Association between single nucleotide polymorphisms in TLR4, TLR2, TLR9, VDR, NOS2 and CCL5 genes with acute viral bronchiolitis.

BACKGROUND: Acute viral bronchiolitis is the leading cause of hospitalization among infants during the first year of life. Most infants hospitalized for bronchiolitis do not present risk factors and are otherwise healthy. Our objective was to determine the genetic features associated with the risk and a severe course of bronchiolitis. METHODS: We prospectively evaluated 181 infants with severe bronchiolitis admitted at three hospitals over a 2-year period, who required oxygen therapy. The control group consisted of 536 healthy adults. Patients were evaluated for the presence of comorbidities …

Co-immunization with virus-like particle and DNA vaccines induces protection against respiratory syncytial virus infection and bronchiolitis.

This study demonstrates that immunization with non-replicating virus-like particle (FFG VLP) containing RSV F and G glycoproteins together with RSV F DNA induced T helper type 1 antibody responses to RSV F similar to live RSV infection. Upon RSV challenge 21weeks after immunization, FFG VLP vaccination induced protection against RSV infection as shown by clearance of lung viral loads, and the absence of eosinophil infiltrates, and did not cause lung pathology. In contrast, formalin-inactivated RSV (FI-RSV) vaccination showed significant pulmonary eosinophilia, severe mucus production, and exte

Th1 and th2 cytokine expression in nasopharyngeal secretions during acute bronchiolitis in children younger than two years old.

OBJECTIVES: We characterised the T helper cytokine profiles on the surface of nasal mucosa of children with acute bronchiolitis caused by Respiratory Syncytial Virus, Parainfluenza Virus, Influenza Virus, Adenovirus, or without any viral identification, in order to examine whether these viral types modified cytokine responses. As an additional objective we sought to determine if T helper polarisation was associated with other demographic and environmental factors. METHODS: A prospective study of children with acute bronchiolitis was performed. Patients were recruited from the emergency depart…

Neonatal pertussis presenting as acute bronchiolitis: direct detection of the Bordetella pertussis genome using loop-mediated isothermal amplification.

We report a 28-day-old female infant with pertussis presenting as severe acute bronchiolitis with cyanosis. On admission, the patient's symptoms were similar to those of acute bronchiolitis. However, occasional apneic episodes with cyanosis and peripheral lymphocytosis suggested neonatal pertussis and prompted us to examine the presence of Bordetella pertussis using loop-mediated isothermal amplification (LAMP) based on the insertion sequence IS481. LAMP of the nasopharyngeal and intratracheal aspirates was positive for B. pertussis and a diagnosis of neonatal pertussis was made. As the clinic…

Point-of-care echocardiographic assessment of right ventricle systolic function in infants with severe bronchiolitis: a prospective observational study.

The aim of this study was to assess right ventricle (RV) systolic function in infants with severe bronchiolitis using tricuspid annular plane systolic excursion (TAPSE) and to explore its prognostic value. We hypothesized that lower TAPSE at pediatric intensive care unit (PICU) admission could be associated with prolonged length of stay (LOS) and duration of respiratory support. We conducted a prospective single-center observational pilot study in a tertiary general PICU during the 2022-2023 bronchiolitis season. All children < 12 months admitted to PICU for bronchiolitis were eligible. TAPSE …

Asthma and Lung Function in School-Aged Children After Severe RSV-A and RSV-B Bronchiolitis, Is There Any Difference?

INTRODUCTION: Human respiratory syncytial virus (HRSV) is a major cause of respiratory disease. It is classified into two antigenic subgroups. While several authors have studied the potential link between HRSV infection and later development of recurrent wheezing (RW) or asthma, no previous studies have assessed the role of HRSV subtypes. The aim of this study was to assess the medium-term respiratory morbidity in children and adolescents with prior severe bronchiolitis due to HRSV-A or HRSV-B infection. METHODS: From September 2023 to June 2025 an observational, prospective, cross-sectional …

Genetic susceptibility, terminal bronchiole pathology, and inflammation: An integrated mechanisms of the pathogenic process in chronic obstructive pulmonary disease.

Chronic obstructive pulmonary disease (COPD) remains a leading cause of mortality yet understanding of disease initiation and progression is incomplete. Current therapeutic agents provide symptomatic relief but fail to halt disease progression, highlighting the urgent need to elucidate underlying pathogenic mechanisms, particularly in the preclinical and early stages before irreversible lung damage becomes established. COPD exhibits profound heterogeneity in clinical presentation and disease trajectories, even among patients with similar spirometric measures, indicating that divergent pathogen

Integrated nasopharyngeal airway metagenome and asthma genetic risk endotyping of severe bronchiolitis in infancy and risk of childhood asthma.

BACKGROUND: Infants with bronchiolitis are at increased risk of developing asthma. Growing evidence suggests bronchiolitis is a heterogeneous condition. However, little is known about its biologically distinct subgroups based on the integrated metagenome and asthma genetic risk signature and their longitudinal relationships with asthma development. METHODS: In a multicentre prospective cohort study of infants with severe bronchiolitis (i.e. bronchiolitis requiring hospitalisation), we profiled nasopharyngeal airway metagenome and virus at hospitalisation, and calculated the polygenic risk sco…

The association of genetic variants in toll-like receptor 2 subfamily with allergy and asthma after hospitalization for bronchiolitis in infancy.

BACKGROUND: Toll-like receptors (TLRs) are a pivotal part of the innate immunity system. Variations in TLR genes have been connected to autoimmune conditions, such as allergy and asthma. The TLR2 subfamily comprises TLR1, TLR2, TLR6 and TLR 10. We hypothesized that polymorphism of the TLR2 subfamily may be associated with prevalence of post-bronchiolitic asthma and/or atopy. METHODS: TLR1rs5743618, TLR2rs5743708 and TLR6rs5743810 single nucleotide polymorphisms of 133 children who had been hospitalized for bronchiolitis at <6 months of age were analyzed.
