Expert-Based, Institutional Approaches for Reducing the Diagnostic Odyssey of Patients With Inborn Errors of Immunity.

Inborn errors of immunity (IEIs) are a heterogeneous group of genetic disorders with long diagnostic delay that negatively impacts patient outcomes. To combat delay, 8 multidisciplinary experts from different disciplines convened to generate 11 consensus-based statements that address approaches that could reduce the diagnostic journey of patients with IEIs. Nine of these statements relate to the development and implementation of automated decision support tools to improve rapidity of diagnosis by augmenting clinical decision-making for practitioners with or without IEI experience. Strengths an…

[Impact of anchoring bias in medical diagnostic decision-making: an experimental study].

OBJECTIVE: Cognitive biases, such as anchoring bias, significantly influence clinical decisions and contribute to up to 75% of diagnostic errors. Anchoring bias affects both general practitioners and specialists, although its impact may vary depending on the clinician's experience and other factors. Biases are responsible for a substantial proportion of medical errors, highlighting the need to address them through interventions in training and clinical practice. The aim of this paper was to evaluate the impact of anchoring bias on diagnostic decision-making among general practitioners and spec

Antireflux surgery - choosing the right candidate.

INTRODUCTION: Surgical gastric fundoplication is an effective treatment option for gastroesophageal reflux disease. In contrast to acid suppression, fundoplication nearly abolishes all types of reflux, acid and nonacid. However, in some cases, lasting side effects of the procedure may overshadow its positive effects. It has remained difficult to determine which patients are the most suitable candidates for fundoplication. AREAS COVERED: This review aims to evaluate the available data on preoperative factors that are associated with the outcome of fundoplication and to determine which combinat

Surgical Decision Making in Incisional Blepharoplasty in Asians.

BACKGROUND: Owing to the prevalence of congenital single eyelids among Asians, methods are needed to create a double-eyelid fold. Incisional blepharoplasty is considered an ideal method for this purpose. The classical approach tends to create a stable but less dynamic fold. Subsequent refinements aim to achieve a balance between stability and dynamism. However, there are many procedural variations due to anatomical differences, which may account for the varying surgical outcomes. This systematic review explored the current techniques used in Asian blepharoplasty and focuses on the efforts to b

Innovative treatments of pediatric spinal muscular atrophy: The decision-making process in France.

Spinal muscular atrophy (SMA) is a devastating early-onset genetic disease characterized by motor neuron degeneration. For several years, an early access program has facilitated the use of three innovative therapies in France. To better define the therapeutic strategy following innovative therapy approval, an online expert committee within the French Rare Health Care for Neuromuscular Diseases Network (FILNEMUS) evaluates early diagnosed children and treatment-naive SMA cases during pediatric SMA multidisciplinary team meetings (psMTMs). The decision process leading to molecule choice or palli

To mesh or not mesh "apical prolapse," that is the question!

AIMS: The management of pelvic organ prolapse (POP) involves both nonsurgical and surgical management. Views on best surgical approach for apical prolapse is an evolving field based on recent evidence and events related to transvaginal mesh. METHODS: At the 97th Annual Meeting of the North Central Section of the American Urological Association (Scottsdale, AZ, USA), this topic was discussed and debated. The following article summarizes the concepts presented that day as well as the interactive debate with three perspectives on apical prolapse surgeries. The authors were asked to support their

Machine Learning: A Potential Therapeutic Tool to Facilitate Neonatal Therapeutic Decision Making.

Bacterial infection is one of the major causes of neonatal morbidity and mortality worldwide. Finding rapid and reliable methods for early recognition and diagnosis of bacterial infections and early individualization of antibacterial drug administration are essential to eradicate these infections and prevent serious complications. However, this is often difficult to perform due to non-specific clinical presentations, low accuracy of current diagnostic methods, and limited knowledge of neonatal pharmacokinetics. Although neonatal medicine has been relatively late to embrace the benefits of mach

Navigating the Impact of the Dobbs Decision: Perspectives from Pediatric Surgeons on Reproductive Healthcare.

Nationwide abortion restrictions resulting from the Dobbs v Jackson Women's Health Organization (2022) decision have generated confusion and uncertainty among healthcare professionals, with concerns for liability impacting clinical decision-making and outcomes. The impact on pediatric surgery can be seen in prenatal counseling for fetal anomaly cases, counseling for fetal intervention, and recommendations for pregnant children and adolescents who seek termination. It is essential that all physicians and healthcare team members understand the legal implications on their clinical practices, enga…

HBV Biomarkers and Their Role in Guiding Treatment Decisions.

Over 300 million individuals worldwide are chronically infected with hepatitis B virus and at risk for progressive liver disease. Due to the lack of a therapy that reliably achieves viral elimination and the variability of liver disease progression, treatment decisions are guided by the degree of liver disease and viral biomarkers as the viral life-cycle is well characterized and largely conserved between individuals. In contrast, the immunological landscape is much more heterogeneous and diverse and the measurement of its components is less well standardized. Due to the lack of a universal an

Exploratory application of machine learning methods on patient reported data in the development of supervised models for predicting outcomes.

BACKGROUND: Patient-reported outcome measurements (PROMs) are commonly used in clinical practice to support clinical decision making. However, few studies have investigated machine learning methods for predicting PROMs outcomes and thereby support clinical decision making. OBJECTIVE: This study investigates to what extent different machine learning methods, applied to two different PROMs datasets, can predict outcomes among patients with non-specific neck and/or low back pain. METHODS: Using two datasets consisting of PROMs from (1) care-seeking low back pain patients in primary care who par

Association of Frailty With Treatment Selection and Long-Term Outcomes Among Patients With Chronic Limb-Threatening Ischemia.

Background The optimal treatment strategy for patients with chronic limb-threatening ischemia (CLTI) is often unclear. Frailty has emerged as an important factor that can identify patients at greater risk of poor outcomes and guide treatment selection, but few studies have explored its utility among the CLTI population. We examine the association of a health record-based frailty measure with treatment choice and long-term outcomes among patients hospitalized with CLTI. Methods and Results We included patients aged >65 years hospitalized with CLTI in the Medicare Provider Analysis and Review da

STEP improves long-term survival for pediatric short bowel syndrome patients: A Markov decision analysis.

INTRODUCTION: Increasingly, for pediatric patients with short bowel syndrome (SBS), intestinal lengthening procedures such as serial transverse enteroplasty (STEP) are being offered with the hope of improving patients' chances for achieving enteral autonomy. However, it remains unclear to what extent STEP reduces the long-term need for intestinal transplant or improves survival. METHODS: Based on existing literature, a decision analytic Markov state transition model was created to simulate the life of 1,000 pediatric SBS patients. Two simulations were modeled: 1) No STEP: patients were listed

Practical Approach to Children Presenting with Eosinophila and Hypereosinophilia.

Eosinophilia is not a rare finding in clinical practice, and often poses problems in terms of etiologic research and differential diagnosis. Peripheral eosinophilia is defined by a blood eosinophil count > 500 cells/μL. It is classified into mild (500-1500 cells/μl), moderate (1500-5000 cells/μl) and severe for an eosinophil count > 5000 cells /μl. The term "hypereosinophilia" defines a condition characterized by a blood eosinophil count >1500 cells/μl in at least two consecutive tests made with a minimum of a 4-week interval. The causes of eosinophilia are various, and can be summarized by th
