The majority of the world's population still live without access to healthy water and the contamination of drinking water with protozoan pathogens poses a serious threat to millions of people in the developing world. Even in the developed world periodic outbreaks of diarrhoeal diseases are caused by the protozoan parasites Cryptosporidium sp., Giardia duodenalis and Entamoeba histolytica. Thus, surveillance of drinking water is imperative to minimize such contaminations and ensure continuous supplies of healthy water world-wide. This article reviews the progress in technology for detection and…

In experimental acute amebic liver abscess, produced in hamsters by the intraportal inoculation of 1 x 10(6) axenic trophozoites of Entamoeba histolytica strain HM-1, we examined the blood perfusion of the lesions 5, 10, 24 and 72 h after injection of the parasites. India ink introduced into the portal circulation filled all liver vessels but was systematically excluded from even the earlier amebic lesions. The absence of serum proteinase inhibitors from the lesions may allow the participation of amebic proteinases in the causation of tissue necrosis.

Among the wide variety of clinicopathological manifestations of intestinal amebiasis, amebomas occur rarely and their pathogenesis is not well understood. When cholesterol-fed, 2- to 4-week-old guinea pigs were infected intracecally with a virulent, monoaxenic strain of Entamoeba histolytica, gross and histologically characteristic amebomas developed in 85% of the animals by the 3rd day, in 94% by the 9th day, and in 96% by the 12th day postinfection, by which time most of them had died. Amebomas were confirmed by histopathology. Thus, a model of consistent production of amebomas was documente…

Globally, liver abscess caused by Entamoeba histolytica is a common condition. In New Zealand, however, it occurs infrequently. Eight cases were diagnosed in Auckland between 1972 and 1982, each patient having travelled through an endemic area within two years of presentation. Management often was not ideal, with both delays in diagnosis and inappropriate treatment. There is a need for increased awareness of possible amoebic abscesses in those who have been in endemic areas because with prompt use of effective treatment complications can be avoided.

Aboriginal children who were aged from six to nine years and living in Cherbourg Community were treated for Giardia Lamblia and Entamoeba histolytica infection with either the manufacturer's recommended dose of metronidazole (Flagyl) or with tinidazole (Fasigyn), at two-dose levels. Tinidazole successfully cleared both protozoans from stools, a single dose being as effective as the longer regimens which were studied. The obvious advantage of single-dose administration is that it should permit more effective treatment of these intestinal parasites, especially in Aboriginal populations.

The procedure used to diagnose amebiasis varies depending on the geographic location of the laboratory. However, in general, the microscopic diagnosis and differentiation of E. histolytica from other intestinal amebae is most satisfactorily achieved when the specimen is preserved immediately and both a concentration and stained smear are examined. The number of additional positive findings achieved by the cultivation of fecal material does not justify the time and cost involved. On the other hand, serology is a useful adjunct to diagnosis, especially in patients with extraintestinal amebiasis.

Amebiasis is a widespread parasitic disease caused by Entamoeba histolytica. This protozoan organism is the third leading parasitic cause of death in the developing world and is an important health risk to travelers in endemic areas. Amebiasis most commonly results in asymptomatic colonization of the gastrointestinal tract, but some patients may develop intestinal invasive disease or extraintestinal disease-amebic liver abscess being the most common extraintestinal manifestation. This article reviews epidemiologic features, pathophysiology, clinical features, diagnostic tests, imaging studies,…

An 8-year-old Turkish boy had recurrent abdominal colics associated with marked eosinophilia. Four different kinds of parasites were isolated from stool and duodenal juice: Fasciola hepatica, Entamoeba histolytica, Ascaris lumbricoides and Lamblia intestinalis. Conventional drugs were successful in treating the latter three parasites. But because of problems connected with the treatment of F. hepatica, praziquantel was administered, 15 mg/kg five times daily for 5 days, with curative results. This is the first published successful treatment of Fasciola hepatica with this drug in Europe.

Amebiasis is caused by Entamoeba histolytica, commonly found in the liver and colon. Its presentation is rare in the gallbladder. A 25-year-old female presented with epigastric pain radiating towards the right hypochondrium. The patient was diagnosed with calculous cholecystitis. Later on, performing a laparoscopic cholecystectomy, histopathology resulted in acute-on-chronic necrotizing cholecystitis with E. histolytica trophozoites. The treatment included metronidazole 500 mg thrice daily for 14 days, resulting in full healthy recovery. This case report highlights the importance of considerin…

Protozoan parasites represent a major threat to health and contribute significantly to morbidity and mortality worldwide, especially in developing countries. This is further compounded by lack of effective vaccines, drug resistance and toxicity associated with current therapies. Multiple protozoans, including Plasmodium, Entamoeba, Toxoplasma, and Leishmania produce homologs of the cytokine MIF. These parasite MIF homologs are capable of altering the host immune response during infection, and play a role in immune evasion, invasion and pathogenesis. This minireview outlines well-established an…

