Endoscopic removal of a toothpick perforating the sigmoid colon and causing chronic abdominal pain: a case report.

Toothpick ingestion is implicated in gut injuries which may cause severe complications, mimicking diseases causing acute abdomen. However, toothpick ingestion-related perforation may also cause mild, non-specific gastrointestinal symptoms without significant findings or major complications. We describe a young male with chronic postprandial lower abdominal pain caused by a toothpick impaction at the rectosigmoid junction after inadvertent ingestion. The foreign body was detected and successfully removed during flexible sigmoidoscopy. Perforation due to foreign body ingestion must be considered

Learning curves for laparoscopic sigmoidectomy used to manage curable sigmoid colon cancer: single-institute, three-surgeon experience.

BACKGROUND: Laparoscopic surgery demands mastery of a steep learning curve. Defining a learning curve in laparoscopic surgery is useful for planning training programs or clinical trials. This study aimed to define the learning curves for laparoscopic sigmoidectomy used to manage curable sigmoid colon cancer by evaluating early surgical outcome data from three colorectal surgeons. METHODS: This study analyzed data from 138 consecutive patients undergoing laparoscopic sigmoidectomy for curable sigmoid colon cancer performed by three colorectal surgeons between May 2001 and November 2006. The le

Survival benefit of high ligation of the inferior mesenteric artery in sigmoid colon or rectal cancer surgery.

BACKGROUND: The aim of this study was to assess the impact of inferior mesenteric artery (IMA) root nodal dissection before high ligation of the artery on survival in patients with sigmoid colon or rectal cancer. METHODS: Data on 1188 consecutive patients who underwent resection for sigmoid colon or rectal cancer, with high ligation of the IMA, were identified from a prospective database (April 1965 to December 1999). Survival of patients with involvement of nodes along the IMA proximal to the origin of the left colic artery (root nodes, station 253) through the bifurcation of the superior re

Distribution of lymph node metastasis in T1 sigmoid colon carcinoma: should we ligate the inferior mesenteric artery?

OBJECTIVE: In standard oncological sigmoid colectomy, the inferior mesenteric artery is ligated either at its origin or at the level of the left colic artery. However, in patients with early-stage carcinoma, the distribution of metastatic nodes may be limited. The aim of this study was to clarify the prevalence and distribution of lymph node metastasis in T1 sigmoid colon carcinoma and to determine the adequate range of lymph node dissection. MATERIALS AND METHODS: The study included 121 consecutive patients treated for T1 sigmoid colon carcinoma. Clinicopathologic factors associated with nod

[Sigmoid colon perforation with local peritonitis caused by indirect trauma--case report and review of the literature].

Injuries to the sigmoid occur either as acute or protracted events. In the first case, enteral contents discharge into the abdominal cavity and a generalized, fecal, life-threatening peritonitis with a bad prognosis develops. In the protracted form, the rupture is covered by peritoneum and adherent organs before perforation. The ensuing abscess formation may lead to perforation into contiguous visceral organs or the cutis. Frequently an intestinal or cutaneous fistula results. The trigger for a sigmoid perforation can be a spontaneous rupture in an already vulnerable intestine. Common precurso

Mucinous adenocarcinoma arising in a neovagina using the sigmoid colon thirty years after operation: a case report.

BACKGROUND: Because of a relative rarity of the cases with an artificial vagina, the incidence of a case with malignant disease arising in the neovagina is extremely rare. A case of adenocarcinoma arising from a neovagina is presented with a review of the literature. CASE: A neovagina was constructed using the sigmoid colon at the age of 23 for congenital agenesis of the vagina, Rokitansky-Küster-Hauser syndrome. Subsequently, the patient had regular sexual intercourse for about 20 years. At the age of 53, she came to our outpatient clinic with a complaint of vaginal bleeding, and adenocarcin

Circular muscle contraction, messenger signalling and localization of binding sites for neurokinin A in human sigmoid colon.

1. Neurokinin (NK)A is the endogenous ligand for the tachykinin NK2 receptor. In the present study, tachykinins and selective receptor agonists were tested as contractile agonists in human colon circular muscle and [125I]-NKA was used to localize binding sites in human colon. 2. In strips of circular muscle, removal of mucosa and submucosa significantly (P < 0.05) increased the potency and the maximum response achieved by NKA. 3. The rank order of potency of tachykinin and selective receptor agonists in contracting circular muscle strips was NKA > or = [Lys5,MeLeu9,Nle10]NKA(4-10) > or = neuro

Continent urinary reservoir using sigmoid colon and appendix after pelvic exenteration for bulky leiomyosarcoma: a case report.

