Crohn's disease of the appendix, manifested as acute appendicitis with postoperative fistula.

Two cases are reported of Crohn's disease localized to the appendix and manifested as acute appendicitis; after appendectomy a fistula developed. In none of 18 patients with Crohn's disease reported by other authors, where the appendix was the primary site, did a fistula develop postoperatively. When the appendix is the primary site of Crohn's disease the presence of mild inflammation of adjacent organs such as the terminal ileum may be mistakenly attributed to ordinary appendicitis. If, at exploratory laparotomy performed on a tentative diagnosis of appendicitis, Crohn's disease is suspected …

Partial appendiceal duplication with double bases converging into a single body and tip: a first reported case from Nepal.

INTRODUCTION: Anomalies of the appendix are very rare, occurring only in 0.004% to 0.009% of appendectomy cases. Among these, appendiceal duplication represents a particularly unusual finding. We are presenting a case of an appendicular lump with an appendix having two bases converging into one body and tip. CASE PRESENTATION: A 50 year-old female presented to a tertiary care center in Nepal with chief complaints of abdominal pain for 3 days and vomiting for 3 days. The abdominal pain was acute in onset, sharp, and aching, located in the right iliac region. Ultrasonography revealed that the a…

Patients may present with abdomina … On laparotomy, there was complete intussusception of the appendix with normal ileocaecal junction. Appendectomy was performed. Post-operative period was uneventful. DISCUSSION: Appendiceal intussusceptions are mostly diagnosed intra-operatively. The Understated Malignancy Potential of Nonoperative Acute Appendicitis.

Cancer of the appendix is rare and is most commonly found incidentally on pathology after an appendectomy for uncomplicated appendicitis (UA). The medical management alternative with antibiotics and observation remains an ongoing debate. The purpose of our study was to develop modern epidemiological data for adult patients completing an appendectomy for UA secondary to an appendiceal neoplasm (AN). ACS-NSQIP database was queried (2005-2016) to identify patients completing an appendectomy. Cohorts of patients who were diagnosed with UA and an AN were included in the study. Relevant perioperativ…

Predictors of recurrent appendicitis after non-operative management of children with perforated appendicitis presenting with an appendicular inflammatory mass.

AIM: To determine clinical and imaging predictors of recurrent appendicitis after non-operative management for children with perforated appendicitis presenting with an appendicular inflammatory mass. METHODS: A case-control study was carried out of children with clinical and ultrasonographic diagnosis of an appendicular inflammatory mass. Patients were divided into two groups according to the presence or not of recurrent appendicitis (or predisposing pathological findings during interval appendectomy). Several clinical and imaging factors were compared between both groups using an independent…

Acute appendicitis due to appendiceal endometriosis: Two case report and literature review.

INTRODUCTION: Appendiceal endometriosis (AE) is a rare condition, with a prevalence ranging from 0.05 % to 1.7 % in patients with endometriosis. It represents <1 % of cases of acute appendicitis (AA). CASES PRESENTATION: We present two cases of AA where the histological cause was endometriosis. Both cases involved patients around 40 years old who presented with abdominal pain in the right iliac fossa. AA was diagnosed through abdominal computed tomography (CT), which in the first case showed acute appendicitis, successfully treated with laparoscopic appendectomy. In the second case, the CT sh…

Paediatric appendiceal neuroendocrine tumours: a review of 33 cases from a single center.

BACKGROUND: Appendiceal neurendocrine tumours (NETs) are rare neoplasms and diagnosis is commonly incidental following appendectomy. We aimed to review our experience with appendiceal NETs. METHODS: Records of children with appendiceal NETs were reviewed and data concerning demographic characteristics, clinical findings, surgical procedures, histopathological findings, management and outcomes were recorded. RESULTS: Between 1985 and 2021, 33 cases with appendiceal NETs (median age 11.8 years, range 7.8-16; male/female = 10/23) were identified. All but one patients presented with abdominal pa…

Effect of computed tomography of the appendix on treatment of patients and use of hospital resources.

BACKGROUND: In patients with clinically suspected appendicitis, computed tomography (CT) is diagnostically accurate. However, the effect of routine CT of the appendix on the treatment of such patients and the use of hospital resources is unknown. METHODS: We performed appendiceal CT on 100 consecutive patients in the emergency department who, on the basis of history, physical examination, and laboratory results, were to be hospitalized for observation for suspected appendicitis or for urgent appendectomy. Outcomes were determined at surgery and by pathological examination in 59 patients, and …

Health care utilization after operative versus nonoperative appendicitis management using an administrative claims database.

BACKGROUND: Use of nonoperative management for uncomplicated appendicitis is increasing. Recurrent appendicitis is only one measure of successful nonoperative management. We examined health care utilization and exposure to medical imaging between patients postappendectomy and those with an in situ appendix over the year after initial diagnosis. We hypothesized that nonoperative management would result in greater health care utilization than operative management in the year following index diagnosis. METHODS: Using MarketScan, a commercial and fee-for-service Medicare claims database, we extra…

Idiopathic Granulomatous Appendicitis: A Case Report and Review of the Literature.

Idiopathic granulomatous appendicitis (IGA) is a rare condition that clinically presents similar to acute appendicitis, with symptoms such as abdominal pain, fever, and anorexia. We report a case of a 30-year-old postpartum female who presented with symptoms of acute appendicitis. Imaging revealed a thickened appendix, and laparoscopic appendectomy was performed. Intraoperative findings led to conversion to laparotomy with ileocolic resection. Histopathology confirmed granulomatous appendicitis with focal suppuration and no evidence of Crohn's disease or infection. IGA is diagnosed by excludin…

Retroperitoneal mucinous cystadenoma of the appendix mimicking hydatid cyst: A case report.

Appendiceal mucocele is a cystic dilatation of the appendix due to abnormal appendiceal mucinous secretion. Cystadenoma of the appendix is one of the most common causes and is encountered in 0.6% of all appendectomy specimens. The diagnosis may be difficult due to the asymptomatic nature of the disease; pain in the right lower quadrant may be the only symptom. Complex ovarian cyst, urolithiasis or cystic hydatid disease of the liver have been reported as mimicking appendiceal mucocele in the literature. In this study, we present a case of mucinous cystadenoma of the appendix mimicking retroper…

Significance of intraepithelial lymphocytes in appendix.

The aim of this study was to investigate the importance of the increase in intraepithelial lymphocytes (IELs) in the mucosa of the appendix. One hundred and four retrospective appendectomy specimens were examined to evaluate the IELs. Intraepithelial lymphocytosis was identified in 11.5% (12 cases) of the specimens. Of these 12 cases, 6 cases with intraepithelial lymphocytosis were associated with parasitic infection. No increase in IELs was found in the 36 appendices that were removed in other primary operations. A wide range of immunologic stimuli can raise IELs in the gastrointestinal syste…

Appendicitis secondary to metastatic melanoma: review of the National Institutes of Health experience.

IMPORTANCE: Malignant melanoma has an unusual propensity to metastasize to the small bowel; however, malignant melanoma with metastatic spread to the appendix presenting as acute appendicitis has rarely been reported. We describe cases of melanoma of the appendix presenting with appendicitis and review our institutional experience with this entity. OBSERVATIONS: Medical records were reviewed in patients with melanoma at the National Cancer Institute between January 1, 1953, and December 31, 2010, who underwent appendectomy. Of 5822 cases of melanoma treated at the National Institutes of Healt…
