The treatment efficacy of antiresorptive therapy, including bisphosphonates, is not fully accounted for by change in bone mineral density (BMD). The 1-year increases in hip BMD were reported to be associated with a reduction in spine fracture by using the data from FIT trial, but the changes in spine BMD that was greater than hip BMD were not associated with the risk of spine, non-spine, or hip fracture. On the other hand, the diagnosis of osteoporosis is made mainly by measurement of BMD. In this sense, BMD measurement is thought to be useful in will improvement for the treatment of the patie

Gorham's disease (Gorham-Stout syndrome) is a rare condition of unknown etiology involving a localized endothelial proliferation of lymph vessels resulting in destruction with bone resorption. The syndrome is rarely seen in the facial skeleton and has a large variety of prognoses and treatments. A case of this syndrome in a 9-year-old boy is presented. The clinical aspects are described, together with the treatment involving zoledronic acid. Other treatments described in the literature are reviewed. The authors believe that this report is one of the first cases in which a child afflicted in th

Bisphosphonates are the current standard of care for treatment of osteoporosis. However, oral bisphosphonates are associated with complicated dosing regimens because of poor absorption and have the potential for upper gastrointestinal (GI) tract irritation, resulting in poor adherence and persistence. Zoledronic acid (ZOL) 5 mg, a once-yearly intravenous bisphosphonate, is approved for treatment and prevention of postmenopausal osteoporosis, increasing bone mass in men with osteoporosis, and treatment and prevention of glucocorticoid-induced osteoporosis. Because it is administered as an infus

Bisphosphonates are endogenous pyrophosphate analogs that work to inhibit osteoclast activity. They are commonly used in the treatment of patients with bone related diseases, such as solid tumor metastasis and osteoporosis. One of the infrequent but not rare side effects, especially with high doses of bisphosphonates, is osteonecrosis of the jaw. Although predominantly recognized by dentists because of the bony and intraoral manifestations of the disease, it may also present on the skin. We present a case of osteonecrosis of the jaw resembling two flesh-colored papules associated with dental s

Transient osteoporosis is a rare condition causing regional pain, immobility and functional compromise. The authors present the case of a pregnant lady with multiple sites of transient osteoporosis, including a stress fracture, successfully treated with intravenous pamidronate and a physical rehabilitation programme. The authors discuss the mechanisms for the syndrome and its overlap with complex regional pain syndrome type I; as well as the implications of bisphosphonate administration to a nursing mother. The authors also highlight the advantages of MRI over standard radiography when investi

Osteogenesis imperfecta (OI), is a heterogeneous group of inherited disorders of connective tissue characterized by bone fragility. Patients with this disease frequently suffer fractures, over 80% of the extremities due to the more intensive mechanical load. Fractures of the facial bones occur very infrequently. Several studies have proved that bisphosphonate therapy may be effective in reducing fracture risk in OI patients. We report here an unusual case of pathological mandibular fracture following simple molar extraction in an adult patient with OI type I and oral alendronic acid treatment.

Paget disease of bone is a focal disorder of the skeleton that can affect one or more bones. Many patients are discovered accidentally because of elevated serum alkaline phosphatase activity or an abnormal skeletal radiograph intended to evaluate an unrelated condition. Patients are often asymptomatic, but a subset experience considerable morbidity that can include bone pain and skeletal deformity, as well as a variety of regional complications, such as hearing loss associated with cranial involvement, degenerative arthritis of the hip or knee, fractures of the lower extremities and, rarely, s

OBJECTIVE: To analyze the clinical features of pregnancy and lactation-associated osteoporosis (PLO).
METHODS: The clinical data of 4 patients with PLO were retrospectively analyzed.
RESULTS: Bone pain and limitation of motion appeared within 2 months after childbirth in these 4 patients. X-ray films showed multiple vertebral fractures of thoracic and/or lumbar spine. Three of the 4 patients had positive family history of osteoporosis or low trauma fractures. After the diagnosis of PLO was confirmed, lactation was discontinued, and treatment of calcium, active vitamin D analogues, and bispho

