Unraveling the Material Basis of Compound Nanxing Zhitong Plaster: A Multimodal Study Combining Chemical Analysis, Network Pharmacology, and Transdermal Pharmacokinetics.

Compound Nanxing Zhitong Plaster (CNZP) is a topical formulation widely used for rheumatoid arthritis (RA), yet its pharmacodynamic material basis is not fully understood. We established a strategy integrating chemical analysis, network pharmacology, and multi-compartment transdermal pharmacokinetics. Comprehensive chemical profiling by UPLC-Q-TOF-MS identified 100 constituents. Network pharmacology screened 15 core bioactive components. A validated UPLC-QqQ-MS/MS method was developed to simultaneously quantify eight components in the stratum corneum (SC) and viable epidermis plus dermis (VD)

Targeting prostaglandin catabolism via topical nanotherapy rescues vision in ischemic optic neuropathy.

Ischemic optic neuropathy is a leading cause of acute vision loss and currently lacks effective therapy. Here, we identify 15-hydroxyprostaglandin dehydrogenase (15-PGDH), the key enzyme responsible for prostaglandin degradation, as a metabolic checkpoint in ischemic optic neurodegeneration. Analysis of human ischemic optic nerve tissue and a murine model of optic nerve ischemia reveals marked upregulation of retinal 15-PGDH, suggesting dysregulated prostaglandin homeostasis after ischemic injury. To therapeutically target this pathway, we develop a nano-micellar formulation of the hydrophobic

Deep-Penetrating Transdermal Lipopeptide Liposomes for Sustained IL-17 Inhibition and Prevention of Psoriatic Recurrence.

Psoriasis is a multisystemic, immune-mediated inflammatory disease characterized by high recurrence rates upon treatment cessation. Biotherapeutics are highly anticipated for treating psoriasis via transdermal delivery. However, the topical administration of potent immunomodulators is fundamentally hindered by the stratum corneum barrier, stringent molecular size constraints, and rapid proteolytic degradation within the skin microenvironment. Here, we report the development of transdermal lipopeptide liposomes (TLLs) designed for the site-specific delivery of a double-bridged cyclic tetradecap

Topical MTH1 Inhibition Suppresses SKP2-WNT5a-Driven Psoriatic Hyperproliferation.

Topically applied TH1579 alleviated the psoriatic phenotype in the imiquimod-induced psoriasis mouse model by decreasing CD45+, Ly6b+, and CD3+ cell infiltration and downregulating the expression of the proliferation marker PCNA. Moreover, TH1579 strongly suppressed IL-17 expression in mouse skin, accompanied by reduced infiltration of IL-17-producing γδ-T cells. Furthermore, TH1579 decreased keratinocyte viability and proliferation. Mass spectrometry data analysis revealed the enrichment of proteins associated with nucleotide excision repair and cell cycle regulation. The key cell cycle regul

Complete Resolution of Cutaneous Leishmaniasis With a Novel Topical Combination Therapy: A Case Series.

Cutaneous leishmaniasis (CL) remains a therapeutic challenge in endemic regions, with 600,000-1 million new cases annually facing limited treatment options. We present a case series evaluating an innovative topical combination therapy for acute-phase CL in Shiraz, Iran. Three treatment-naïve patients (45-year-old female with forearm lesions, 18-year-old male with foot involvement, and 52-year-old female with hand lesions) received daily applications of a novel solution containing ciprofloxacin, ketoconazole, and metronidazole for 30 days, followed by alpha hydroxy acid for scar rehabilitation.

A case of Corynebacterium propinquum microbial keratitis after amniotic membrane transplantation.

BACKGROUND: Amniotic membrane transplantation (AMT) is a relatively safe procedure with few complications. Postoperatively, patients are treated with topical antibacterial eye drops. However, even antibacterial drug administration cannot completely prevent antimicrobial-resistant keratitis. This paper presents a case of Corynebacterium propinquum keratitis associated with AMT. CASE PRESENTATION: An 84-year-old female patient who underwent AMT for bullous keratopathy developed keratitis with colony formation on the transplanted amniotic membrane graft 45 days postoperatively. The colony was sc

Enhanced Ocular Delivery of Beva via Ultra-Small Polymeric Micelles for Noninvasive Anti-VEGF Therapy.

Pathological ocular neovascularization resulting from retinal ischemia constitutes a major cause of vision loss. Current anti-VEGF therapies rely on burdensome intravitreal injections of Bevacizumab (Beva). Herein ultrasmall polymeric micelles encapsulating Beva (P@Beva) are developed for noninvasive topical delivery to posterior eye tissues. Beva is efficiently loaded into 11 nm micelles fabricated via self-assembly of hyperbranched amphiphilic copolymers. The neutral, brush-like micelles demonstrate excellent drug encapsulation and colloidal stability. In vitro, P@Beva enhances intracellular

Permeation enhancer nanovesicles mediated topical delivery of curcumin for the treatment of hyperpigmentation.

The main aim of the present study was to develop curcumin (CUR) loaded permeation enhancer-lipid vesicles for the treatment of hyperpigmentation. Hyperpigmentation is an acquired skin disorder characterized by uneven skin coloration, mainly in the regions of the facial skin, affecting millions of people worldwide. It often occurs in visible areas, hence causing significant negative psychological and social impacts. In the present study, curcumin-loaded permeation enhancer nanovesicles (PE-NVs) were developed by modified ethanol injection method and dimethyl sulfoxide was added as a penetration

Hyaluronic acid-based supramolecular medicine with polyamines sequestration capability for cooperative anti-psoriasis.

