Masticatory muscle pain: an important indicator of giant cell arteritis.

Giant cell arteritis (GCA) is a polysymptomatic disease which constitutes an ophthalmic emergency because early recognition and management can prevent blindness. There is conflicting information in the literature on the validity, sensitivity, and specificity of various systemic symptoms and signs of GCA. This paper presents a review of our prospective studies on the subject, and our findings are particularly relevant to dentists. We investigated 363 patients in a prospective study. Positive temporal artery biopsy was seen in 106 patients and negative in 257 referred for diagnosis of GCA. Syste…

Disease patterns and tissue cytokine profiles in giant cell arteritis.

OBJECTIVE: To determine whether clinical heterogeneity in patients with giant cell arteritis (GCA) is correlated with different patterns in the tissue-specific inflammatory response. METHODS: Twenty-three patients with typical histomorphologic findings of GCA were grouped according to the presence or absence of jaw claudication and/or visual abnormalities, fever, concomitant polymyalgia rheumatica (PMR), and histologic evidence of giant cell formation. The inflammatory response in temporal artery biopsy specimens was characterized by semiquantification of cytokine messenger RNA (mRNA) transcr…

Giant cell arteritis and polymyalgia rheumatica: clues to early diagnosis.

Giant cell arteritis (GCA) and polymyalgia rheumatica (PMR) are closely related disorders found predominantly in older patients. These disorders, which are being recognized more frequently, are more common in women, in Caucasians, and in various geographic locations. Early recognition and treatment may prevent possible catastrophic consequences of GCA, such as blindness, stroke, or dissection of the aorta. Although diagnosis is fairly easy with the classic presentation, it may be missed when the patient presents with nonspecific constitutional symptoms. An increased awareness among primary car…

[Diffuse panbronchiolitis with P-ANCA-positive arteritis and necrotizing glomerulitis].

A 53-year-old man was admitted to our hospital with a skin eruption, a high fever, and diplopia in April of 1992. He had been given a diagnosis of diffuse panbronchiolitis in 1981. After administration of erythromycin began in 1987, symptoms and chest roentgenographic findings gradually improved. Arteritis of peribronchial muscular arteries with medial destruction was seen in lung biopsy specimens, and periglomerular granulomatous inflammation and necrotizing glomerulitis were seen in renal biopsy specimens. P-ANCA was found, but a test for cytoplasmic ANCA (which is the most specific antigen …

[Giant-cell temporal arteritis. Anatomo-clinical study of 2 cases].

The Authors describe two cases of giant cell arteritis in male and elderly patients 72 and 82 years old. In both cases the early symptoms and signs of disease (headache, fever, weight loss and sedimentation rate raised) were associated to classical histology of temporal arteritis (granulomatous inflammation with giant cells). In case 1 there was impaired vision, while in case 2 coexisted polymialgia rheumatica (pain and stiffness in the muscles of the neck, back and proximal portions of the limbs).

Raised agalactosyl IgG and antimycobacterial humoral immunity in Takayasu's arteritis.

OBJECTIVE: Takayasu's arteritis is an inflammatory occlusive disease of the aorta and its main branches of unknown etiology. Some suggested causes include inapparent infection with Mycobacterium tuberculosis, or autoimmunity evoked by this organism. We have therefore sought links with mycobacterial disease. METHODS: We assayed the % agalactosyl IgG, antibody to a tuberculosis-specific 38 kDa protein, and antibody to the mycobacterial 65 kDa heat shock protein (HSP), in patients with active or inactive Takayasu's arteritis, in whom the diagnosis of tuberculosis was excluded. The results were c…

Cardiac lesions in non-specific aorto-arteritis. An autopsy study.

Autopsy data of 58 cases (1958-1986) was analysed for cardiac lesions. The cases were divided into 2 groups; paediatric (23) and adult (35). The heart was normal in 8.7% and 20.5% of the above groups respectively. Rest of the cases showed left ventricular hypertrophy with variable dilatation. Obstructive lesion in the aorta and or renal arteries was present in 91.3% of paediatric and 80.0% of adult cases. Congestive cardiac failure was very common in paediatric group (60.8%) and was not seen in absence of obstructive lesion in the aorta or renal arteries. Histologically the aortic lesion was h

Polymyalgia rheumatica and giant cell arteritis in emergency department patients.

Elderly patients with undiagnosed polymyalgia rheumatica and giant cell arteritis are not uncommon, and they may present to the emergency department with nonspecific physical complaints. We describe the cases of two patients who had the "typical" constellation of symptoms. It is important that emergency physicians consider the diagnosis and provide appropriate testing, therapy, and follow-up.

