INTRODUCTION AND IMPORTANCE: Mucoepidermoid carcinoma (MEC) is an extremely rare type of lung cancer, accounting for approximately 0.2-0.4 % of all lung cancers. It typically presents as a central airway lesion, with symptoms such as cough, hemoptysis, bronchitis, wheezing, fever, chest pain, and clubbing of the fingers. Histologically similar to tumors in major salivary glands, it mostly arises from small salivary glands that line the tracheobronchial tree.
CASE PRESENTATION: We have reported a rare case of intrabronchial mucoepidermoid carcinoma that extended extra-bronchially in a 25-year-

BACKGROUND: Despite demonstration of its clear benefits relative to open approaches, a video-assisted thoracic surgery technique for pulmonary lobectomy has not been universally adopted. This study aims to overcome potential barriers by establishing the essential components of the operation and determining which steps are most useful for simulation training.
METHODS: After randomly selecting experienced thoracic surgeons to participate, an initial list of components to a lower lobectomy was distributed. Feedback was provided by the participants, and modifications were made based on anonymous

BACKGROUND: Most surgeons experience work-related pain and musculoskeletal injuries (WRMSI). Though robotic surgery has been speculated to reduce the risk of WRMSI compared to open and laparoscopic approaches, the majority of robotic surgeons still experience neck, back, and upper extremity discomfort. Despite growing use of robot-assisted surgery (RAS), there is limited research on the ergonomic risks faced by robotic surgeons. This study aims to evaluate ergonomic strain during RAS.
METHODS: Artificial intelligence (AI)-assisted video analysis was used on a convenience sample of experienced

BACKGROUND: Involuntary craniofacial erythema, or blushing, due to autonomic dysfunction can be a cause of psychological distress. Although anecdotal reports have suggested that pharmacologic treatments or cognitive behavioral therapy can be used to treat the condition, no rigorous analyses of their efficacy have been reported.
OBJECTIVES: To assess the efficacy of video-assisted thoracoscopic sympathectomy and to study phobic anxiety and other personality traits in a series of patients with involuntary facial blushing.
MATERIALS AND METHODS: We carried out a retrospective observational stud

Accurate and efficient recognition of surgical actions in videos is critical for advancing AI-driven surgical robotics. However, current surgical video action recognition (SVAR) datasets suffer from limitations such as small scale, low resolution, inconsistent annotations, and insufficient action coverage. Most latest video recognition models are trained on large-scale common datasets and underperform in SVAR due to architectures that suppress high-frequency visual details (crucial for recognizing surgical tools and motions) and lack a strong spatial inductive bias, requiring extensive trainin

OBJECTIVES: Lung volume reduction surgery (LVRS) is a clinically effective palliation procedure for patients with chronic obstructive pulmonary disease. LVRS has recently been commissioned by the NHS England. In this study, a costing model was developed to analyse cost and resource implications of different LVRS procedures.
METHODS: Three pathways were defined by their surgical procedures: bronchoscopic endobronchial valve insertion (EBV-LVRS), video-assisted thoracic surgery LVRS and robotic-assisted thoracic surgery LVRS. The costing model considered use of hospital resources from the LVRS

Enhanced recovery after surgery (ERAS) protocols are increasingly adopted in thoracic surgery to optimize recovery, but evidence integrating surgical outcomes with imaging-based postoperative recovery assessment remains limited, particularly for video-assisted thoracoscopic surgery. This single-center retrospective cohort study included patients undergoing elective video-assisted thoracoscopic surgery, divided into an ERAS group and a conventional care group. Outcomes analyzed included clinical indices (postoperative complications, chest tube duration, hospital stay), numerical rating scale pa

BACKGROUND: Video-assisted thoracic surgery has been shown to be a safe and effective method of closing the patent ductus arteriosus in infants and children. We have applied this technique in low birth weight premature infants and now report our experience.
METHODS: Since 1996, we have used video-assisted thoracic surgery ligation as the treatment of choice for all patent ductus arteriosus, including 100 performed on premature infants (23 to 31 weeks' gestation, mean 25.6 weeks; 0.420 to 1.5 kg, mean 0.859 kg). A modification of our previously described technique was used with a three-port ap

OBJECTIVE: Anesthetic management of patients with giant mediastinal tumors is challenging from the perspective of both cardiovascular and respiratory management, and airway assessment is important for both concerns. We report the successful induction of general anesthesia and double-lumen tube intubation in the right lateral position for a patient with a giant mediastinal tumor with tracheal compression, using pre-operative chest radiograph imaging to minimize tracheal compression during induction.
METHODS: A 41-year-old man required thoracoscopic giant superior mediastinal tumor resection. H

INTRODUCTION: There are several anomalies of the pulmonary vessels. Clinicians need to be well informed about anatomy, particularly before video-assisted thoracic surgery, to prevent fatal complications.
PRESENTATION OF CASE: We report the case of an 80-year-old woman who was suspected of having lung cancer in the right lower lobe. The patient was accordingly scheduled for surgery. Three-dimension multidetector computed tomography (3D-MDCT) showed an extremely rare anomaly in which A4 + 5 ran between V2 and V1 + 3. We scheduled a non-anatomical wedge resection of the lesion and performed rapi

A 38-year-old man with bilateral Pryce III intralobar sequestration was admitted. At first, left lower lobectomy and division of the aberrant artery were performed because of continuous hemoptysis after admission. Two months later, we planned a video-assisted thoracoscopic wedge resection of the right sequestration and a division of the 2 aberrant arteries. We chose indocyanine green fluorescence navigation because of the small volume of the right sequestration, with a goal to reduce respiratory functional loss as much as possible. After the aberrant arteries and the small pulmonary vein of th

BACKGROUND: The standard treatment of care for melanoma metastatic to the inguinal lymph node basin is lymphadenectomy. However, up to 50% of patients forgo the operation partly due to concerns about morbidity. Videoscopic inguinal lymphadenectomy (VIL) is a minimally invasive technique designed to minimize wound complications while achieving comparable oncological control.
METHODS: We reviewed pertinent literature related to open inguinal lymphadenectomy and VIL specific to melanoma, offering personal experience where appropriate.
RESULTS: Despite efforts to minimize the complications of op

Parenchymal-preserving surgery is the preferred technique for selected patients with centrally located airway tumors limited to the secondary carina; however, reconstruction is challenging. We present a simplified video-assisted thoracoscopic surgery technique for secondary carinal reconstruction with three barbed sutures on each side. In each patient, the first thread was used to approximate the main bronchus and lower lobe bronchus on the left side or the bronchus intermedius and upper lobe bronchus on the right side. A horizontal mattress suture was critical to secure the backward joint sec

Robotic-assisted thoracic surgery is increasingly recognized for its utility in complex pulmonary resections, offering enhanced dexterity, superior 3-dimensional visualization and precise, 'tremor-free' tissue manipulation. Compared to open thoracotomy, minimally invasive robotic techniques are associated with reduced perioperative morbidity, shorter hospital stays and faster recovery. Sublobar lung resections, however, remain technically demanding, especially in the deep anatomical segments such as S8. Three-dimensional reconstructive imaging provides detailed preoperative visualization of th

We report a case of 19-year-old asymptomatic man with posterior mediastinal mass thought to be of neurogenic origin on computed tomography scan. During video-assisted thoracic surgery the mass appeared to be an extralobar pulmonary sequestration. Surgery was straightforward by division of vascular pedicle. Histopathology confirmed diagnosis.
