Knowledge of diagnosis, treatment history, and risk of late effects among childhood cancer survivors and parents: The impact of a survivorship clinic.

BACKGROUND: Childhood cancer survivors are at risk for treatment-related adverse health outcomes, known as late effects. Through matched and longitudinal cohorts, we assessed the impact of survivorship care on patient and parent knowledge of treatment history and associated health risks. PROCEDURE: Childhood cancer survivors were recruited from a single-institution survivorship clinic and matched with survivors receiving routine follow-up care (controls) on diagnosis, age, and time off therapy. One hundred seventy-four participants completed telephone interviews assessing knowledge of diagnos

The theoretical model of the school-based prevention programme Unplugged.

Unplugged is a school-based prevention programme designed and tested in the EU-Dap trial. The programme consists of 12 units delivered by class teachers to adolescents 12-14 years old. It is a strongly interactive programme including a training of personal and social skills with a specific focus on normative beliefs. The aim of this work is to define the theoretical model of the program, the contribution of the theories to the units, and the targeted mediators. The programme integrates several theories: Social Learning, Social Norms, Health Belief, theory of Reasoned Action-Attitude, and Probl

A qualitative study of pandemic influenza preparedness among small and medium-sized businesses in New York City.

Small businesses need to engage in continuity planning to assure delivery of goods and services and to sustain the economy during an influenza pandemic. This is especially true in New York City, where 98 per cent of businesses have fewer than 100 employees. It was an objective therefore, to determine pandemic influenza business continuity practices and strategies suitable for small and medium-sized NYC businesses. The study design used focus groups, and the participants were owners and managers of businesses with fewer than 500 employees in New York City. The main outcome measures looked for w

Gender differences in the impact of families on alcohol use: a lagged longitudinal study of early adolescents.

AIMS: From the pre-teen to the mid-teen years, rates of alcohol use and misuse increase rapidly. Cross-sectional research shows that positive family emotional climate (low conflict, high closeness) is protective, and there is emerging evidence that these protective mechanisms are different for girls versus boys. The aim of this study was to explore gender differences in the longitudinal impact of family emotional climate on adolescent alcohol use and exposure to peer drinking networks. DESIGN: Three-wave two-level (individual, within-individual over time) ordinal logistic regression with alco

Can ill-structured problems reveal beliefs about medical knowledge and knowing? A focus-group approach.

BACKGROUND: Epistemological beliefs (EB) are an individual's cognitions about knowledge and knowing. In several non-medical domains, EB have been found to contribute to the way individuals reason when faced with ill-structured problems (i.e. problems with no clear-cut, right or wrong solutions). Such problems are very common in medical practice. Determining whether EB are also influential in reasoning processes with regard to medical issues to which there is no straightforward answer, could have implications for medical education. This study focused on 2 research questions: 1. Can ill-structur

[Female-to-male transsexual's stereotype among university men and women on the basis of semantic differential].

AIM: The aim of this study is to determine female-to-male transsexual's (FtM) stereotypes created in female and male university students and comparing them in the analysed groups. METHOD: 300 university students (from the Technical University of Łódź, University of Łódź, Medical University of Łódź) received a questionnaire with 10 pairs of the opposite meaning adjectives (good-bad, weak-strong, fast-slow, ugly-pretty, fat-thin, suppressed-loud, clever-stupid, light-heavy, active-passive, kind-cruel) related to a notion 'female -to-male transsexual' and placed on the extremes of the 7-point sc

Assessment of attitudes and practices of providers of services for individuals at high risk of HIV and sexually transmitted infections in Karnataka, south India.

BACKGROUND: A key component of prevention programmes aimed at reducing the risk of HIV and sexually transmitted infections (STI) among high-risk groups such as female sex workers (FSW) is the provision of quality STI services. The attitudes and practices of care providers are critical factors in the provision of services and in achieving better participation of high-risk individuals in accessing services. OBJECTIVES: To assess the attitudes and practices of STI care providers towards FSW, and the perceptions of FSW towards STI services, following training. METHODOLOGY: After an intensive phy

Fear, fatalism and breast cancer screening in low-income African-American women: the role of clinicians and the health care system.

BACKGROUND: African-American women have the highest breast cancer death rates of all racial/ethnic groups in the US. Reasons for these disparities are multi-factorial, but include lower mammogram utilization among this population. Cultural attitudes and beliefs, such as fear and fatalism, have not been fully explored as potential barriers to mammography among African-American women. OBJECTIVE: To explore the reasons for fear associated with breast cancer screening among low-income African-American women. METHODS: We conducted four focus groups (n = 29) among a sample of African-American wome

Exploring diaphragm use as a potential HIV prevention strategy among women in the United States at risk.

Given the immediate need for physical cervical barrier methods like the diaphragm to protect against HIV/STIs, understanding what factors influence the acceptability of these products and how to overcome obstacles to their use is important. We explored perceptions of the diaphragm and factors that might enhance its acceptability in 25 focus groups with racially/ethnically diverse young women in the U.S. at risk for HIV/STIs (N = 140). Women believed the diaphragm has positive attributes, and most indicated they would be more likely to use the diaphragm if they were confident they could use it

More than just vaginal intercourse: anal intercourse and condom use patterns in the context of "main" and "casual" sexual relationships among urban minority adolescent females.

