Results of ring (Ilizarov) fixator in high energy Schatzker type VI fractures of proximal tibia.

BACKGROUND: High-energy fractures of proximal tibia (Schatzker VI) are associated with severe articular depression, separation of both condyles, diaphyseal comminution & dissociation with loss of integrity of the soft-tissue envelope. Complications of plating are well known since last 50years in these difficult fractures. An alternative method was proposed by Ilizarov (ring fixator) and was adopted for the treatment of these complex injuries. AIM OF STUDY: To analyse the results of patients who had ring fixator for the management of high-energy fractures (Schatzker VI) of the proximal tibia.M

Fully automated segmentation of callus by micro-CT compared to biomechanics.

BACKGROUND: A high percentage of closed femur fractures have slight comminution. Using micro-CT (μCT), multiple fragment segmentation is much more difficult than segmentation of unfractured or osteotomied bone. Manual or semi-automated segmentation has been performed to date. However, such segmentation is extremely laborious, time-consuming and error-prone. Our aim was to therefore apply a fully automated segmentation algorithm to determine μCT parameters and examine their association with biomechanics. METHODS: The femura of 64 rats taken after randomised inhibitory or neutral medication, in

Biologic plating of unstable distal radial fractures.

BACKGROUND: Volar locking plating through the flexor carpi radialis is a well-established technique for treating unstable distal radial fractures, with few reported complications. In certain circumstances, including metaphyseal comminuted fractures, bridge plating through a pronator quadratus (PQ)-sparing approach may be required to preserve the soft tissue envelope. This study describes our prospective experience with bridge plating through indirect reduction. METHODS: Thirty-three wrists (four 23A2, six 23A3, 15 23C1, and eight 23C2) underwent bridge plating through a PQ-sparing approach wi

Midterm results with a bipolar radial head prosthesis: radiographic evidence of loosening at the bone-cement interface.

BACKGROUND: Metal prostheses are useful for restoring elbow and forearm stability when the radial head cannot be fixed after a fracture. Because the anatomy of the radial head is difficult to reproduce with a prosthesis, two different options have been proposed: a bipolar prosthesis with a fixed stem and a mobile head, and a monoblock prosthesis with a smooth stem that is intentionally fixed loosely in the neck of the radius. One concern with a fixed-stem implant with a mobile head has been the risk of osteolysis. The purpose of this study was to evaluate radiographic changes reflecting or sug

[Treatment of complex hand injuries by stable osteosynthesis using a "multiple pinning" technique].

UNLABELLED: The multiple pinning technique has been reported by the authors as an effective method for the treatment of comminuted osteo-articular fractures of the digits. This procedure can be performed rapidly, offers good stability and thereby allows early post-operative physiotherapy. The aim of this type of management is to offer the patient recovery of thumb to digit pinch by restoring two functional joints (MCP, PIP) or at least one functional joint (MCP), particularly as far as the radial digits are concerned. TECHNIQUE: After performing reduction, very small diameter pins are introdu

Risk factors for loss of reduction following locked plate fixation of proximal humerus fractures in older adults.

PURPOSE: To identify characteristics associated with loss of reduction following open reduction and locked plate fixation (ORIF) of proximal humerus fractures in older adults and determine if loss of reduction affects patient reported outcomes (PROs), range of motion (ROM), and complication rates during the first postoperative year. METHODS: Patients >55 years old who underwent proximal humerus ORIF were reviewed. Patient and fracture characteristics were recorded. Fixation characteristics were measured on the initial postoperative AP radiograph including humeral head height (HHH) relative to

Posterior malleolus fracture.

Posterior malleolus fractures are a common component of ankle fractures. The morphology is variable; these fractures range from small posterolateral avulsion injuries to large displaced fracture fragments. The integrity of the posterior malleolus and its ligamentous attachment is important for tibiotalar load transfer, posterior talar stability, and rotatory ankle stability. Fixation of posterior malleolus fractures in the setting of rotational ankle injuries has certain benefits, such as restoring articular congruity and rotatory ankle stability, as well as preventing posterior talar translat

Hawkins' type-II talar fracture with subtalar dislocation: A very unusual combination.

We report the unusual case of a 16-year-old young man who sustained a rare association of a Hawkins' type-II talar neck fracture with a complete medial subtalar dislocation (Hawkins type-IIB) that occurred as an isolated injury after indirect trauma during a soccer game. Following closed reduction of the subtalar dislocation, standard radiographs and computed tomography (CT) demonstrated a comminuted fracture of the talus involving the base of the talar neck. Open reduction was performed and the fracture was stabilized by ORIF. At 1-year follow-up, functional and radiographic outcomes were gra

Fractures of the lateral process of the talus: two case reports and a comprehensive literature review.

Fractures of the lateral process of the talus are frequently overlooked and should be considered in the differential diagnosis of patients with acute and chronic ankle pain. Early diagnosis is emphasized in all series reviewed in the literature to prevent long-term complications. Thorough radiographic evaluation is necessary to determine the need for operative vs. nonoperative management. Small nondisplaced fractures are treated with cast immobilization, whereas large or displaced fractures usually require open reduction and internal fixation. Comminution of fragments may necessitate surgical

A plea for judgment in management of thoracolumbar fractures and fracture-dislocations. A reassessment of surgical indications.

