Thirty-five Years of Acute Pain Services: Where Do We Go From Here?

Acute pain services (APS) have developed over the past 35 years. Originally implemented solely to care for patients with regional catheters … l services within a hospital (eg, procedure-specific patient pathways as indicated by the programs "enhanced recovery after surgery" or the "perioperative surgical home"), with patient-reported outcome measures as one central quality criterion. Pain-related functional impairment, treatment-induced side effects, speed of mobilization, as well as return to normal function and ever …

Effects of Different Rehabilitation Protocols in Inpatient Cardiac Rehabilitation After Coronary Artery Bypass Graft Surgery: A RANDOMIZED CLINICAL TRIAL.

PURPOSE: Patients undergoing coronary artery bypass graft (CABG) surgery typically experience loss of cardiopulmonary capacity in the post-operative period. The purpose of this study was to evaluate the effects of different rehabilitation protocols used in inpatient cardiac rehabilitation on functional capacity and pulmonary function in patient status post-CABG surgery. METHODS: This was a single-blind randomized controlled trial. The primary endpoint of functional capacity and secondary endpoints of lung capacity and respiratory muscle function were assessed in patients scheduled to undergo

Randomised comparison of three types of continuous anterior abdominal wall block after midline laparotomy for gynaecological oncology surgery.

Effective analgesia after midline laparotomy surgery is essential for enhanced recovery programs. We compared three types of continuous abdominal wall block for analgesia after midline laparotomy for gynaecological oncology surgery. We conducted a single-centre, double-blind randomised controlled trial. Ninety-four patients were randomised into three groups to receive two days of programmed intermittent boluses of ropivacaine (18 ml 0.5% ropivacaine every four hours) via either a transversus abdominis plane (TAP) catheter, posterior rectus sheath (PRS) catheter, or a subcutaneous (SC) catheter

The effect of nerve growth factor on corneal nerve regeneration and dry eye after LASIK.

Dry eye is the most common complication after refractive surgery, especially after laser in situ keratomileusis (LASIK), in which nerves may be cut when making the corneal flap. Nerve growth factor (NGF) has been demonstrated to stimulate corneal sensitivity and nerve regeneration and NGF has been suggested as a potential treatment for dry eye disease (DED). Hence, this study aimed to investigate the effect of NGF on corneal nerve regeneration, sensitivity and dry eye symptoms after LASIK, compared to hycosan and normal saline (NS) treatments. Thirty-eight New Zealand white rabbits that underw

Auricular vagus nerve stimulation promotes functional recovery and enhances the post-ischemic angiogenic response in an ischemia/reperfusion rat model.

Electrical stimulation of the vagus nerve, which has been used to treat epilepsy patients since 1997, also enhances long-term restoration after central nervous system (CNS) injury. Angiogenesis is a complex restorative mechanism that occurs in response to ischemic stroke, and it positively affects the recovery of neurological functions in a rat model of stroke. The aims of our study were to determine whether auricular vagus nerve stimulation (aVNS) promoted functional recovery and enhanced angiogenesis in the ischemic boundary following ischemia/reperfusion and to uncover the possible molecula

Precision application of target-controlled infusion of esketamine combined with sufentanil in anesthesia for thoracoscopic surgery and its effect on hemodynamics, postoperative pain, and safety.

We aimed to expound the precise application of target-controlled infusion (TCI) of esketamine combined with sufentanil in anesthesia for video-assisted thoracoscopic surgery (VATS) and its effect on hemodynamics, postoperative pain, and safety. Eighty patients scheduled for thoracoscopic procedures were randomly assigned to either a control group [n=40, conventional empiric anesthesia (sufentanil plus propofol)] or an observation group [n=40, TCI of esketamine and sufentanil]. Hemodynamic indices [mean arterial pressure (MAP), heart rate (HR), central venous pressure (CVP), stroke volume (SV),

A Case Series: Methylene-Blue-Related Toxic Anterior Segment Syndrome.

An intracameral injection of methylene blue 1% during cataract surgery induces extreme cytotoxicity, primarily affecting the corneal endothelium and iris epithelium. This case series illustrates the clinical presentation, management, and outcomes of three patients who underwent cataract surgery, with unintended exposure to methylene blue. All patients demonstrated early-onset corneal edema and anterior segment inflammation shortly after surgery. Two patients developed severe, irreversible endothelial decompensation, leading to persistent corneal edema, decreased visual acuity, and eventual nee

Thyroid hormone enhances transected axonal regeneration and muscle reinnervation following rat sciatic nerve injury.

Improvement of nerve regeneration and functional recovery following nerve injury is a challenging problem in clinical research. We have already shown that following rat sciatic nerve transection, the local administration of triiodothyronine (T3) significantly increased the number and the myelination of regenerated axons. Functional recovery is a sum of the number of regenerated axons and reinnervation of denervated peripheral targets. In the present study, we investigated whether the increased number of regenerated axons by T3-treatment is linked to improved reinnervation of hind limb muscles.

