Evaluation of Valsalva Maneuver in Alpha-Synucleinopathies.

Parkinson's disease (PD) and multiple system atrophy (MSA) are neurodegenerative diseases that not only cause motor symptoms but also impair the autonomic nervous system. This study aims to characterize the response to Valsalva maneuver (VM) in individuals affected by PD and MSA by analyzing the systolic arterial blood pressure (SBP) and heart rate (HR) signals. During the maneuver SBP and HR signals are highly nonstationary, therefore we decided to adopt a multi-level uniform phase empirical mode decomposition (UPEMD), a modification of empirical mode decomposition (EMD), together with standa…

Transdermal scopolamine: a review of its effects upon motion sickness, psychological performance, and physiological functioning.

Scopolamine is the most effective single drug for the prophylaxis and treatment of motion sickness. However, oral or injected scopolamine di … sickness protection, similar in extent to that provided by oral scopolamine or dimenhydrinate. Its autonomic nervous system effects comprise reduced salivation, bradycardia, and blurred vision due to reduced visual accommodation. The visual problems increase following repeated patch applications, with hypermetropic ("long sighted") individuals particularly at risk. Centr …

Neuroimmune crosstalk in the gut and liver.

It has long been assumed that the nervous system exerts distinct effects on immune functions, given the large number of immune disorders tha … iled to capture a comprehensive overview of neuroimmune interactions between distant organs and the autonomic nervous system in vivo, and the molecular mechanisms underlying local immune regulation of the nervous system have long remained unclear. However, the recent rapid progress in genetic recombination, microscopy and single-cell analysis has deepened our understandin …

Influence of cardiac nerve status on cardiovascular regulation and cardioprotection.

Neural elements of the intrinsic cardiac nervous system transduce sensory inputs from the heart, blood vessels and other organs to ensure adequate cardiac function on a beat-to-beat basis. This inter-organ crosstalk is critical for normal function of the heart and other organs; derangements within the nervous system hierarchy contribute to pathogenesis of organ dysfunction. The role of intact cardiac nerves in development of, as well as protection against, ischemic injury is of current interest since it may involve recruitment of intrinsic cardiac ganglia. For instance, ischemic conditioning,

Neuronal acetylcholine receptor autoimmunity.

Nicotinic acetylcholine receptors (AChRs) are ligand-gated cation channels that are present throughout the nervous system. The muscle AChR mediates transmission at the neuromuscular junction; antibodies against the muscle AChR are the cause of myasthenia gravis. There are numerous subtypes of AChRs present throughout the nervous system. These neuronal AChRs are closely related to the muscle AChR structurally, but are pharmacologically and immunologically distinct. The ganglionic (alpha3-type) neuronal AChR mediates fast synaptic transmission in sympathetic, parasympathetic, and enteric autonom

Cardiology meets neurology: clinical presentation and management of patients with primary neurogenic disorders and orthostatic intolerance.

Patients with primary neurogenic disorders such as pure autonomic failure or multiple system atrophy may initially present with cardiocirculatory symptoms such as orthostatic intolerance or fluctuations in heart rate with symptomatic tachycardia. It is therefore clinically important to identify such patients since circulatory manifestations are only one of a series of symptoms resulting from autonomic dysfunction in various organ systems. These patients require a multimodal diagnostic and therapeutic approach and should undergo extensive evaluation in a specialized autonomic nervous system (AN…

Pure autonomic failure.

In this chapter, we describe the history, presentation, diagnosis and treatment of pure autonomic failure (PAF). The pathology underlying this condition is thought to involve the deposition of alpha synuclein in the autonomic ganglia leading to diminished norepinephrine release and progressive autonomic dysfunction. We focus on various neurophysiological tests that may be used to evaluate the function of the peripheral autonomic nervous system including quantitative sudomotor axon reflex testing, thermoregulatory sweat testing, and others. These may help evaluate and diagnose various disorders…

The coordination of gastrointestinal hormones and the autonomic nerves.

We have conducted investigations on the relationship between gastrointestinal hormones and the autonomic nervous system. In this paper are described main themes of our investigations where a close association between gastrointestinal hormones and the autonomic nervous system was shown from the functional and morphological points of view. The "nervism" proposed by Pavlov, where the regulation of various organs in the body was thought to be accomplished wholly by the nervous system, was based on the findings on the physiology of the digestive glands. In the meantime, the discovery of secretin fr…

Baroreflex control model for cardiovascular system subjected to postural changes under normal and orthostatic conditions.

