Linking Reductions in Alcohol and Birth Control Use Risk Behavior to Prevention of Alcohol-Exposed Pregnancy: A Population-Level Simulation of Preconceptual Prevention Programs.

BACKGROUND: The most popular alcohol-exposed pregnancy (AEP) prevention program (AEP-P) is a brief intervention that dually promotes reducing alcohol use and increasing birth control use. The primary outcome variable used to evaluate AEP-P involves aggregating behavioral change into a binary variable, despite differences in how AEP-P impacts each behavior. This study examined if all three possible AEP-P behavior-change outcomes (alcohol only (AO), birth control only (BCO), or both behaviors [BB]) are equally effective in preventing subsequent AEP itself. METHODS: We simulated implementing an

Multifaceted case management during pregnancy is associated with better child outcomes and less fetal alcohol syndrome.

BACKGROUND: Pregnant women participated in multifaceted case management (MCM) to prevent Fetal Alcohol Spectrum Disorders (FASD). METHODS: Women recruited from antenatal clinics for a longitudinal child development study were screened for alcohol use. Forty-four pregnant women were defined as high-risk drinkers on the Alcohol Use Disorder Identification Test (AUDIT) by an AUDIT score ≥8 and participated in 18 months of MCM to facilitate reduction or cessation of alcohol consumption. Forty-one women completed MCM. Fifty-five equally high-risk women who received standard antenatal care comprise…

Secondary physical features in children with FASD.

OBJECTIVE: The diagnoses included within the umbrella term fetal alcohol spectrum disorders (FASD), are based on the documentation of prenatal alcohol exposure (PAE), growth deficits and a pattern of dysmorphic physical features and neurobehavioral impairments. Although 3 key facial features (short palpebral fissures, a smooth philtrum and a thin vermilion of the upper lip) are the only dysmorphic features taken into account for the diagnosis of Fetal Alcohol Syndrome (FAS) or partial FAS (pFAS), several other features are commonly seen in individuals with these diagnoses. The goals of our stu…

The Alcohol Industry and Social Responsibility: Links to FASD.

Fetal Alcohol Spectrum Disorder is directly linked to the consumption of alcohol during pregnancy. Prevention programs have been targeted at women of childbearing age and vulnerable populations. The beverage alcohol industry (manufacture, marketing, distribution, and retail) is often seen as playing a role in prevention strategies such as health warning labels. In this paper we explore the nature of the relationship between the industry and prevention programming. We consider the place of alcohol in society; the prevalence, social and economic costs of FASD; the ethical notion of alcohol-relat

Fathers' Role in Alcohol-Exposed Pregnancies: Systematic Review of Human Studies.

CONTEXT: The role of paternal alcohol consumption on fetal and infant health outcomes, and on social facilitation of maternal alcohol consumption during pregnancy, has not been well established. This review identifies the range of impacts of paternal preconception alcohol consumption and alcohol consumption during partner's pregnancy, on maternal consumption, and fetal and infant health outcomes. EVIDENCE ACQUISITION: The review accessed articles from the following databases: Scopus, Science Direct, Wiley Online, MEDLINE, ProQuest Central, PsycINFO, and Web of Science. The review included med

Reviewers' bias against the null hypothesis: the reproductive hazard of binge drinking.

We examined whether scientific reviewers exhibit bias in scoring a simulated "positive" study (i.e. showing adverse fetal effects) as compared to a simulated "negative" study on the fetal effects of binge drinking. The reviewers of the "negative" study tended to reject it more commonly, to give it lower scores, and there was significantly more variability from the median in their scores. Scientific journals should make an effort to eliminate this source of bias against negative results.

Promoting alcohol abstinence among pregnant women: potential social change strategies.

Fetal Alcohol Syndrome Disorder (FASD) is one of the most preventable sources of developmental abnormalities, and has a singular cause-alcohol consumption during pregnancy. Estimates for the costs of treatment of a single case of FASD range often above one million dollars. The primary strategy for prevention currently centers on no alcohol consumption during pregnancy. However, a sizeable number of North American women currently drink during pregnancy. A literature review examined the behavior of maternal alcohol consumption in order to understand the rationale associated with drinking. Genera

Alcohol use in pregnancy.

The epitome of FAS and ARBD has been documented substantially in the medical literature. There appears to be little question that alcohol is a teratogen. The precise nature for the mechanism of causing congenital defects, whether it be direct or indirect, is still unknown. There is no unequivocal information that can be conveyed to patients regarding the quantity of alcohol that can be safely consumed during pregnancy. Because pregnant women generally are receptive to suggestions about controlling their alcohol consumption during pregnancy, the professional is presented with an excellent oppor

[Alcohol embryopathy. Review and case example].

Fetal alcohol syndrome is a disorder that occurs in children of alcoholic mothers and is characterized by typical craniofacial dysmorphia and retardation of mental and physical growth. Semiquantitative evaluation of the most common symptoms permits clinical classification into three stages. Although numerous hypotheses have been suggested, the pathogenetic mechanism is not yet well understood. Prevention through medical advice and counselling still remains the most successful form of therapy. A case of fetal alcohol syndrome caused by excessive maternal drinking accompanied by high nicotine co

Estradiol, estriol, progesterone, prolactin, and human chorionic gonadotropin in pregnant women with alcohol abuse.

Serum estradiol, estriol, progesterone, PRL, and hCG concentrations were measured at 4-week intervals in 40 pregnant women who chronically abused alcohol and in 20 abstinent pregnant women. Sixteen alcohol users gave birth to infants with the fetal alcohol syndrome (FAS), whereas the remaining 24 infants of alcohol users and all infants of the abstinent women were healthy. The women whose infants had the FAS had decreased estradiol and estriol levels throughout pregnancy. Progesterone levels also tended to be low. In contrast, the alcohol users had increased PRL levels during the 16-24th weeks

A single dose of ethanol suppresses rat embryo development in vivo.

In humans and animal models, maternal ethanol consumption during pregnancy results in a variety of fetal defects collectively termed the fetal alcohol syndrome (FAS). Limited follow-up studies suggest that FAS children fail to achieve normal physical or mental development despite significant postnatal intervention. Although the complete FAS appears to result only when chronic, excessive alcohol consumption occurs throughout pregnancy, several investigators have suggested that ethanol consumption at intermediate levels may induce components of FAS. The defect most consistently observed in neona

[Maternal alcoholism and its impact on child health].

Maternal alcoholism hinders the normal development of child and threatens his mental and physical health due to three factors: the hereditary transmission of predisposition to alcohol abuse; alcohol consumption during pregnancy; adverse family environment. The children of mothers suffering from alcoholism revealed are characterized by increased risk of depression, anxiety and other mental disorders, including alcohol and substance dependence. The adverse impact of maternal alcoholism (or, to speak more widely, parents' alcoholism) on the child health requires special preventive and treatment p