Polymyalgia rheumatica and temporal arteritis. Related conditions with differing therapies.

Polymyalgia rheumatica and temporal arteritis are entities seen chiefly in older adults. Polymyalgia rheumatica is characterized by muscle and joint aches and an elevated erythrocyte sedimentation rate, and it responds rapidly to low-dose corticosteroid therapy. Temporal arteritis is a vasculitic process, the diagnosis of which must be established by invasive procedures. Higher doses of steroids are necessary to treat it, and the potential for steroid-induced side effects is high.

Histology and morphometry of Strongylus vulgaris-mediated equine mesenteric arteritis.

Histological and morphometric evaluation of equine cranial mesenteric arteries was performed on 239 and 89 arteries, respectively. Histological examination revealed that thrombosis and the severity of inflammation varied on a seasonal basis and were directly associated with larval presence. Intimal and adventitial fibrosis were generally of greater severity than medial fibrosis. Fibrosis of the vasa vasorum was less frequent than fibrosis of the artery itself. Morphometry revealed a significant increase in intimal, adventitial and, to a lesser extent, medial area in affected as compared with n

Localized asymptomatic giant cell arteritis of the female genital tract.

Three postmenopausal patients with giant cell arteritis involving the female genital tract are presented. None of these patients had a prior history of vasculitis. The giant cell arteritis was confined to the myometrium in one patient, involved the cervix and myometrium in another, and involved all the internal genital organs in the third patient. Based on follow-up of our patients and analysis of similar cases in the literature, we conclude that asymptomatic patients may best be managed by close follow-up and monitoring of erythrocyte sedimentation rate levels, but that treatment may be unnec…

[Ulrich oculo-oscillodynamography in carotid artery stenosis and temporal arteritis].

Oculo-oscillodynamography (OODG) after Ulrich was performed on 21 patients suffering from unilateral hemodynamically relevant carotid stenosis. In ten patients the perfusion pressure was low on the affected side. In the other eleven patients there was no difference between the two sides and these patients were thought to have well-developed collateral anastomoses. In nine patients with temporal arteriitis low perfusion pressure was found by OODG on the side with the lower visual acuity. After systemic therapy with corticosteroids the perfusion pressure normalized on both sides.

[Temporal arteritis: a syndrome. From Horton's disease to periarteritis nodosa].

Four cases of histologically proven temporal arteritis presenting with atypical clinical, anatomical or evolutive features and raising nosological problems concerning the type of vasculitis involved are presented. In patients with temporal arteritis, some elements are suggestive of periarteritis nodosa. They include peripheral neurological lesions, renal or pleuro-pulmonary lesions and the histological appearance of the temporal artery. Diagnosing periarteritis nodosa in a case of temporal arteritis leads to a special therapeutic strategy.

Plasma levels of fibronectin in polymyalgia rheumatica giant cell arteritis.

In order to verify whether measurement of plasma fibronectin (Fn) could represent a useful tool in acute-phase-response assessment, Fn was measured in 16 previously untreated patients (group A) affected by polymyalgia rheumatica giant cell arteritis (PMR-GCA), both before, during, and after 45 days of steroid therapy, and its course was compared with the behavior of some acute-phase reactants such as erythrocyte sedimentation rate (ESR), fibrinogen (Fng), and prealbumin (Preal). No difference was detected between the baseline Fn levels found in patients and those registered in a control group …

[Rheumatic polymyalgia and temporal arteritis. Case contribution and bibliographic review].

Polymyalgia rheumatica (PMR) is a frequent clinical syndrome in the elderly, characterised by morning aching and stiffness in the shoulder and girdle muscles associated with an elevated erythrocyte sedimentation rate and responsive to low-dose corticosteroid therapy. PMR may be associated with temporal arteritis (AT), a systemic granulomatous vasculitis predominantly affecting branches of the carotid artery in patients over the age of 50. Recent studies suggest an immunological pathogenesis for PMR and TA. Personal experience on 56 cases of PMR and TA, and a survey of the literature is reporte…

Progressive systemic sclerosis, immunosuppression and necrotising arteritis: cause or effect?

Necrotising arteritis is rare in progressive systemic sclerosis (PSS). We report a patient with PSS who died with renal failure due to necrotising arteritis and segmental crescentic glomerulonephritis more typical of polyarteritis nodosa. Possible pathogenetic mechanisms are discussed. Renal biopsy is an important investigation in PSS, but is especially important in patients without hypertension, as other pathology needs to be excluded.
