Snoring is a common complaint in the community. Its psychosocial effect upon the relationship between sleeping partners is not known. This study examines prospectively by detailed tailored questionnaires the differences in the physical, psychological and social aspects in 27 snoring patients and their partners presenting to a snoring clinic and compares them with a control group of 24 patients who had undergone successful Laser-Assisted Uvulopalatoplasty (LAUP) and their partners. All areas examined showed improvement in the non-snoring group, particularly for the patient. Improvement was demo

OBJECTIVE: The purpose of this study was to investigate the histologic alterations in the upper airway in patients with obstructive sleep apnea syndrome.
STUDY DESIGN: Surgical specimens were obtained during non-laser-assisted uvulopalatopharyngoplasty from 5 adult patients (4 male and 1 female) with polysomnographically confirmed obstructive sleep apnea syndrome.
RESULTS: In all of the specimens, interstitial edema was present with mild inflammatory reaction. Fat was not at all prominent.
CONCLUSIONS: It is confirmed histologically that there is edema present in the upper airway in patient

OBJECTIVE: To assess the efficacy of an author-modified LAUP technique using the principles of UPPP surgery. This new one-stage method of LAUP is described in detail.
DESIGN: A retrospective review of all one-stage LAUPs performed.
SETTING: The Division of Otolaryngology, University of British Columbia, Vancouver, BC.
METHOD: This one-stage LAUP was performed on all patients.
MAIN OUTCOME MEASURES: Improvement of snoring, postoperative pain, polysomnography, home oximetry, the Respiratory Disturbance Index, and the need for a second treatment.
RESULTS: Two hundred twenty patients have und

It is now widely accepted that snoring causes significant social dysfunction. In the absence of obstructive sleep apnoea syndrome, palatal surgery offers a very good chance of eliminating or reducing snoring. The traditional operation of uvulopalatopharyngoplasty remains the 'gold standard', but may be complicated by velopharyngeal incompetence, severe post-operative pain and even nasopharyngeal stenosis. A newer technique to reduce snoring caused by palatal flutter by using a neodymnium:yttrium aluminum garnet laser to stiffen the soft palate has been introduced recently by another unit. We s

Regular, continuous snoring usually does not have any sleep disturbing effects; however, discontinuous snoring may be associated with increased upper airway resistance and leads to recurrent arousals, sleep fragmentation and excessive daytime sleepiness. Loud snoring, recurrent obstructive apneas and oxygen desaturation during sleep with excessive daytime sleepiness are the hallmarks of the obstructive sleep apnea syndrome (OSAS). A tentative diagnosis of disturbed breathing during sleep can be made from a careful history and confirmed by a sleep study. Continuous positive airway pressure (CPA

The effect of stiffening the soft palate by inducing scarring after removing a central strip of mucosa with a CO2 laser was investigated in 25 heavy snorers. The results were assessed using a series of Visual Analogue Score (VAS) assessment questionnaires completed by the partner over a period of 6 months. Of the 22 patients who had the laser-strip and uvulectomy, snoring improved in 18 (82%) (median improvement of 75% at 3 months). Four patients did not improve. Between 3 and 6 months, snoring increased in one patient and decreased in four (< 10% change on VAS). Snoring did not improve in the

This case report describes 3 patients with Down syndrome and obstructive breathing problems, ages: 5 months, 15 months, and 22 years. The youngest one had normal cardiopulmonary function at birth, but soon developed a pulmonary hypertension. The next had a severe atrioventricular defect and additional pulmonary hypertension and there was little hope for her to survive heart surgery. The oldest one had had apneas since childhood with increasing severity, but was regarded as having normal heart function. All 3 were operated to relieve their breathing obstruction. The 5-month-old boy improved onl

We performed nocturnal diagnostic polysomnography (PSG 1), PSG early after UPPP on the second to the fifth postoperative night (PSG 2) and PSG late after UPPP (PSG 3) six or more weeks after surgery, on 15 male patients with obstructive sleep-disordered breathing. On PSG 1, the A + H/I for the group was 66.6 +/- 8.8 (mean +/- SE). During non-REM sleep the A + H/I on PSG 3 (29 +/- 10) was lower than it was on PSG 2 (54.3 +/- 11.3) (p = 0.004) and lower than that on PSG 1 (70.8 +/- 10.3) (p = 0.003). Similarly, during non-REM sleep, the AI on PSG 3 (16.1 +/- 7.4) was less than that on PSG 2 (39.

Obstructive sleep apnea is the underlying cause of a variety of pediatric maladies, including pulmonary hypertension and failure to thrive. In children, unlike adults, obstruction secondary to lymphoid hyperplasia is often encountered; adenotonsillectomy restores airway patency. Patients who fail this procedure, such as children with cerebral palsy and associated muscular hypotonia, may face tracheotomies. We report on 10 pediatric patients with severe mental insufficiency and obstructive sleep apnea in whom palatal hypotonicity and lack of adenotonsillar hypertrophy was identified. Uvulopalat

The objective of this study was to analyze the results of uvulopalatopharyngoplasty on 133 adult patients with sleep apnea. This group of patients was subjected to computer analysis by a statistician. Two previously unreported observations were revealed: There was a straight line decline in measurable improvement with advancing age so, by age 60, there were no patients with improvement; and patients who had tonsillectomy as a part of the uvulopalatopharyngoplasty had a markedly increased success rate compared to those who did not have tonsils. These two previously unreported observations might

The results of the treatment of the obstructive sleep apnea syndrome with tracheostomy (68 patients), continuous positive airway pressure (67 patients) or uvulopalatopharyngoplasty (14 patients) have been presented. The best results were after tracheostomy or during continuous positive airway pressure application, worse results gave uvulopalatopharyngoplasty.

The interpretation of panoramic radiographs requires an understanding of the formation of real and ghost images in rotational panoramic radiology. This article illustrates the use of principles of interpretation in panoramic radiology to identify paraoral soft tissue calcifications, which appear as lesions in the third molar-ramus region of the mandible. The known object rule, a variation of the buccal object rule, is presented as an aid in identifying soft tissue calcifications of the region of the head and neck.

BACKGROUND: The syndrome of Loeys-Dietz (LDS) is a rare connective tissue disorder. A classic triad of symptoms is seen: hypertelorism, atypical uvula or clef palate, and multiple tortuous arteries and aneurysms of the aorta and main arterial branches. Mutations in genes involving the transforming growth factor-beta (TGFB) signaling pathway are the cause of this syndrome. There are six subtypes of LDS, categorized based on the gene mutation that is involved. LDS type V and VI, concerning the TGFB3 and SMAD2 gene respectively, are the two subtypes that are least frequently seen. Mostly, in the …