INTRODUCTION: An amebic liver abscess is the most common presentation of extraintestinal amebiasis. This condition is the result of a parasite infection caused by Entamoeba histolytica.
MATERIALS AND METHODS: We report a case of a 53-year-old male who presented with abdominal pain in the right upper quadrant, jaundice, and a 10-kg weight loss within a 1-month span.
RESULTS AND CONCLUSION: A wide range of symptoms and findings in the imaging tests suggestive of neoplasia, elevated levels of CA 19-9 and CA 125, and the presentation of biliary peritonitis as a complication makes this case a cha…

We present a case of fulminant amebic colitis in a human immunodeficiency virus (HIV)-infected homosexual man. The patient developed colonic perforation over a short time despite empirical therapy with metronidazole, and underwent right hemicolectomy. Amebic colitis was pathologically diagnosed by identifying invasive trophozoites of Entamoeba in a surgical specimen. Amebic colitis is one of the important differential diagnoses of acute abdomen in HIV-infected patients and/or homosexual men, especially in East Asia. Although fulminant amebic colitis is a rare manifestation of amebiasis, early …

The effect of the dehydroepiandrosterone analog 16alpha-bromoepiandrosterone (EpiBr) was tested on the tapeworm Taenia crassiceps and the protist Entamoeba histolytica, both in vivo and in vitro. Administration of EpiBr prior to infection with cysticerci in mice reduced the parasite load by 50% compared with controls. EpiBr treatment induced 20% reduction on the development of amoebic liver abscesses in hamsters. In vitro treatment of T. crassiceps and E. histolytica cultures with EpiBr, reduced reproduction, motility and viability in a dose- and time-dependent fashion. These results leave ope…

The host-parasite relationship is based on a series of interplays between host defense mechanisms and parasite survival strategies. Progress has been made in understanding the role of host immune response in amebiasis. While host cells elaborate diverse mechanisms for pathogen expulsion, amebae have also developed complex strategies to modulate host immune response and facilitate their own survival. This paper will give an overview of current research on the mutual interactions between host and Entamoeba histolytica in human and experimental amebiasis. Understanding this crosstalk is crucial f…

We looked at the effect of inhibiting caspases on amebic liver abscess in the mouse model of infection. A dose of the pan-caspase inhibitor benzyloxycarbonyl-V-A-D-O-methyl fluoromethyl ketone (Z-VAD-FMK; R & D Systems) given to SCID mice 2 h prior to direct hepatic inoculation with Entamoeba histolytica trophozoites, and 12 h after amebic inoculation, reduced the mean liver abscess size by 70% at 24 h compared to a control group. These data indicate that apoptosis plays a significant but not an exclusive role in amebic liver abscess formation in the mouse model.

Four cases of amoebiasis are described: two symptomatic with intestinal and hepatic involvement and two asymptomatic, diagnosed in two, heterosexual, Italian couples. Infection was probably acquired first by the men, via an indirect faccal-oral route, and then transmitted to their partners in the same way. The two amoebic strains isolated, from the woman of one couple and the man of the other, were characterized by electrophoresis as zymodemes II alpha- and XIX of Entamoeba histolytica. These four cases emphasise once more the role of cyst-passers in the spread of infection and the importance …

Hepatitis A virus (HAV) infection and amoebic liver abscess have an identical mode of transmission (fecal-oral route) and similar epidemiology. Either of these diseases might be considered in patients presenting with clinical signs and symptoms of liver disease in areas of the world where hepatitis and Entamoeba histolytica are endemic. The following two cases describe patients with simultaneous infection of HAV and amoebic liver abscess. This report emphasizes the role of dual infection in patients from endemic areas. It also challenges the assumption that jaundice in patients with amoebic li…

Results of histopathological examination of a solitary chronical amoebic abscess in the liver of a patient from Cambodia are described. The abscess was at a proliferously reparative phase and Entamoeba histolytica was detected at the border between the necrosis and inner wall of abscess, as well as in the proper non-specific granulation tissue of the inner layers of abscess capsule. A differential diagnosis of the amoebic abscess from similar parasitary or pseudoparasitary liver lesions and differential diagnosis of amoebae in histological sections are given. For orientation examinations of th…

The comparative histopathological details of amoebic liver abscess were studied following intracaecal and intramesenteric inoculation of Entamoeba histolytica into guinea-pigs. The histological changes of amoebic liver abscess formed via the former route were similar to those seen in man. These consisted of a central necrotic area with few lymphomononuclear cells and E. histolytica trophozoites at the periphery of the lesion. The amoebic liver abscess formed via the intramesenteric route, however, differed from that formed by intracaecal inoculation in three ways: (i) in having an intense infl…

Homosexual men are known to have an increased incidence of sexually transmitted proctitis. A knowledge of the pathogenesis and the radiographic appearance of these processes has resulted in earlier diagnosis and more rapid institution of appropriate therapy. While gonococcus (Neisseria gonorrhoeae) and lymphogranuloma venereum (usually Chlamydia) have long been considered the common etiologies of proctitis in this population, other organisms, such as herpes, Mycoplasma, and Entamoeba have been implicated and could give an identical radiographic and clinical pattern. Diffuse narrowing and ulcer…