Baroreflex dysfunction is one of the common causes associated with the cardiovascular system. The buffering capability and baroreflex gain influences large variation in blood pressure for short term control. For regulating the blood pressure, an integrated analytical model for baroreflex control along with the cardiovascular system is presented to study the complex interactions between autonomic nervous system and cardiovascular system. In the proposed model, the autonomic nervous system utilizes sympathetic and parasympathetic nerve activities. This comprises a heart modeled by Mulier's appro…

A comprehensive analysis of the clinical characteristics and laboratory features in 179 patients with autoimmune autonomic ganglionopathy.

The clinical importance of autoantibodies against the ganglionic acetylcholine receptor (gAChR) remains to be fully elucidated. We aimed to … te that gAChRα3 and gAChRβ4 autoantibodies cause functional changes in postganglionic fibres in the autonomic nervous system and extra-autonomic manifestations in seropositive patients with AAG.

Autonomic dysfunction in pediatric patients with headache: migraine versus tension-type headache.

PURPOSE: To examine symptoms indicating central nervous system (CNS) autonomic dysfunction in pediatric patients with migraine and tension-type headache. METHODS: A retrospective chart review assessed six symptoms (i.e. constipation, insomnia, dizziness, blurry vision, abnormal blood pressure, and cold and clammy palms and soles) indicating central nervous system (CNS) autonomic dysfunction in 231 patients, ages 5-18 years, diagnosed with migraine, tension-type headache (TTH), or Idiopathic Scoliosis (IS). RESULTS: Higher frequencies of "insomnia," "dizziness," and "cold and clammy palms and

Cardiovascular responses to cold pressor and LBNP tests before and after 20-day-bedrest.

The orthostatic hypotension after weightlessness has been suggested mainly because of the blood volume reduction, and decrease in stroke volume and cardiac output as its consequence. However, since bedrest is characterized as removal of orthostasis and exercise, which are major physiological stressors to the autonomic nervous system in normal daily life of humans, thus it may be possible that general autonomic activity itself would be deconditioned in the central nervous system after bedrest. Thus, we hypothesized that 20-day bedrest may modify the cardiovascular responses to cold pressor test…

[Sleep and autonomic function: sleep related breathing disorders in Parkinson's disease and related disorders].

In patients with multiple system atrophy (MSA), sleep related breathing disorders (SRBD), including obstructive and central sleep apnea, vocal cord abductor paralysis and dysrhythmic breathing pattern, are frequently observed. SRBD may have a considerable impact on variation of autonomic nervous activity during sleep. The previous studies correlated upper airway muscle dysfunction related parkinsonism with increased prevalence of SRBD in patients with Parkinson's disease (PD). However, recently, the clinical significance of SRBD and its impact on sleepiness and disease severity have been debat

Acetylcholine in Brain-Body Communication: Biological Mechanisms and Physiological Roles.

Acetylcholine (ACh) is an evolutionarily conserved neurotransmitter that is widely distributed in the central and peripheral nervous systems and plays essential roles in multiple physiological processes. This review summarizes the full biological cycle of ACh, including its synthesis, vesicular storage, release, degradation, and reuptake, and discusses the regulatory mechanisms underlying its functions in the nervous system and peripheral organs. Through nicotinic acetylcholine receptors (nAChRs) and muscarinic acetylcholine receptors (mAChRs), ACh is involved in central nervous system functio

Gastric emptying in patients with vitamin B(12) deficiency.

The clinical presentation of patients with vitamin B(12) deficiency varies in a spectrum ranging from haematological disorders to neuropsychiatric diseases. In rare cases, orthostatic hypotension, impotence, constipation and urinary retention have been attributed to autonomic nervous system dysfunction due to vitamin B(12) deficiency. The aim of this study was to evaluate the effect of vitamin B(12) deficiency on autonomic nervous system function by studying gastric emptying times ( T(1/2)). Twenty patients with newly diagnosed vitamin B(12) deficiency and 12 control patients with gastritis an…

Syncope due to autonomic insufficiency syndromes associated with orthostatic intolerance.

Recurrent syncope may be either a sign or a symptom and may occur due to a wide variety of very different causes. Extensive investigations into the nature of this disorder soon uncovered that it represents only one aspect of a broad, heterogenous group of disturbances of the autonomic nervous system (ANS) that can result in hypotension, orthostatic intolerance, and often syncope. Disorders of orthostatic regulation may be subgrouped into both primary and secondary forms. In primary autonomic failure syndromes, as opposed to the intermittent periods of hypotension seen in the reflex syncopes, p…