PURPOSE: Receptive anal intercourse has long been recognized as a risk factor for the transmission of HIV and STIs. Most HIV/STI prevention messages continue to emphasize penile-vaginal transmission, so many teens do not recognize anal intercourse as risky sexual behavior. This purpose of this study was to determine the prevalence of anal intercourse, and to describe the relationship between types of sexual relationships (main vs. casual), types of sexual behaviors (vaginal vs. anal sex), and types of contraception used among inner-city sexually experienced adolescents. METHODS: Reproductive

Professional role and identity in a changing society: three paradoxes in Swedish midwives' experiences.

OBJECTIVES: to explore how experienced midwives understand and experience their professional role and identity after the continuing changes that have taken place within their working domain over the last 20-25 years. DESIGN: four focus group interviews with 20 a total of participants in total were conducted and analysed by thematic content analysis. SETTING: one university hospital in mid-Sweden. FINDINGS: two main themes were conceptualised: (1) organisation of and situation at the workplace; and (2) the societal context. Three paradoxes became apparent. The midwives felt that their profes

Diagnosis of genital chlamydia in primary care: an explanation of reasons for variation in chlamydia testing.

OBJECTIVES: To explore the reasons for the 40-fold variation in diagnostic testing for genital Chlamydia trachomatis by general practices. METHODS: A qualitative study with focus groups. We randomly selected urban and rural high and low testing practices served by Bristol, Hereford, and Gloucester microbiology laboratories. Open questions were asked about the investigation of C trachomatis in men and women in different clinical contexts. RESULTS: The high and low testing practices did not differ in their age/sex make-up or by deprivation indices. There were major differences between high and

Treatment of severe thrombocytopenia in alloimmunized, transfusion-refractory patients.

A significant proportion of patients with hematologic malignancies who are exposed to multiple transfusions will develop alloantibodies to platelet human leukocyte antigens (HLA), resulting in poor responses to subsequent platelet transfusions. Transfusion of HLA-identical platelets is an effective method of platelet support in these patients, but perfectly HLA-matched platelets are often not available. In this paper, we review the recent literature on platelet transfusion support in alloimmunized individuals and illustrate alternative management strategies with cases from our own practice.

Attitudes about skin cancer prevention: a qualitative study.

BACKGROUND: Skin cancer represents a significant threat to the health and well-being of Americans. By engaging in both primary and secondary preventive behaviors, individuals can reduce their risks of developing skin cancer. METHODS: Focus groups were used as a qualitative technique to explore the similarities and differences between those who practiced skin cancer prevention (a high-concern group) and those who did not (a low-concern group). Transcripts from the discussions were analyzed to identify themes regarding participants' attitudes, beliefs, and practices about skin cancer prevention

Pseudosclerodermatous panniculitis after irradiation: an unusual complication of megavoltage treatment of breast carcinoma.

An unusual edematous and indurated erythema developed in four patients with breast carcinoma 1 to 6 months after conservative surgical treatment and irradiation. The radiation therapy consisted of megavoltage x-ray photon with or without either electron beam or iridium-192 interstitial boost. Several tissue biopsy specimens revealed pronounced lymphocytic dermal and fat inflammation in conjunction with focal areas of plasma cells. The connective tissue bundles were enlarged and hyalinized. Macrophages and isolated giant cells were noted in the dermis. One biopsy specimen showed elastic tissue

Changes of upper-limb kinematics during practice of a redundant motor task in patients with Parkinson's disease.

The ability to learn novel motor skills is essential for patients with Parkinson's disease (PD) to regain activities of daily living. However, the underlying mechanisms of motor learning in PD remain unclear. To identify motor features that are distinctively manifested in PD during motor learning, we quantified a rich set of variables reflecting various aspects of the learning process in a virtual throwing task. While the performance outcome improved similarly over 3 days of practice for both PD patients and age-matched controls, further analysis revealed distinct learning processes between th

Regression-based cognitive change norms applied in biochemically defined predementia Alzheimer's disease.

OBJECTIVE: We aim to develop 2-year cognitive change norms for adults ages 41-84 for six cognitive tests, and to evaluate these norms in groups with AD biomarkers. BACKGROUND: Practice effects are common in repeated neuropsychological testing. Not accounting for practice effects may obscure cognitive decline in early Alzheimer's disease (AD). METHOD: We developed standardized regression-based change norms from normative samples consisting of healthy controls from the Dementia Disease Initiation study (n = 125), the Trønderbrain study (n = 57), and the Gothenburg mild cognitive impairment (MC

Taking another's perspective promotes right parieto-frontal activity that reflects open-minded thought.

Critical thinking (CT) is important for consensus building. Although the practice of CT using debate is widely used to improve open-minded thought, the cognitive processes underlying this improvement remain poorly understood. Here, functional magnetic resonance imaging (fMRI) was employed to assess how neural responses while considering another's opinion are changed by CT practice and to determine whether the cortical network showing activation changes related to personality traits is relevant to consensus building. A total of 52 healthy participants were divided into three groups for an inter
