A case of melanoma-associated retinopathy with autoantibodies against TRPM1.

PURPOSE: To report a case of melanoma-associated retinopathy (MAR) with autoantibodies against the transient receptor potential cation channel, subfamily M, member 1 (TRPM1) with asymmetric severe vision loss. METHODS: We evaluated a patient with heel skin melanoma showing progressive vision loss in both eyes confirmed with a baseline ophthalmic examination, fluorescein angiography, spectral domain optical coherence tomography (OCT), visual field test, and full-field electroretinogram (ERG). Immunofluorescence assays and western blot analysis revealed autoantibodies in the patient's serum. R

Fixational eye movements enable robust edge detection.

Human vision relies on mechanisms that respond to luminance edges in space and time. Most edge models use orientation-selective mechanisms on multiple spatial scales and operate on static inputs assuming that edge processing occurs within a single fixational instance. Recent studies, however, demonstrate functionally relevant temporal modulations of the sensory input due to fixational eye movements. Here we propose a spatiotemporal model of human edge detection that combines elements of spatial and active vision. The model augments a spatial vision model by temporal filtering and shifts the in

Clinical course of a cohort in the Cuban epidemic optic and peripheral neuropathy.

Nearly 51,000 Cubans were afflicted during an outbreak of an optic neuropathy (ON) and peripheral neuropathy (PN) between 1991 and 1993. We re-examined 14 of 20 affected individuals 16 months after an initial evaluation. The optic features were painless symmetric vision loss with poor visual acuity, color vision loss, central or cecocentral scotoma, optic disc pallor, and nerve fiber layer drop-out. The neurologic symptoms included stocking-glove sensory changes, hearing loss, leg cramps, sensory ataxia, hyperactive or absent reflexes, and complaints of memory loss. Two of 11 ON probands teste

Acquired Visual Deficits Independent of Lesion Site in Acute Stroke.

Most clinical diagnoses of stroke are based on the persistence of symptoms relating to consciousness, language, visual-field loss, extraocular movement, neglect (visual), motor strength, and sensory loss following acute cerebral infarction. Yet despite the fact that most motor actions and cognition are driven by vision, functional vision per se is seldom tested rigorously during hospitalization. Hence we set out to determine the effects of acute stroke on functional vision, using an iPad application (Melbourne Rapid Field-Neural) that can be used to assess vision (visual acuity and visual fiel

Novel Concept and Method of Endoscopic Urethral Stricture Treatment Using Liquid Buccal Mucosal Graft.

PURPOSE: We describe a novel method of urethral stricture treatment using liquid buccal mucosal grafts to augment direct vision internal urethrotomy. MATERIALS AND METHODS: A rabbit stricture model was used to test this method. In phase 1 the concept of endoscopic liquid buccal mucosal graft implantation was tested by performing direct vision internal urethrotomy in 3 rabbits with immediate intraurethral injection of autologous liquid buccal mucosal grafts suspended in fibrin glue. Animals were sacrificed at 2 to 3 weeks and the urethras were examined for the presence of buccal mucosa engraft

Progressive visual and hearing loss secondary to neurosyphilis.

PURPOSE: There is a rising incidence of syphilis in economically advanced countries. Early diagnosis can spare the patient devastating vision and hearing loss, as well as other significant morbidity. Ocular presentations of neurosyphilis are varied and numerous, requiring syphilis to be considered in many clinical situations, even if a negative medical history is reported. Features of Argyll Robertson pupils, however, are virtually pathognomonic of syphilis; therefore, careful pupil testing can be the key to making a correct and timely diagnosis. CASE REPORT: A 51-year-old woman with a histor

A Comparative Study to Assess the Accommodation and Vergence Relationship of Myopia in Indian Adolescent.

BACKGROUND: Accommodation and Vergence disorder are diverse visual anomalies which can interfere with a child's school performance and impair one's ability to function efficiently. Its association with refractive error and its intervention were studied less in Indian myopia children; hence, there is a need for research in such setting. METHOD: One hundred and fifty Indian adolescents aged 10 to 17 years were divided into three refractive error groups (high, moderate, and low myopia). Baseline vision examination and a comprehensive binocular vision assessment were performed on all eligible ado

A method to characterize compensatory oculomotor strategies following simulated central vision loss.

