Spinal stenosis poses a significant healthcare challenge in the United States. This case report delineates the progressive interventions in a male over 90 years of age with lumbar spinal stenosis, emphasizing the shift toward minimally invasive endoscopic spine surgery. The patient, burdened by neurogenic claudication, failed conservative measures, leading to substantial pain and diminished quality of life. Traditional open spine surgery, fraught with morbidity and mortality risks, prompted consideration of alternative approaches. The case details the journey of the patient through various int

STUDY DESIGN: A biomechanical study on the cadaveric human lumbar spine.
OBJECTIVE: We focused on a biomechanical comparison of the changes wrought on motion segments after a minimally invasive decompression and after a conventional medial facetectomy.
SUMMARY OF BACKGROUND DATA: Minimally invasive posterior decompression using a microscope or an endoscope is becoming popular for elderly patients with lumbar spinal canal stenosis. An advantage of the technique is that the cauda equina and nerve roots are in clear view and the facet joints, paravertebral muscles, and spinous process are well

Lumbar spinal stenosis can be effectively treated by performing an extensive ipsilateral spinal decompression, including a partial pediculotomy, and contralateral posterior bone fusion. Infrequently, complications can arise following radical decompression to alleviate symptoms of stenosis, and one such complication is a pedicle fracture. Three reports of pedicle fractures following extensive spinal decompression and contralateral posterior fusion are detailed. This complication is emphasized, and interventions are discussed. Three patients presented with symptoms attributable to lumbar stenosi

BACKGROUND: Endoscopic Endonasal Decompression for Traumatic Optic Nerve Injury has become an established endoscopic technique. However, Poor sphenoid sinus pneumatization often obscures landmarks, elevating the risk to vital neurovascular structures.
METHOD: By decompressing the orbital apex region and determining the contour of the optic nerve, sequential decompression of the medial wall, superomedial wall, and inferomedial wall of the optic canal.
CONCLUSION: This surgical technique can gradually reveal the contour of the optic canal when the anatomical landmarks on the lateral wall of th

Isthmic spondylolisthesis is a common degenerative disease of the spine and seriously affects people's quality of life. At present, surgical indications for lumbar spondylolisthesis have basically reached consensus. The surgical plan for the disease is mainly isthmus repair, decompression of spinal canal, reduction of spondylolisthesis, and spinal fusion. The principle of treatment is mainly to relieve nerve compression and restore spinal stability, but for each the specific method and degree of implementation of the link still remains controversial. Open surgery can complete decompression, re

The role of surgery in the treatment of idiopathic facial paralysis (Bell's Palsy) has been the subject of much controversy. Some have advocated aggressive surgical therapy to prevent nerve injury based on evoked electromyography (EEMG) results. The present study analyzes the outcome of 23 patients who presented with Bell's palsy and were evaluated with EEMG. Of the 15 patients who showed greater than 90 percent compound action potential reduction in the affected side, a widely used criterion for surgical decompression of the facial nerve, almost half (47%) had normal to near-normal recovery,

Patients with progressive blindness resulting from bony proliferative diseases such as osteopetrosis may benefit from optic canal decompression. A radiologic technique is described whereby the practicality of surgical optic canal widening is evaluated preoperatively using the intrathecal water-soluble contrast agent lopamidol. Conceptually, if a patient who is losing vision demonstrates a block on the optic neurogram, then likely there is still sufficient neural tissue within the optic canal and sheath to cause obstruction, and therefore decompression may be indicated. However, if preoperative

Lipofibromatous hamartoma (LFH) is a rare benign peripheral nerve tumor. The median nerve (MN) is most commonly affected in the upper extremity. We report a case of a 39-year-old male with LFH of the median nerve presented with swelling and symptoms of carpal tunnel syndrome treated successfully with decompression. LFH is reported with various descriptions because of the proliferative nature of its adipocytes and the fibrofatty infiltration within the peripheral nerves. Swelling around the volar aspect of the wrist remains the most frequent presentation of LFH. Surgical decompression without t

The objective of our study was to use decision analysis to compare four common surgical treatments for cubital tunnel syndrome: simple decompression of the cubital tunnel, medial epicondylectomy, anterior subcutaneous transposition and anterior submuscular transposition. The variables used for this decision analysis model were based on data from the literature. Extensive sensitivity analyses were carried out to test the impact of the values given to these variables on the outcome of the model. The highest expected utility, 0.973, was associated with simple decompression. The expected utility w

Traditional approaches to thoracic metastases and spinal cord compression have been well worked out and validated in the literature. Anterior decompression is clearly superior to laminectomy; vertebrectomy and reconstruction are indicated for sagittal collapse, instability, and pain; and surgical decompression is necessary in cases of bony impingement. The role of endoscopic and minimally invasive techniques in treatment of metastatic disease is evolving. Dr. Lieberman advocates the use of thoracoscopic anterior approaches as the principal application in these patients, whereas Dr. McLain has

The choice of a surgical approach for multi-level cervical spondylotic myelopathy (CSM) and ossification of the posterior longitudinal ligament (OPLL) is still a controversial issue. While most of the surgeons are still performing decompression by laminectomy, some are doing multi-level anterior decompression. Few neurosurgeons are performing decompression by corpectomy. We have treated 26 patients by median cervical corpectomy during the last 4 years. These patients were followed up for a mean period of 25 months. Twenty one (80%) patients had a good outcome, 2 patients remained unchanged and