Until basic knowledge of fracture anatomy and stability includes experiments with comminuted anterior column injuries, posterior column injuries, and these injuries in combination with ligament and capsular injuries, and until the long-term implications of the long fusion in an otherwise healthy patient are fully known, clinicians making decisions about individual patients must assemble all the variables that bear on the patient's ability to heal and become fully functional again before making a decision about surgical stabilization of the thoracolumbar fracture. An assessment of plain roentge

Venous insufficiency as a late complication after tibial fracture.

Thirty-eight patients were followed up 13 to 17 years after sustaining a fracture of the tibia. The aim was to establish the frequency of post-thrombotic venous insufficiency. All patients were examined clinically and with a modified ultrasound technique. In addition some were studied using phlebography. Fifteen patients had symptoms of venous insufficiency in the fractured leg. Seven had only local symptoms, while in the other eight the venous insufficiency was verified objectively. The following factors predisposed to the development of venous insufficiency: Age above 45 years at the time of

[Fractures and dislocations of the lumbar vertebrae (author's transl)].

The authors report 6 cases of fracture and dislocation of the lower lumbar vertebrae (L3, L4, L5) and note that they were usually lesions following violent trauma usually in hyperflexion. The following were treated 1 dislocation L3-L4, 2 dislocations L4-L5, 2 comminuted fractures of L4, 1 fracture of L5, and included in all cases intersomatic arthrodesis at one (five cases) level or two levels (one case) and one posterior internal fixation with Kempf's material. The simplicity of the follow up and the stability of the fixation, permit us to propose the method for all unstable lesions of the lo

To plate, or not to plate? A systematic review of functional outcomes and complications of plate fixation in patellar fractures.

PURPOSE: Poor outcomes and high complication and reoperation rates have been reported with tension-band wiring (TBW) in the management of patellar fractures and particularly the comminuted ones. The purpose of this study was to investigate the functional outcomes and complication rates of patellar fractures managed with open reduction and internal fixation (ORIF) with a plate. METHODS: MEDLINE, EMCare, CINAHL, AMED and HMIC were searched, and the PRISMA guidelines were followed. Two independent reviewers extracted the data from the included studies and assessed them for the risk of bias. RES

Submandibular approach through the submandibular gland fascia for treating mandibular fractures without identifying the facial nerve.

BACKGROUND: : The submandibular, or Risdon, approach is the classic, reliable extraoral approach for treating mandibular fractures. Although the intraoral approach has been used recently for open reduction and internal fixation, in some cases, such as comminuted fractures or fractures in an atrophic mandible, the submandibular approach is still prerequisite. Damage to the marginal mandibular branch of the facial nerve is the only concern other than skin scarring. METHODS: : To minimize the risk of neuropraxia, we prospectively assessed an approach for treating mandibular fractures at 24 surgi

Bilateral anterior shoulder dislocation: A case report.

Bilateral anterior shoulder dislocation (BASD) is a rare condition typically associated with a history of trauma. This case report presents a patient with BASD resulting from trauma and discusses treatment options. A 51-year-old male patient was admitted to the emergency department following a fall from a height. Radiographic examination revealed BASD and a comminuted intra-articular fracture of the distal end of the left radius. The patient did not have any other additional pathologies. After a preoperative evaluation, the patient was urgently taken to the operating room, where both shoulders

Suicide in parachuting: A case report and review of the literature.

Skydiving fatalities are mostly accidental and the result of human errors. However, suicides may be greatly underreported in skydivers. We present the case of a young civilian skydiver who committed suicide by jumping from an altitude of 4000m without activating his chutes. Witnesses reported that the victim had remained in a freefall position until ground impact. Besides an extensive blunt trauma, the autopsy showed an antero-posterior flattening of the body with symmetrical abrasions on its front, which were consistent with a high-energy impact on the ground in a "belly-down" position. Staged treatment of osteonecrosis of the femoral head induced by delayed treatment of polytrauma complicated by chronic acetabular fracture and hip dislocation: a case report.

Delayed intervention for hip fracture and dislocation is a core risk factor for osteonecrosis of the femoral head (ONFH). According to the latest research, the incidence of ONFH in patients with acetabular fracture combined with hip dislocation can reach 30%-40%, and this risk increases 1.89 times if treatment is delayed for more than 3 weeks. This presents a clinical challenge in polytrauma management, where systemic stability must be balanced against timely hip injury intervention under the principles of damage control orthopedics. This study reports the case of a 45-year-old man with severe

Atypical Femoral Fracture Secondary to Long-Term Bisphosphonate Use: A Case Report.

Atypical femoral fractures (AFFs) are uncommon complications of long-term use of bisphosphonates (BPs), a common medication used for osteoporosis and other conditions that reduce bone strength and density. BPs are effective in reducing the risk of fractures. Prolonged use of the medication can inhibit bone remodeling and increase the risk of insufficiency fractures, most especially in the subtrochanteric and diaphyseal regions of the femur. This case follows an 81-year-old Caucasian woman with a history of multiple myeloma (MM) in remission and long-term sodium clodronate use for skeletal prot
