported in this preliminary communication were all first diagnosed by microscopy, through passive surveillance, and co

The vervet monkey (Chlorocebus aethiops) as an experimental model for Trypanosoma brucei gambiense human African trypanosomiasis: a clinical, biological and pathological study.

It has long been known that the vervet monkey, Chlorocebus (C.) aethiops, can be infected with Trypanosoma rhodesiense, but this model has not been described for T. gambiense. In this study, we report the development of such a model for human African trypanosomiasis. Twelve vervet monkeys infected with T. gambiense developed chronic disease. The duration of the disease ranged between 23 and 612 days (median 89 days) in five untreated animals. Trypanosomes were detected in the blood within the first 10 days post-infection and in the cerebrospinal fluid, with a median delay of 120 days (n = 4, r…

[Camel trypanosomiasis caused by Trypanosoma evansi (Steel 1885), Balbiani 1888, in Islamic Republic of Mauritania: results of surveys in the Trarza region].

A first survey concerning camel trypanosomiasis due to Trypanosoma evansi was carried out in the Trarza region (south-western Mauritania). Blood smears were made from 218 animals and 160 were submitted to an indirect immunofluorescence test. Young calves below one year old seem to be free of T. evansi infection, while in dairy females, average rates of infection of 7.3% (blood smears) and 24.5% (serological results) were found. The type of herd management seems to be an important risk factor: the herds that frequent, even temporarily, the wooded areas of the Senegal river valley or permanent w…

Epidemiology of bovine trypanosomiasis in the Ghibe valley, southwest Ethiopia. 3. Occurrence of populations of Trypanosoma congolense resistant to diminazene, isometamidium and homidium.

In July 1989, blood samples were collected from parasitaemic cattle in the Ghibe valley, Ethiopia, frozen in liquid nitrogen and transported to Nairobi, Kenya. Twelve of the stabilates were inoculated into individual Boran (Bos indicus) calves and characterised for their sensitivity, in turn, to diminazene aceturate (Berenil), isometamidium chloride (Samorin) and homidium chloride (Novidium). All 12 stabilates produced infections which were shown to be Trypanosoma congolense and resistant to treatment with diminazene aceturate at a dose of 7.0 mg kg-1 body weight (b.w.). Eleven of the infectio

[Characteristics of trypanosomiasis in children. Apropos of 19 case reports at the CNPP (Neuro-Psycho-Pathology Center), University Hospitals of Kinshasa, Zaire].

The clinical study of 19 cases of trypanosomiasis in children aged 2 to 13 years showed an image similar to that in adults but the circumstances of infestation and especially the evolution should receive special attention. It was of interest to note that maternal trypanosomiasis frequently was an anamnestic element in the diagnosis of these children. The short and long term evolution depend on the neuropsychological sequels which are themselves related to the gravity of the infection at initial examination. The errors in diagnosis and the delay in the institution of specific treatment are equa…

Trypanosomiasis control in Boran cattle in Kenya: a comparison between chemoprophylaxis and a parasite detection and intravenous treatment method using isometamidium chloride.

Two methods of trypanosome control in Boran cattle kept under very high trypanosomiasis risk were compared: the traditional intramuscular isometamidium chloride prophylaxis with a parasite detection and intravenous isometamidium chloride treatment method. The results were related to a control group under diminazene aceturate treatment. Isometamidium chloride at 0.25 mg/kg as routinely used by the ranch was of little benefit by either method, with breakthrough infections occurring as early as one week after treatment. When isometamidium chloride at 1 mg/kg was used, the curative intravenous met…

[Contribution of immunoglobulin assays to the diagnosis of human African trypanosomiasis in patients immunologically suspect (immunofluorescence and ELISA). A preliminary report (author's transl)].

Can the diagnosis of sleeping sickness in patients immunologically suspect, without seeing the parasite, be obtained by dosing IgG and IgM? The authors demonstrate that two groups of people with positive immunodiagnosis, one parasitologically confirmed and the other not confirmed, do not show significant difference when dosing immunoglobulins. These two groups respectively show a significant difference when compared with a parasitologically and immunologicaly negative group. The simultaneous increase in IgG and IgM might confirm the immunodiagnosis.
