Nasal hypersensitivity in purulent middle ear effusion.

The existence of a physiopathologic connection between nose and middle ear is widely accepted so that chronic purulent middle ear effusion (CPMEE) could be expected to be usually associated with nasal chronic disease or impaired function. Nevertheless such association is less frequently observed in clinical practice than one could expect, possibly because of inadequate nasal function evaluation. Thirty-five patients affected by CPMEE were included in this study in order to assess the incidence of nasal disorders. E.N.T. clinical history was obtained and E.N.T. physical examination, nasal endos…

Bioglass middle ear prosthesis: long-term results.

The purpose of this study was to review the University of Florida's long-term results with Bioglass middle ear prostheses. Between April 1984 and November 1987, 37 patients were implanted with Bioglass prostheses (25 total and 12 partial ossicular replacements). Twenty-one patients had postoperative data of at least 24 months (range, 24 to 126 months; mean, 86 months; median, 100 months), and five patients had > 10 years' follow-up. In three cases, portions of fractured prostheses extruded, leaving an intact tympanic membrane. One patient with a total ossicular prosthesis was reexplored at 38 …

Middle ear infection and sixth nerve palsy.

Three cases of patients with acute middle ear infections complicated by a sixth nerve palsy are described. The possible causes of sixth nerve palsy in such cases are discussed. We propose that where there is a sixth nerve palsy without petrositis and without raised intracranial pressure, it can arise as a result of spreading phlebitis along the inferior petrosal sinus from the lateral sinus.

Middle ear imaging in neurotological work-up.

Middle ear imaging constitutes a homogeneous test battery for evaluation of neurotological disease. The imaging comprises infra-sound fistula test, ABR, tympanoscopy, ECoG, and trans-promontiorial cochlear blood flow measurement. We used a fistula test with infra-sound loading on posturography. In tympanoscopy we used 5 degrees and 25 degrees endoscopes with a diameter of 1.9 mm and length of 125 mm. In blood flow measurement we used laser-Doppler system with a stainless steel tip placed on the basal turn against stira vascularis. The flux was analyzed with a computer with custom-made software…

Chronic middle ear disease and cochlear implantation.

We have presented three patients with bilateral CSOM who were otherwise suitable for cochlear implantation. These patients had conventional surgical management of CSOM and were implanted after a period of 6 months through apparently healthy middle ears. However, all three had a recurrence of middle ear disease requiring further surgery, and one had a potentially life-threatening complication necessitating removal of the implant. Before considering cochlear implantation in patients with bilateral CSOM, it is essential to eradicate all disease, and radical mastoidectomy with obliteration may be …

Inner ear involvement in mixed cryoglobulinaemia patients.

In order to evaluate the nature and prevalence of audiovestibular disturbances in mixed cryoglobulinaemia (MC), 32 consecutive MC patients were studied by a wide audiological and vestibular examination. Pure tone audiometry, impedance audiometry, brainstem response audiometry and vestibular function were performed. Patients with a previous history of ear damage due to other well-known agents were excluded from the study. In MC patients we found a rather frequent audiovestibular involvement (34.3%). Bilateral sensorineural hearing loss was found in seven MC patients (22%) and altered vestibular…

Allergy caused by light-cured ear-moulds.

This investigation was conducted after the occurrence of allergic contact dermatitis in three laboratory technicians all engaged in the production of UV-light-cured acrylate ear-moulds. Ninety patients fitted with such moulds claimed discomfort to the same extent as patients with traditionally made acryl moulds. Of thirteen patients seen for clinical examination, seven were tested for allergy in the same way as the technicians and four were found to be sensitized to monomere metacrylates in the wax. The handling of the base materials in the laboratory as well as the UV-light-curing procedure n…

[Hemodilution therapy of acute inner ear damage].

Two independently conducted randomized clinical studies (sudden deafness; acute acoustic trauma) showed that in terms of hearing gain and the elimination of tinnitus low-molecular dextran 40 and 6% hydroxyethyl starch 40/0.5 do not produce any significant difference in therapy. Clear significant results were obtained with patients who suffered from acute acoustic trauma, when only NaCl-infusions were applied. Both rheologically effective substances show side effects, but they lie in the per-mil-range. On the grounds of measured clinical results we refuse zero or next to zero therapy of patient…

Clinical treatment of vascular inner ear diseases.

