Theorizing "Big Events" as a potential risk environment for drug use, drug-related harm and HIV epidemic outbreaks.

Political-economic transitions in the Soviet Union, Indonesia, and China, but not the Philippines, were followed by HIV epidemics among drug users. Wars also may sometimes increase HIV risk. Based on similarities in some of the causal pathways through which wars and transitions can affect HIV risk, we use the term "Big Events" to include both. We first critique several prior epidemiological models of Big Events as inadequately incorporating social agency and as somewhat imprecise and over-generalizing in their sociology. We then suggest a model using the following concepts: first, event-specif

Association between posttreatment gambling behavior and harm in pathological gamblers.

This study investigated the relationship between posttreatment gambling behavior and harm in a sample of treatment-seeking pathological gamblers. One year after initiating treatment, participants (n = 178) completed the Gambling Timeline Followback (D. C. Hodgins & K. Makarchuk, 2003; J. Weinstock, J. P. Whelan, & A. W. Meyers, 2004) and the South Oaks Gambling Screen (SOGS; H. R. Lesieur & S. B. Blume, 1987), both of which assessed gambling behaviors and problems over the prior 6 months. Based upon self-reports of gambling-related problems on the SOGS, participants were classified as problem

Research Review: Conceptualizing and measuring 'problem behavior' in early intervention autism research - a project AIM secondary systematic review.

BACKGROUND: Some autistic children exhibit behavior that caregivers, clinicians, and researchers consider problematic. However, there is little consensus about the types of behaviors that should be treated as a problem and reduced via intervention. In autism intervention research, problem behaviors range from inherently harmful behaviors such as aggression and self-injury to nonnormative but not harmful behaviors associated with autism such as repetitive movements. Likewise, there are a variety of conceptualizations and measurement practices used to assess these behaviors. METHODS: In this se

Measurement invariance and other psychometric properties of the Short Inventory of Problems (SIP-2R) across racial groups in adults experiencing homelessness and alcohol use disorder.

OBJECTIVE: People experiencing homelessness are disproportionately impacted by alcohol-related harm. Racially minoritized groups are disproportionately represented in the homeless population and are likewise disproportionately impacted by alcohol-related harm. Most alcohol outcome measures have not been adequately psychometrically studied in this marginalized population and across racial groups. This study documents psychometric properties, including measurement invariance, reliability, and convergent validity, of a measure of alcohol-related harm, the Short Inventory of Problems (SIP-2R), acr

Eliminating Seclusion Use for Women in Secure Forensic Hospitals. Are We Doing Enough to Consider Sex and Gender Differences for Elimination?

The reduction and eventual elimination of seclusion is a global priority. In 2021, The Royal Commission into Victoria's Mental Health System recommended that seclusion use be eliminated from mental health services within 10 years. This is long needed given human rights concerns and known harms associated with seclusion use. Yet the Commission failed to adequately consider gender-specific needs of women, and women who may be at increased risk of harms associated with seclusion use. Further, current approaches for seclusion reduction lack an explicit gendered lens. There are noted gendered diffe

Tetanus: A Rare Complication of Black Tar Heroin Use.

Tetanus is associated with high morbidity and mortality, although this is rarely encountered in high-income countries. We present a case of tetanus in an unvaccinated patient secondary to black tar heroin use that highlights the importance of considering tetanus in appropriate clinical contexts, harm reduction interventions, and universal tetanus vaccination campaigns.

Informing the Equitable Implementation of Hepatitis C Interventions: Cost-Effectiveness Modeling in Rural Populations of People Who Inject Drugs.

OBJECTIVES: This study evaluates the cost effectiveness of implementing evidence-based interventions among persons who inject drugs (PWID) in rural settings, where structural barriers and limited resources contribute to disproportionate hepatitis C virus (HCV) burden. Findings are intended to guide equitable implementation planning and resource allocation. METHODS: We developed an economic-epidemiological model to describe HCV transmission among PWID to assess the cost effectiveness of testing, treatment, and harm reduction interventions over a 20-year time horizon. Outcomes included healthca…

Hidradenitis suppurativa, a rare skin disease.

INTRODUCTION: Accessible, culturally responsive, and high-quality harm reduction services, including needle and syringe programs and regional services, are essential for addressing disproportionate rates of blood-borne viruses (BBVs) and STIs among Aboriginal and Torres Strait Islander people who inject drugs. However, there is limited understanding of stakeholder perspectives to guide service design, and this is exacerbated by limited engagement with Aboriginal and Torres Strait Islander people who inject drugs. This project used the CONSolIDated critERia for Strengthening the Reporting of He…

Policy reform to shift the health and human rights environment for vulnerable groups: the case of Kyrgyzstan's Instruction 417.