We report on a man with bulky leiomyosarcoma occupying the entire pelvic cavity that was treated by pelvic exenteration. A urinary reservoir was constructed using a detubularized segment of sigmoid colon. The appendix was tunneled into the taenia to form a continent catheterizable stoma using the Mitrofanoff principle. Descending colostomy was also created. There is no evidence of recurrence 24 months after treatment and the patient has returned to full physical activity. Continence is complete with a capacity of up to 500 ml. and regular bowel movements. Because of the lack of intestinal anas

[Tissue blood flow of the sigmoid colon for safe anastomosis following ligation of the inferior mesenteric artery].

We examined the tissue blood flow (TBF) of the sigmoid colon to determine which level of inferior mesenteric artery. In 13 patients, TBF of the sigmoid colon was measured in the control state and the clamped state, in which the inferior mesenteric artery (IMA), left colic artery and the rectal were wall clamped. In the clamped state, means of TBF at 10 cm and 15 cm distal to the descend-sigmoid junction were 1.87 +/- 0.77 and 1.10 +/- 0.40, respectively. Mean of TBF at 15 cm was significantly lower than that at 10 cm. In conclusion, TBF which is enough for safe anastomosis reaches as far as 10

Epithelial cell proliferation in the sigmoid colon of patients with adenomatous polyps increases during oral calcium supplementation.

To study the effect of oral supplemental calcium on colonic epithelial proliferation, 17 adenomatous polyp patients received 1.5 g Ca2+ as calcium carbonate daily during 12 weeks, while on a calcium constant diet, based on the patients' habitual diet. Seven subsequently continued calcium supplementation for 9 months without dietary restrictions. Epithelial proliferation rate in colonic biopsies, expressed as labelling index (%), was determined with 5-bromodeoxyuridine and immunohistochemistry. Biopsies were taken from the midsigmoid at time of polyp excision and at the end of the intervention

Stereo X ray photogrammetry applied for prevention of sigmoid-colon damage caused by radiation from intrauterine sources.

Radiation therapy of cervix carcinoma is applied in this Institute by means of a modified Stockholm method in combination with external beam irradiation. In 1968, parametrial portals were replaced by large planparallel opposed fields extending cranially to LIII/LIV with central shielding in order to avoid overdosage in the area of intracavitary treatment. This resulted in a marked increased incidence of severe sigmoid-colon radiation lesions from 0.25% to 4%; predominantly in Stage I and II patients. Therefore two measures have been introduced: beginning in 1972 measures were taken to prevent

Long-term prognosis for patients with ulcerative proctosigmoiditis (ulcerative colitis confirmed to the rectum and sigmoid colon).

Despite its frequency, ulcerative proctosigmoiditis is poorly recognized and its prognosis is not well understood. This study reviews experience at the Cleveland Clinic from 1960 through 1972, with a 97% follow-up. A total of 359 patients with a diagnosis of ulcerative proctosigmoiditis were followed for a mean of 11 years (range 3.2--17.7 years). There were 219 men and 140 women. Mean age at diagnosis was 40.6 years. All had rectal bleeding, typical proctosigmoidoscopic findings of diffuse granularity, mucosal ulcerations and friability, and normal findings above the rectosigmoid on barium en

Case Report: Delayed complete transection of the sigmoid colon after severe pelvic fracture leading to septic shock.

Severe pelvic fractures caused by high-energy trauma may be complicated by bowel injuries. These injuries may be evident at initial presentation or remain occult, resulting in delayed diagnosis. Delayed complete transection of the sigmoid colon associated with pelvic fractures is extremely rare. We describe a 57-year-old woman who was admitted to the Intensive Care Unit (ICU) following a traffic accident that resulted in a Young-Burgess lateral compression type III pelvic ring injury, hemorrhagic shock, impaired consciousness, and multiple associated injuries. Abdominal CT on post-injury days

Case Report: A case of Cronkhite-Canada syndrome associated with sigmoid colon cancer and early esophageal cancer.

This article reports a case of a patient with Cronkhite-Canada syndrome combined with sigmoid colon cancer and early esophageal cancer. In this case, CCS was diagnosed simultaneously with the discovery of sigmoid colon cancer and early esophageal cancer. After radical resection of sigmoid colon cancer, the patient received hormone therapy for 12 months. The symptoms such as diarrhea, hair loss, and malnutrition were relieved, and the gastric-duodenal and colonic polyp-like lesions were reduced. The early esophageal cancer was followed up for 3 years, but no active treatment was given for the e

Laparoscopic management of aortofemoral graft erosion into the sigmoid colon following secondary aortoenteric fistula: a case report.

Secondary aortoenteric fistula is a rare and life-threatening complication following aortic reconstructive surgery, most commonly involving the duodenum. Erosion of an aortofemoral bypass graft into the sigmoid colon is exceptionally uncommon; all reported cases were managed via open surgery with significant morbidity. Laparoscopic management with graft preservation has not been documented. A 59-year-old man presented with massive hematochezia 9 months after bilateral aortobifemoral bypass for an abdominal aortic and common iliac aneurysm. Colonoscopy revealed a vascular graft 25 cm from the a
