Neurobiological mechanisms of control in alcohol use disorder - Moving towards mechanism-based non-invasive brain stimulation treatments.

Alcohol use disorder (AUD) is characterized by excessive habitual drinking and loss of control over alcohol intake despite negative consequences. Both of these aspects foster uncontrolled drinking and high relapse rates in AUD patients. Yet, common interventions mostly focus on the phenomenological level, and prioritize the reduction of craving and withdrawal symptoms. Our review provides a mechanistic understanding of AUD and suggests alternative therapeutic approaches targeting the mechanisms underlying dysfunctional alcohol-related behaviours. Specifically, we explain how repeated drinking …

Regular cannabis use is associated with blunted affective, but not cardiovascular, stress responses.

AIMS: To examine the potential impact of regular cannabis use on cardiovascular and subjective responses to acute stress. DESIGN: We used a quasi-experimental design in which subjective states and cardiovascular measures were obtained during rest and in response to acute stress challenges in a sample of regular cannabis users and non-users. PARTICIPANTS: Seventy-nine adults (forty-five cannabis users and thirty-four non-users). MEASUREMENTS: We measured subjective states (positive affect, state stress, state anxiety, cannabis craving) and cardiovascular indicators (blood pressure, heart rat…

[The ambivalence at the heart of withdrawal : insights from addiction medicine].

This article describes the case of a patient hospitalized for cocaine withdrawal, whose journey highlights the tension between the desire for abstinence and the compelling need to use, raising questions about the management of craving, the role of comorbidities, and the balance between therapeutic alliance and institutional framework.

Unusual presentation of desiderosmia in major depressive disorder: A case series of four patients from Eastern India.

Abnormal and excessive desire to smell certain odors or objects (desiderosmia) is reported to be seen with iron deficiency anemia. We present here four cases of desiderosmia in diagnosed patients of major depressive disorder without iron deficiency, where treatment of depression was associated with improvement in abnormal craving.

Large-scale cohort studies showed a reduction in AUD-related hospitalizations for patients on semaglutide compared to non-users. Dual agonists like tirzepatide also showed sign …

Temporal relationships between mobile phone addiction and mental health: A longitudinal network analysis study from non-clinical adolescents.

OBJECTIVES: Previous psychometric network models have been limited to cross-sectional designs, while current longitudinal models also suffer from methodological shortcomings. This study challenges and refines previous longitudinal psychometric network models, addressing the temporal and static correlational relationships between mobile phone addiction and mental health symptoms, as well as gender differences. METHODS: We conducted a one-year follow-up study involving 1,479 adolescents (M age=15.28±1.51, 786 males), and measured mobile phone addiction (inability to control cravings, feeling an…

The role of reward and reinforcement in understanding alcohol use among adults living with HIV in South Africa.

OBJECTIVE: Most research investigating reinforcement in alcohol use is from high-income countries. The current study sought to understand the association of different reinforcement types (alcohol-specific reward, environmental reward, behavioral activation) with multiple measures of alcohol use and cravings among individuals living with HIV in South Africa. METHOD: Baseline data were obtained from a substance use clinical trial in Cape Town (N = 65). Unhealthy alcohol use was measured using the biomarker phosphatidylethanol (PEth), Alcohol Use Identification Test (AUDIT-C), average number of …

Cue-induced activation of implicit affective associations with heroin use in abstinent heroin abusers.

BACKGROUND AND OBJECTIVES: While drug-related contexts have been shown to influence drug users' implicit and explicit drug-related cognitions, this has been minimally explored in heroin abusers. This study examined the effect of heroin-related cue exposure on implicit and explicit valence and arousal-sedation associations with heroin use for abstinent heroin abusers. METHODS: In Experiment 1, 39 male abstinent heroin abusers were exposed to heroin-related words and reported cravings before and after cue exposure. They subsequently performed two Extrinsic Affective Simon Tasks (EASTs), which w…

Treatment response of bipolar and unipolar alcoholics to an inpatient dual diagnosis program.

BACKGROUND: Depressed and bipolar alcoholics represent a significant affective subgroup that has a poorer prognosis than either diagnosis alone. To date few systematic treatment programs have been developed to treat dual diagnosis. METHODS: An inpatient treatment program was developed at St Patrick's Hospital Dublin to treat dual diagnosis clients with alcohol dependence and either unipolar or bipolar affective disorder. Clients (N=232) were assessed for depression, anxiety, elation, cravings, drink and drug intake on admission, discharge, 3 and 6 months post-discharge from the program. RESU…

Long-term methadone maintenance effectiveness: psychosocial and pharmacological variables.

Data were collected from 265 heroin-dependent patients in long-term methadone maintenance treatment for methadone dosage, administration method, illicit drug and problematic alcohol use, psychiatric diagnoses, quality of interpersonal relationships, employment, legal problems, health, and cravings. Patients receiving higher methadone doses (more than 80 mg) were more likely to respond to methadone treatment than patients receiving lower doses. Superior outcome was also related to good quality of interpersonal relationships, stable employment, and lower craving scores. Comorbid psychiatric diso…

Harmonizing mind acupuncture combined with nicotine replacement therapy for tobacco use disorder: a protocol for a pragmatic randomized controlled trial.

BACKGROUND: Tobacco use disorder (TUD) remains a leading cause of preventable morbidity and mortality worldwide. Although nicotine replacement therapy (NRT) is recommended as a first-line treatment, its effectiveness is limited by persistent craving and withdrawal symptoms, resulting in high relapse rates. Acupuncture has been investigated as an adjunctive therapy; however, existing evidence is inconsistent and limited by methodological heterogeneity. High-quality pragmatic randomized controlled trials are needed. The co-primary outcomes are nicotine craving reduction (VAS) and biochemically v…

Sex Differences in Subjective and Behavioral Responses to Stressful and Smoking Cues Presented in the Natural Environment of Smokers.

INTRODUCTION: Some evidence suggests that female smokers may show more context-dependent smoking and that males may show more stereotyped smoking (regardless of stress or cue exposure). The goal of this study was to characterize sex differences in response to stressful and smoking cues ecologically presented in daily life and variability in day-to-day smoking behavior. METHODS: Adult smokers (N = 177) provided ratings of mood and cigarette craving before and after stress and smoking cues were presented four times daily for 14 days via a mobile device. Linear mixed models tested whether (1) fe…

State-Dependent Effects of Ventromedial Prefrontal Cortex Continuous Thetaburst Stimulation on Cocaine Cue Reactivity in Chronic Cocaine Users.

Cue-induced craving is a significant barrier to obtaining abstinence from cocaine. Neuroimaging research has shown that cocaine cue exposure evokes elevated activity in a network of frontal-striatal brain regions involved in drug craving and drug seeking. Prior research from our laboratory has demonstrated that when targeted at the medial prefrontal cortex (mPFC), continuous theta burst stimulation (cTBS), an inhibitory form of non-invasive brain stimulation, can decrease drug cue-related activity in the striatum in cocaine users and alcohol users. However, it is known that there are individua…