Standardizing Preoperative Care for Elective Spine Surgery: The University of Oklahoma Elective Spine Surgery Protocol.

As rates of spinal surgery in the United States continue to rise, the field has undergone a paradigm shift towards standardized, evidence-based care aimed at optimizing surgical outcomes. Indeed, numerous Enhanced Recovery After Surgery (ERAS) protocols have emerged and demonstrated significant success in improving outcomes in spinal surgery. In this article, we present a preoperative optimization protocol for elective spinal surgery from our own institution, which is in a state with notoriously high rates of comorbidities relative to the rest of the nation. This standardized protocol addresse…

Multicentre, prospective, randomized control non-inferiority trial of bladder catheter management in colon surgery.

AIM: Perioperative bladder catheterization is a controversial issue. Most current recommendations are based on data from open surgery extrapolated to enhanced recovery after surgery or fast-track programmes ranging between 24 and 48 h. The aim of this study is to provide a rationale for reducing catheterization time while at the same time avoiding acute urine retention (AUR), in patients undergoing scheduled laparoscopic colon surgery. METHOD: This is a multicentre, prospective, controlled, randomized non-inferiority study of bladder catheter management in patients undergoing scheduled laparo…

Effects of preoperative oral carbohydrate on clinical outcomes of patients undergoing hip fracture surgery: a protocol for systematic review and meta-analysis of randomised controlled trials.

INTRODUCTION: Hip fractures are a prevalent type of fracture and a leading cause of disability and mortality among older patients, imposing a substantial burden on both families and society. As surgical intervention is considered the primary means of improving patient outcomes, perioperative optimisation management is essential for enhancing prognosis. A key component of the Enhanced Recovery After Surgery (ERAS) protocol for hip surgery is the innovation of preoperative fasting concepts, which includes the recommendation of preoperative oral carbohydrates for surgical procedures, particularly…

Motor recovery and synaptic preservation after ventral root avulsion and repair with a fibrin sealant derived from snake venom.

BACKGROUND: Ventral root avulsion is an experimental model of proximal axonal injury at the central/peripheral nervous system interface that results in paralysis and poor clinical outcome after restorative surgery. Root reimplantation may decrease neuronal degeneration in such cases. We describe the use of a snake venom-derived fibrin sealant during surgical reconnection of avulsed roots at the spinal cord surface. The present work investigates the effects of this fibrin sealant on functional recovery, neuronal survival, synaptic plasticity, and glial reaction in the spinal motoneuron microenv

Co-transplantation of olfactory ensheathing cells from mucosa and bulb origin enhances functional recovery after peripheral nerve lesion.

Olfactory ensheathing cells (OECs) represent an interesting candidate for cell therapy and could be obtained from olfactory mucosa (OM-OECs) or olfactory bulbs (OB-OECs). Recent reports suggest that, depending on their origin, OECs display different functional properties. We show here the complementary and additive effects of co-transplanting OM-OECs and OB-OECs after lesion of a peripheral nerve. For this, a selective motor denervation of the laryngeal muscles was performed by a section/anastomosis of the recurrent laryngeal nerve (RLN). Two months after surgery, recovery of the laryngeal mov

Cardiac magnetic resonance imaging, myocardial scar and coronary flow pattern in anomalous origin of left coronary artery from the pulmonary artery.

BACKGROUND: Anomalous origin of left coronary artery from pulmonary artery (ALCAPA) is a very rare congenital heart defect characterized by myocardial ischemia and ultimately scaring. The scar burden will determine eventual recovery of left ventricular function after corrective surgery. MATERIAL METHOD: All patients with proven diagnosis of ALCAPA and who underwent treatment at present centre were included. Detail echocardiography and cardiac magnetic resonance imaging (CMR) (delayed Gadolinium enhancement) was performed before and after surgery. RESULTS: There were 4 patients (3 females, ag

Thoracoscopy-Guided vs. Ultrasound-Guided Paravertebral Block in Thoracoscopic Surgery: A Non-Inferiority Randomized Trial.

Background: Thoracic paravertebral block (TPVB) is an established component of multimodal analgesia and enhanced recovery pathways following thoracoscopic lung resection. A surgeon-performed, thoracoscopy-guided approach has been proposed to improve intraoperative workflow, but high-quality comparative data are limited. Methods: In this single-center, randomized, non-inferiority trial, adult patients undergoing thoracoscopic lobectomy or segmentectomy received either thoracoscopy-guided TPBV (T-TPVB) conducted by surgeons or ultrasound-guided TPBV (U-TPVB) conducted by anesthesiologists.