BACKGROUND: An increasing number of cases of osteonecrosis of the jaws (ONJ) in patients treated with bisphosphonates has been reported in the literature. ONJ significantly affects the patients' quality of life and its management is still extremely difficult.
METHODS: A woman with ONJ secondary to therapy with zoledronic acid came to our attention for recurrent oral infections and orocutaneous fistula unresponsive to antibiotic therapy combined with minor surgical debridements. The patient underwent major surgery to remove the fistula and clean the necrotic bone and soft tissues; a lobed skin

Bisphosphonates are synthetic analogs of naturally occurring pyrophosphates, and they inhibit osteoclast-mediated bone resorption. They are widely used for the prevention and treatment of osteoporosis, treatment of Paget's disease of bone, management of metastatic cancer in bone, and short-term management of acute hypercalcemia. Osteonecrosis of the jaw is an adverse drug effect of bisphosphonates, and it typically presents as infection and necrotic bone in the mandible or maxilla. This article discusses pharmacology of bisphosphonates and osteonecrosis of the jaw, including its contributing f

Osteonecrosis of the jaws associated with the use of bisphosphonates (alendronate, pamidronate, risedronate et zoledronate) are described as an avascular necrosis. We have listed 184 cases of bisphosphonates-associated osteonecrosis of the maxillary in the literature. Many mechanisms are discussed in bisphosphonates-associated osteonecrosis: a disturbing bone turnover, an accumulation of bone's microdamage or an antiangiogenic effect. Other risk factors seem however to be involved: cancer, chemotherapy, glucocorticoids, infection and renal insufficiency.

The therapeutic opportunities for an improved management of malignant bone disease are currently extensively studied. The conventional management of symptomatic bone lesions in patients with advanced cancer involves various combinations of local and systemic standard anticancer therapies and the symptomatic treatment of skeletal complications. In recent years, bisphosphonates have demonstrated high efficacy to avoid skeletal complications from metastatic bone lesions and to prevent cancer treatment-induced bone loss. Especially in the treatment of patients with bone metastases, secondary to br

OBJECTIVE: To review the evidence and provide an approach to management of low bone density among premenopausal women. QUALITY OF EVIDENCE MEDLINE: was searched from January 1990 to November 2004 and articles graded by level of evidence (I to III). Diagnosis and management recommendations were based on evidence from randomized controlled trials and expert consensus.
MAIN MESSAGE: Bone mineral density (BMD) testing among premenopausal women should be completed only in the presence of approved indications. Current evidence does not support screening for osteoporosis among premenopausal women. L

Osteolysis is the greatest threat to the viability of modern acetabular implants. The etiology and natural history of osteolysis remain unknown. Patients with cementless implants who have osteolysis are challenging to treat. There are no standards for follow-up methods or frequency, and there is little evidence to support timing of the decision to begin treatment in these patients. The natural history of proposed treatment options is also unknown. Reconstruction of the acetabulum in the presence of bone loss is difficult. Classifications have been developed to guide management. Bisphosphonates are effective in the prevention and treatment of bone disease in multiple myeloma (MM). Osteonecrosis of the jaw is Increasingly recognized as a serious complication of long-term bisphosphonate therapy. Issues such as the choice of bisphosphonate and duration of therapy have become the subject of intense debate given patient safety concerns. We reviewed available data concerning the use of bisphosphonates in MM. Guidelines for the use of bisphosphonates in MM were developed by a multidisciplinary panel consisting of hematologists, dental specialists, and nurses specializing in

All patients receiving cortisone for more than 3 months should be offered not only calcium and vitamin D supplementation but also bisphosphonates in order to decrease the fractural risk. A screening for celiac disease should be performed in all patients with idiopathic osteoporosis. Following hepatic transplantation, the administration of bisphosphonates should be the rule.
