Small vessel occlusion in acute acalculous cholecystitis.

Ischemia is one mechanism implicated in the pathogenesis of acute acalculous cholecystitis. Gallbladder specimen arteriography was performed to define the comparative status of cystic artery runoff in the macroscopically normal gallbladder (n = 10), in acute gallstone-associated cholecystitis (n = 10), and in acute acalculous cholecystitis (n = 7). Standardized film exposure and quantity of contrast medium administered permitted objective intergroup comparison. Compared to the macroscopically normal gallbladder, specimens of acute gallstone-associated cholecystitis exhibited arterial dilatatio…

Indium-111-leukocyte imaging in acute cholecystitis.

Eleven patients with suspected acute cholecystitis underwent sequential 99mTc-iminodiacetic derivative (IDA) and 111In-white blood cell (WBC) imaging to determine if 111In-WBCs accumulate within an acutely inflamed hemorrhagic gallbladder wall and, thus, could be employed as a reasonable alternative to 99mTc-IDA scintigraphy in detecting acute cholecystitis. Seven patients had surgically confirmed acute cholecystitis. Of these cases, five had a true-positive 99mTc-IDA and 111In-WBC, one an indeterminate 111In-WBC and true-positive 99mTc-IDA, and one a true-positive 111In-WBC and false-negative…

[Acute alithiasic cholecystitis (pathogenetic and clinical considerations)].

The authors address the topic of acute acalculous cholecystitis and examine the related aetiopathogenetic and clinical problems, taking as their starting point two cases of the disease observed in their department.

Emergency versus elective cholecystectomy in acute cholecystitis.

The present study was carried out at the Department of Surgery, All India Institute of Medical Sciences Hospital, New Delhi, between January 1982 and October 1984. Clinical diagnosis of acute cholecystitis was confirmed by ultrasound scanning or Tc99m labelled HIDA Scan. Group I (n=24) comprised patients who underwent emergency cholecystectomy while Group II (n=23) comprised patients who were managed conservatively, and had an elective cholecystectomy performed 6-12 weeks later. There was no mortality or wound infection in either group. The incidence of other complications was 8.3 per cent and…

Early versus delayed cholecystectomy for acute cholecystitis.

The controversy between early or delayed cholecystectomy continues, although recent opinion appears to favour the early approach. If this policy of early operation is to be adopted then an accurate diagnosis must be obtained at the earliest opportunity. An accurate diagnosis of acute cholecystitis is made from a careful history, the relevent clinical findings and if possible a HIDA scan demonstrating non-filling of the gallbladder. However, if radionucleotide scanning is not available then real-time ultrasonography is the next best choice, but must be interpreted with care as it is not as spec…

Acute acalculous cholecystitis complicating aortic aneurysm repair.

In a series of 374 consecutive abdominal aortic aneurysmal repairs, the incidence of acute acalculous cholecystitis was 1.1 per cent. This complication occurred in only one of 352 patients (0.3 per cent) after elective aneurysmorraphy, as compared with three of 22 (13.6 per cent) after emergency repair of a ruptured aneurysm. This difference proved highly significant (p = 0.0001). All of the patients who had postoperative acute cholecystitis after aortic aneurysmal repair had acalculous disease. A mortality rate of 50 per cent was noted for this complication. Technetium cholescintigraphy prove…

Rocky Mountain spotted fever mimicking acute cholecystitis.

Rocky Mountain spotted fever can present with predominantly abdominal symptoms including nausea, vomiting, diarrhea, and abdominal pain. Two elderly patients presented with an acute febrile illness and abdominal symptoms. Rash was not present initially. Workup disclosed cholelithiasis in one, and a thickened gallbladder wall surrounded by a sonolucent zone suggesting a pericholecystic abscess was found by ultrasonography in the other. Both patients underwent emergency laparotomy, with cholecystectomy in both and appendectomy in one. Both patients died several days postoperatively. Pathologic s

Computed tomography in acute cholecystitis: new observations.

The computed tomographic CT findings in five patients with acute cholecystitis were analyzed. Common findings included thickening and nodularity of the gallbladder wall, cholelithiasis, and dilatation of the gallbladder lumen. Other more specific findings included poor definition of the gallbladder wall, pericholecystic fluid collections, and gas collections within the gallbladder wall. Most of the CT findings are suggestive but not pathognomonic, and correlation with the clinical, scintigraphic, and sonographic findings is necessary.

Gallbladder epithelial acid hydrolases in human cholecystitis.

The lysosomal enzymes beta-glucuronidase and acid phosphatase were studied in 112 patients with cholecystitis. Acid phosphatase activity was generally lower in patients with cholesterol stones compared with cases with pigment stones. beta-glucuronidase activity was higher in acalculous cholecystitis than in any other group, a fact compatible with the concept that in lithiasis the enzyme is secreted into the bile and therefore may participate in nidus formation. Histochemistry at light microscopical level clearly demonstrates the lysosomal distribution of these enzymes and their presence in the…