A multitude of general disorders of the vascular system may also affect the blood circulation of the cochlea and cause symptoms such as fluctuating or permanent hearing loss. Such is the case for arteriosclerosis combined with hypertension or hypotension, collagenosis, and diabetes. Blood disorders, like leukemia, sickle cell anemia, and polycythemia, and infectious diseases involving the blood vessels, such as lues, may also present their primary symptoms in the ear. The otorhinolaryngologist must be able to establish the correct diagnosis and refer patients requiring more general treatment t…

[Otogenic brain abscess and middle ear cancer].

An uncommon case is reported in which both a brain abscess and a carcinoma of the middle ear were diagnosed at the same time. Before the histology was known, a cholesteatoma was thought to be the cause of the brain abscess. The difficulties in the differential diagnosis are discussed.

Histopathology of chloroform-induced inner ear damage.

Inner ear function loss was caused in guinea pigs and rats by injecting chloroform into the middle ear. After symptoms for cochlear and vestibular deficit had been registered, the animals were permitted to survive for one day to five months. Ear histopathology was then studied in celloidin sections. In both species, hair cells and afferent nerve fibers were intact at all survival times. The acute stage of functional loss in guinea pigs was associated with inner ears of normal histological appearance. Within days after chloroform injection a severe otitis media developed which led to fibrous oc…

Abnormal middle ear muscle reflexes and audiosensitivity.

Reversed or hyperactive stapedial reflexes are found in a wide variety of patients complaining of audiosensitivity, defined as a need to turn down the volume on audio equipment. This was checked in an independent sample of children with learning problems. One or both of these reflex abnormalities were over three times commoner in those reporting audiosensitivity. This relationship was stronger in older children. Intolerance to noise not strictly defined as above was not associated with reflex abnormalities. Hyperacute pure tone thresholds were associated with lowered acoustic reflex thresholds…

Prefabricated ossicular homografts in middle ear reconstructions.

This paper is a review of 132 cases of ossicular reconstruction utilizing prefabricated homografts by clinical staff members of the UCLA Division of Head and Neck Surgery (specializing in ear surgery) in their private practice over the past decade. Another 20 cases completed over the past 2 1/2 years in the UCLA teaching programme are also reviewed. Although ossicular homografts have been used for some time in middle ear reconstruction, this report presents the innovation of prefabricating homografts into specific shapes that lend themselves appropriately to reconstruction problems. In such an…

Inner ear damage induced by Mycobacterium fortuitum.

We used electron microscopy to investigate Mycobacterium fortuitum-induced changes in the inner ears of mice. We found that the inner and outer hair cells had degenerated and disappeared in the organ of Corti. Changes in the lower turn of the cochlea were more severe than those of the upper turn while the changes of the outer hair cells were more severe than those of the inner hair cells. Disappearance, fusion and ballooning of the sensory hairs were observed in the vestibular organs. The bacterial extract also induced inner ear damage which was similar to that caused by live M. fortuitum.

Pursed lips breathing training using ear oximetry.

Pursed lips breathing (PLB) training is often used in the management of patients with chronic obstructive lung disease (COLD). Previous clinical studies have demonstrated that PLB improves arterial oxygen saturation (SaO2) and CO2 removal as well as relieving dyspnea. Twelve hypoxemic subjects with stable COLD were randomly assigned to either the pursed lips (P) or control group consisting of general relaxation (R). The SaO2 was monitored via ear oximetry, and respiratory rate and tidal volume were monitored using a strain gage transducer and the minute volume was calculated. The PLB was taugh…

Experimental evidence against middle ear oxygen absorption.

The present theory of eustachian tube (ET) function and middle ear (ME) ventilation posits that oxygen absorbed by the ME mucosa causes negative ME pressure which is relieved by periodic opening of the ET during swallowing and yawning. After developing a method to cannulate the ET of mongrel dogs we connected the cannulas hermetically to manometers. This system excluded ET function and tested the oxygen absorption capacity of the ME. When we controlled respiration and maintained blood gas PO2 and PCO2 at normal levels, we were unable to find any manometric evidence of negative pressure of gas …

Assessing middle ear disease: beyond visual otoscopy.

Visual otoscopy, the first step in the diagnosis of middle ear conditions, permits an assessment based on the color and appearance of the tympanic membrane. With pneumatic otoscopy, the mobility of the tympanic membrane can be evaluated. Several other techniques can be used to augment visual and pneumatic otoscopy. Impedance tympanometry provides an objective record of tympanic membrane movement. The acoustic otoscope evaluates middle ear contents by the reflectivity of sound from the tympanic membrane.