Loss of central vision can be partially compensated by increased use of peripheral vision. For example, patients experiencing central vision loss due to disease (macular degeneration) or healthy participants trained with simulated central vision loss, tend to develop eccentric fixation spots for reading or other visual tasks. In both patients and in simulated conditions, there are substantial individual variations in the effective use of the periphery. The factors driving these individual differences are still unclear. Although early approaches have described some dimensions of these strategie

Analysis of optical side-effects of fs-laser therapy in human presbyopic lens simulated with modified contact lenses.

BACKGROUND: In a single-blinded study, optical side-effects of a potential femtosecond (fs)-laser therapy in presbyopic human lenses were tested. Simulation of this therapy was carried out by applying fs-laser patterns into standard contact lenses (CL). METHODS: In the first part of the study, the influence of the numerical aperture on optical side-effects was investigated by comparing a typical fs-LASIK configuration to a fs-presbyopia treatment (n = 11). The second part focused on a possible improvement of visual performance by comparing a regular grid pattern to a randomly chosen spacing o

Evaluating a web-based visual acuity and refractive error self-assessment tool in myopic children.

INTRODUCTION: Demands for myopia management are rising. A web-based tool that allows home-performed self-assessments of visual acuity (VA) and refractive error may enable hybrid care pathways and aid in identifying those with deteriorating visual performance. The tool has been validated in adult populations, but has yet to be evaluated in children. This study compared home-performed VA and refraction self-assessments to conventional measurements obtained at the clinic in a population of myopic children. METHODS: Myopic children aged ≥6 years old were invited to perform web-based eye tests at

A Smart Context-Aware Hazard Attention System to Help People with Peripheral Vision Loss.

Peripheral vision loss results in the inability to detect objects in the peripheral visual field which affects the ability to evaluate and avoid potential hazards. A different number of assistive navigation systems have been developed to help people with vision impairments using wearable and portable devices. Most of these systems are designed to search for obstacles and provide safe navigation paths for visually impaired people without any prioritisation of the degree of danger for each hazard. This paper presents a new context-aware hybrid (indoor/outdoor) hazard classification assistive tec

Spectral sensitivity and color discrimination changes in glaucoma and glaucoma-suspect patients.

Color vision changes may occur early in the course of glaucoma and may precede visual field loss. Glaucoma suspects, having raised intraocular pressure and no diagnostic optic nerve head or visual field changes, may also have color vision loss. Unfortunately, the instruments used in the studies that have demonstrated these color vision changes were not feasible for routine clinical use; likewise, the studies did not carefully control for the effects of small pupil size and age or did not point to the underlying mechanisms responsible. We studied 19 glaucoma patients, 19 glaucoma suspects, and

COlour vision assessment for the Detection of Early diabetic Retinopathy (CODER): A study protocol.

INTRODUCTION: Diabetic retinopathy (DR) remains one of the leading causes of preventable vision loss. However, high-contrast visual acuity, which is routinely used in the clinic, is often preserved during the early stages of the disease, limiting its usefulness in detecting early functional deficits. Colour vision is a sensitive marker for detecting these early retinal changes. This study aims to investigate whether the Colour Assessment and Diagnosis (CAD) test, a quantitative measure of chromatic sensitivity, can serve as a functional biomarker for the early detection of DR and can reflect t

Progressive peritumoral edema defining the optic fibers and resulting in reversible visual loss.

Hemangioblastomas are frequently associated with peritumoral edema caused by extravasation of plasma ultrafiltrate through permeable neoplastic vessels. The authors report the clinical and imaging findings in a 62-year-old man with von Hippel-Lindau disease who presented with rapid (within 24 hours) loss of color vision and nearcomplete loss of left eye vision (acuity too poor to test). Serial MR imaging demonstrated a stable vascular tumor in the medioinferior aspect of the left optic nerve, associated with progressive edema extending from the nerve through to the bilateral optic radiations.
