[Alcoholic heart disease].

Alcoholic heart disease is caused by a lifestyle in which alcoholics are continue to consume an excessive amount of alcohol over a long period of time. Total abstinence is a very effective way to treat them to prevent the development of the final stage of this disease. In contrast, repetitive drinking of massive amount of alcohol is very harmful and causes exacerbation of this disease. From our clinical studies, six candidates were nominated as symptoms of alcoholic heart disease, namely(1) tachyarrhythmias (incidence: 33%), (2) left ventricular hypokinesis(17%), (3) QT interval prolongation(1

[Alcohol and rhythm disorders].

Cardiovascular disease is the main cause of mortality in chronic alcoholics. There is a clear association between excessive alcohol consumption and the risk of sudden cardiac death. The pro-arrhythmogenic effect of ethanol could be responsible for some of these cases of arrhythmia and sudden death in subjects with an alcoholic cardiomyopathy and also in those with an apparently normal heart. In any case of supraventricular or ventricular arrhythmia in a chronic alcoholic or in an occasional heavy drinker, the potential role of alcohol consumption in the initiation of these disorders should be

Alcohol and visual performance.

1. The authors examined the effect of acute alcohol consumption on a set of visual tasks: visual short term memory, depth perception, and attention. 2. In a repeated measurement design, thirteen subjects performed the tasks once sober and once intoxicated with 0.8 g/kg body weight pure ethanol in orange juice (33% alcohol). Subjects underwent a neuropsychological (Benton test) and a psychophysical test (vernier discrimination) both assessing visual short term memory, the test d2 as a measure of attention and concentration, and a psychophysical depth perception task. 3. Subjects demonstrated si

Alcohol-histamine interactions.

Alcohol and histamine metabolic pathways in the body have the common enzymes aldehyde dehydrogenase and aldehyde oxidase. The metabolite of ethanol, acetaldehyde, can effectively compete with the metabolites of histamine, methylimidazole acetaldehyde, and imidazole acetaldehyde. At the periphery, alcohol and acetaldehyde liberate histamine from its store in mast cells and depress histamine elimination by inhibiting diamine oxidase, resulting in elevated histamine levels in tissues. Histamine mediates alcohol-induced gastric and intestinal damage and bronchial asthma as well as flushing in Orie

Medications and alcohol craving.

The use of medications as an adjunct to alcoholism treatment is based on the premise that craving and other manifestations of alcoholism are mediated by neurobiological mechanisms. Three of the four medications approved in the United States or Europe for treating alcoholism are reported to reduce craving; these include naltrexone (ReVia), acamprosate, and tiapride. The remaining medication, disulfiram (Antabuse), may also possess some anticraving activity. Additional medications that have been investigated include ritanserin, which has not been shown to decrease craving or drinking levels in h

[Erythroderma and alcohol abuse].

We report on 7 patients with chronic alcohol abuse and ichthyosiform erythroderma. Dry skin (i.e. xerosis cutis) was a striking common feature in all patients who did not present any signs of fluid depletion. Two patients had exclusively exsiccational dermatitis; three had atopic disposition and one nummular eczema of the legs. In another patient a drug allergy could be demonstrated. Xerosis cutis seems to be an important precipitating aetiologic factor for erythroderma, and may be a consequence of alcohol-induced diuresis. All patients recovered rapidly under topical treatment with emollients

Developments in the treatment of alcoholism.

The treatment of alcoholism has changed during the past 2 decades. Notable developments have occurred in pharmacotherapy, psychotherapy, and health-care delivery. A better understanding of the biologic basis for addiction has led to clinical trials of medications that target neuroreceptors. One such medication is the opiate antagonist naltrexone, which decreases the craving for alcohol. Psychosocial interventions continue to be the mainstay of alcohol treatment programs. The efficacy of three different therapies was demonstrated in a study called Project MATCH (Matching Alcoholism Treatments t

Images and realities of alcohol.

The paper discusses the relationship between the images of alcohol and society, on one hand, and the reality of drinking and drinking problems on the other hand, from the point of view of policy-relevant research. Images of alcohol influence policy but they also depend on the social and cultural environment of policy-making. The epidemiological total consumption theory of alcohol-related problems is used as an example. The theory is embedded in the modern welfare state's ideals and its policy relevance presupposes that these ideals--universalism, consequentialism and public planning--are respe

Alcohol and fetal damage.

Since the identification of fetal alcohol syndrome in the end of the 1960s, substantial evidence has accumulated on a number of adverse effects that alcohol consumption during pregnancy may have on the fetus. Long-term effects on child development has also been observed. Although the various types of effects are well documented, less is known about threshold levels of consumption, under which there is no risk of damage. Few studies have assessed the risk associated with high episodic drinking. From a theoretical point of view, binge drinking in critical stages in organ formation may constitute

Alcohol and minor trauma.

A prospective study of breath alcohol levels among mildly injured adult patients was conducted over a 7-month period in the emergency department of a 15-bed hospital on a U.S. Air Force base in rural upstate New York. Severely injured patients were systematically excluded from the analysis. Three hundred twenty-seven patients were invited to participate. Two hundred sixty-seven submitted both a demographic questionnaire and a breath alcohol test (BAT). Forty-six submitted just a questionnaire. Fourteen declined to participate. Eight patients had positive BATs, representing 2.4% of the study po

[Intermittent alcoholic behavior].

Intermittent alcohol disorders are defined by the paroxystic aspect of alcohol consumption followed by periods of time without alcohol ingestion. The most typical form of intermittent alcoholism is dispomania. During the acute episode of dipsomania, the patient presents an extreme craving for alcohol and a complete loss of control. The alcohol consumption usually leads to severe overdoses and coma. The cases of pure dipsomania are relatively rare in daily practice. Most often, intermittent forms of alcohol abuse or dependence are observed in patients who present, between the acute ingestions o

Hypokalaemic myopathy in alcoholism.

We describe the clinical and pathological features of a patient with an acute painless proximal myopathy due to hypokalaemia associated with alcoholism. There was an excellent response to treatment with potassium supplements. The importance of recognition of low potassium states in alcohol-dependent patients with muscular weakness is emphasized.

Tianeptine and alcohol dependence.

Several arguments are in favour of the use of antidepressant drugs in alcohol-dependent patients, especially those acting on the serotoninergic system: (1) neurochemical data indicate the interaction between alcohol and 5-HT metabolism, (2) pharmacological studies show an improvement in the behaviour of alcoholized animals treated with antidepressants, (3) depression is a frequent disease in alcoholic patients. Tianeptine has been shown to be active in the treatment of depression in patients with history of alcohol abuse or dependence. In a first double-blind study performed versus amitryptili