BACKGROUND: Police activities shape behavior and health outcomes among drug users, sex workers, and other vulnerable groups. Interventions to change the policing of drug consumption and sex work in ways that facilitate public health programming and respect for human rights have included policy reforms, education, and litigation. In 2009, the Kyrgyz government promulgated "Instruction 417," prohibiting police interference with "harm reduction" programs, re-enforcing citizen rights, addressing police occupational safety concerns, and institutionalizing police-public health collaboration. Covariation of symptomatic and mechanistic change during dialectical behavior therapy for suicidal self-harming youth.

OBJECTIVE: The effectiveness of psychotherapy for suicidal youth remains a public health priority. Dialectical behavior therapy (DBT; Linehan, 1993) has been recognized as a well-established treatment for suicidal youth (Witt et al., 2021). Although promising, little work has explored mechanisms of change in DBT for suicidal youth (Asarnow et al., 2021). This study aimed to examine covariation in rates of change between symptoms and theorized mechanistic variables during a randomized controlled trial of DBT for suicidal youth. METHOD: The present study was a secondary data analysis of a rando

Should I Stay or Should I Go? A Qualitative Exploration of Stigma and Other Factors Influencing Opioid Agonist Treatment Journeys.

(1) The harm-reduction benefits of opioid agonist treatment (OAT) are well-established; however, the UK government's emphasis on "recovery" may be contributing to a high proportion of people leaving treatment and low retention rates. We wanted to develop a rich and nuanced understanding of the factors that might influence the treatment journeys of people who use OAT. (2) We explored factors at each level of the socioecological system and considered the ways these interact to influence treatment journeys in OAT. We carried out semi-structured interviews with people who use OAT (n = 12) and serv

The public health impacts of drug-induced homicide laws: a community-engaged scoping review.

BACKGROUND: Drug-induced homicide (DIH) laws are state laws that confer criminal liability equivalent to homicide for the sale or delivery of drugs subsequently associated with a fatal overdose. METHODS: We undertook a scoping review of literature on the public health impacts of DIH laws and held focus groups to seek guidance on the interpretation of review results from three directly-impacted community groups: public health and harm reduction professionals; people who use drugs (PWUD); and family members of adult victims of fatal overdose. RESULTS: Eight publications met our inclusion crite…

Latent class profiles of health-risk behaviors and the protective roles of psychological resilience and self-control in Chinese adolescents.

BACKGROUND: Health-risk behaviors (HRBs) frequently co-occur during adolescence and are associated with adverse physical and mental health outcomes. This study aimed to identify latent classes of HRBs and examine the associations of psychological resilience (PR) and self-control (SC) with class membership. METHODS: In this cross-sectional study conducted in 2023, 11,407 students from 90 schools were recruited using multistage stratified cluster sampling. Latent class analysis (LCA) was used to identify patterns of HRBs. Chi-square tests and multinomial logistic regression were used to examine

Willingness to use fentanyl test strips among street drug users in Puerto Rico: a cross-sectional study.

BACKGROUND: Fentanyl contamination in illicit drug markets has significantly increased overdose risk in Puerto Rico and other regions. Fentanyl test strips (FTS) have emerged as a promising harm reduction tool. While several studies in the United States have examined FTS willingness and acceptability, research in Puerto Rico remains scarce. This study offers one of the first large-scale epidemiological assessments of willingness to use FTS among street-recruited drug users on the island. METHODS: We conducted a cross-sectional study of 400 street drug users recruited from syringe exchange pro…

PrEP facilitators and barriers in substance use bridge clinics for women who engage in sex work and who use drugs.

BACKGROUND: Women who engage in sex work and use drugs (WSWUD) experience disproportionate HIV risks. Substance use treatment bridge clinics offer an opportunity to increase HIV pre-exposure prophylaxis (PrEP) delivery to WSWUD, but research on best practices is lacking. Therefore, we explored facilitators and barriers to PrEP across the PrEP care continuum in these settings. METHODS: Bridge clinic and affiliated harm reduction health service providers and WSWUD from Boston were recruited using passive and active outreach between December 2021 and August 2022. Participants were invited to tak…