Psoriasis seriously harms physical and mental health of patients. Hyaluronic acid (HA)-based topical formulation can increase drug concentration in psoriatic skin via CD44-assisted targeting. Herein, we developed a supramolecular medicine composed of curcumin-loaded HA-cucurbit[7]uril (HA-CB[7]@Cur), which could efficiently sequester polyamines (PAs) via host-guest interactions of CB[7] and PAs to suppress RNA-PAs immunocomplex formation. Meanwhile, anti-psoriasis drug Cur could be released from HA-CB[7]@Cur by PAs. With phenotypical disease evaluation, psoriasis area measurements and severity

Clay Nanotube Immobilization on Animal Hair for Sustained Anti-Lice Protection.

Topical administration of drugs is required for the treatment of parasitic diseases and insect infestations; therefore, fabrication of nanoscale drug carriers for effective insecticide topical delivery is needed. Here we report the enhanced immobilization of halloysite tubule nanoclay onto semiaquatic capybaras which have hydrophobic hair surfaces as compared to their close relatives, land-dwelling guinea pigs, and other agricultural livestock. The hair surface of mammals varies in hydrophobicity having a cortex surrounded by cuticles. Spontaneous 1-2 µm thick halloysite hair coverages on the

Ultraflexible lipid vesicles allow topical absorption of cyclosporin A.

Psoriasis and atopic dermatitis are widespread pathologies with a need to improve their treatment. Topical administration of cyclosporine A (CyA) could be used if targeted to the skin, thus avoiding systemic levels. Unfortunately, CyA molecular weight and lipophilicity prevent its diffusion through human skin. Four novel lipid vesicles have been prepared by different methodologies to overcome this problem. The vesicles were characterized in terms of particle size, size polydispersity, Z-potential, morphology, drug encapsulation, phospholipid content, and vesicle flexibility. Freeze-drying in p

Long-acting liposomal corneal anesthetics.

Eye drops producing long-acting ocular anesthesia would be desirable for corneal pain management. Here we present liposome-based formulations to achieve very long ocular anesthetic effect after a single eye drop instillation. The liposomes were functionalized with succinyl-Concanavalin A (sConA-Lip), which can bind corneal glycan moieties, to significantly prolong the dwell time of liposomes on the cornea. sConA-Lip were loaded with tetrodotoxin and dexmedetomidine (sConA-Lip/TD), and provided sustained release for both. A single topical instillation of sConA-Lip/TD on the cornea could achieve

Topical and systemic antifungals in dermatology practice.

Dermatophytosis is generally defined as an infection of the hair, nails, or glabrous skin. These infections are caused by the keratinophilic fungi Trichophyton spp., Microsporum spp., and Epidermophyton, which have been recovered from both symptomatic and asymptomatic individuals. Although dermatophytosis is generally not a life-threatening condition, these types of infections are among the most common infections worldwide, and their incidence has continued to increase consistently in recent years. Area covered: This article provides an overview of the general characteristics of dermatophytes,

Successful Treatment of Demodex gatoi with 10% Imidacloprid/1% Moxidectin.

Demodex gatoi is a transmissible, short-bodied mite found in the stratum corneum of cats. It is known to cause moderate to intense pruritus, often manifested as self-induced alopecia and excoriations. 10% imidacloprid/1% moxidectin (a) is a monthly, topical solution indicated for the treatment and/or prevention of fleas, heartworms, intestinal parasites, and ear mites. A household of cats was presented for pruritus that ranged from mild to severe in 8 of the 13 felines. The most common clinical signs included miliary-type papular dermatitis and focal areas of erythema associated with alopecia.

HIV microbicides: where are we now?

Most human immunodeficiency virus (HIV) infections are acquired during sexual contact, across the genital or rectal mucosal epithelium. At present, HIV preventive strategies such as behavioral and structural interventions (e.g., counseling and condom use) or pre-exposure prophylaxis (e.g., topical microbicides or the oral administration of antiretroviral drugs) seem to be the only effective and most indicated methods against the establishment of systemic HIV infection. The recent success of the CAPRISA 004 phase II clinical trial, using a tenofovir-based gel, as well as the iPreX trial, using

Photoprotection by sunscreens with topical antioxidants and systemic antioxidants to reduce sun exposure.

Skin cancer is the most common cancer diagnosed in the United States, and its incidence continues to rise. Epidemiological studies have documented that excessive sun exposure increases the risk of developing nonmelanoma skin cancer. Consequently, it is mandatory that the skin be protected from the damage that occurs from ultraviolet (UV) exposure. It is the purpose of this report to review the scientific basis for photoprotection by sunscreens, topical antioxidants, and systemic antioxidants to minimize the harmful effect of sun exposure. The US Food and Drug Administration regulates sunscreen

Clinical application of nerve growth factor on human corneal ulcer.

Nerve growth factor (NGF) is a neurotrophic and immunomodulatory factor contributing of the control of cutaneous morphogenesis, wound healing and inflammatory responses. Following the evidence that topical administration of NGF leads to healing of human corneal ulcers, we investigate the therapeutic action of NGF on immune and/or autoimmune cutaneous ulcers. We found that 1-10 micrograms of highly purified murine NGF dissolved in 50 microliters of physiological solution and topically applied to skin ulcer leads, after 4-12 weeks of daily treatment (depending on the size and depth of the lesion
